VETERANS ADMINISTRATION CMOP DALLAS

2962 S. LONGHORN DRIVE

LANCASTER, TEXAS 75134

STATEMENT OF WORK

1. LOCATION:

The Department of Veterans Affairs Dallas Consolidated Mail Outpatient Pharmacy (CMOP) located in Lancaster, Texas. CMOP utilizes advance automation to produce excess of 112 million prescription a year. CMOP utilizes integrated, automated dispensing system to be able to process high volume mail prescription workloads Address is 2962 South longhorn Dr, Lancaster, Tx 75134

2. FACILITY ENTRY PROTOCOL:

Every contractor employee while on site must check in with administration upon arrival, sign in and receive a contractor’s badge. In addition, they must adhere to all safety guidelines for preventive measures of COVID-19 if required at the time of visit. This will include daily temperature check, wear CDC approved face masks always while inside the building, apply hand sanitizer, etc. prior to gaining access into the building daily.

3. DESCRIPTION OF SERVICES/GENERAL INFORMATION

3.1 DESCRIPTION OF SERVICES

A contract is required to ensure that the two Mitsubishi robots receive required maintenance on a yearly basis. Yearly routine Maintenance work to be performed on two (2) Mitsubishi robots SCARA RH1FHR, RH2FH, RH3FH, RH3FHR, RH6FH, RH12FH, RH20FH and Vertical Articulated RV2F, RV4F, RV7F, RV7FLL, RV13F, RV20F (not RV35F, RV50F, RV70F) located at the VA-Dallas CMOP in Lancaster, TX.

The work described shall be performed by competent, qualified, and adequately trained personnel to perform services and will be familiar with the manufacturer’s service procedures on PMs. Services duration shall be base year plus four (4) pre-priced one (1)-year options for renewal.

4. Statement OF WORK: (Deliverable Product)

The contractor will perform preventive maintenance on two (2) Mitsubishi robots SCARA RH1FHR, RH2FH, RH3FH, RH3FHR, RH6FH, RH12FH, RH20FH and Vertical Articulated RV2F, RV4F, RV7F, RV7FLL, RV13F, RV20F (not RV35F, RV50F, RV70F) Contractor will follow manufactures recommended PM procedures for this model of robot equipment, using the attached PM Check sheet and manufactures manual.

Contractor shall document all work done and any additional concerns. Any issues with equipment will be documented in the inspection checklist provided at the bottom of the SOW and the COR shall be notified immediately of any critical issues with equipment immediately upon discovery during the inspection. Critical issue is defined as an issue in the technician’s opinion can cause immediate failure of the equipment if not addressed immediately, see section 5.1.3 for further details. PMs work shall be done while production is not working, this will entail Saturday/Sunday work schedules. In case of emergency callouts, work schedule could be from Monday through Saturday 7:00am through 12:00 midnight.

4.1 The annual inspection and service will be completed within 90 days of contract award and exercising of option years. The Option year PM but must be completed one

(1) year after the first PM is completed. Contractor will have a thirty (30) day grace period to schedule the PM and complete the service at the one-year anniversary and subsequent option years from the date of the first PM. Any deviations to this schedule must be cleared with the CO with COR concurrence for scheduling.

. 4.2 Service shall include labor, parts and supplies required for PM service.

Contractor shall inspect equipment and perform preventive maintenance on the equipment. NOTE: for the initial base year, both robots shall have both timing belts replaced for a total of four (4) belts two per robot as well as all batteries. The belts for the option years will be inspected and adjusted only unless deemed worn and in need of replacement. If a critical issue is discovered during any inspection the COR will be immediately notified of the issue to include the scope and severity the issue poses to the proper functioning of the identified equipment. The contractor can submit a quote for repairs to include materials, labor hours and timelines for completion to be funded as needed on the miscellaneous repairs CLIN. All proposals for repair must be approved by the CO before any repairs are started.

4.3 Emergency Call outs, The contractor will be in contact with COR within 4 hours of notification from COR that emergency repairs need to be conducted. If a technician is not available for emergency repairs within 24 of notification the vendor shall report this to the COR as soon as possible. Contractor will do an assessment and give an estimate quote for repairs to the COR on needed repairs before repairs are made.

Working hours for emergency callouts are, Monday through Saturday 7:00am through 12:00 midnight. Vendor will provide an hourly rate for emergency repairs with funding and work to commence after notice to proceed is given from the Contracting Officer.

4.4 Service tickets shall be provided for each piece of equipment and include Make, Model, and Serial Number of equipment. Service ticket shall include the description of work performed. For corrective maintenance/repairs, the scope of work shall be outlined on the ticket, including a list of any parts replaced. Service ticket shall include any deficiencies noted during the conduct of preventative maintenance and indicate whether the equipment is safe to operate.

5. Documentation:

• All service documentation must contain:

• Date and time of the contractors’ arrival on station

• Type, model, and serial number(s) of all equipment on which service was performed to include reported problem and cause of problem (when applicable)

• Completes list of parts.

• Date and time equipment was returned to serviceability

6. Property Damage:

The contractor shall take all necessary precautions to prevent damage to any government property and will notify the Contracting Officer Representative immediately if damages occur. The COR will notify the Contracting Officer who will authorize the Contractor to remedy the situation in one of the following ways; assessment for current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no addition cost.

7. Noncontract Charges:

The Contractor shall not perform any service that will result in additional charges without prior approval for the Contracting Officer.

8. SERVICE DELAYS:

If for any reason the contractor is unable to perform on the project, the contractor shall immediately notify Contracting Officer, COR and the Dallas CMOP Engineering office, in writing, and provide the reason why service cannot be made (e.g., awaiting parts), and an estimated completion date.

Dallas CMOP is required to be fully operational during normal working hours, if the contractor runs into issues that will prevent the facility from operating the contractor shall provide all services to assure that the facility has power to operate normally.

9. Schedule of Services:

The contractor shall develop, implement, and maintain a schedule of services. The contractor shall submit a copy of the schedule to the Contracting Officer, COR and Dallas CMOP engineering office for review, no later than ten days prior to commencement of PM work.

10. Work Hours:

The contractor will have access to Dallas CMOP, from 7:00 a.m. – 2:30 p.m. Saturday through Sunday for the PMs, for emergency callouts, the building will be open Monday through Saturday, from 7:00 am until Midnight. excluding Federal Holidays, if the contractor wants to work outside of these days/hours; the contractor shall submit the request in writing to Contracting Officer, COR and the Dallas CMOP Engineering office at least one week in advance. The contractor is advised that requests are subject to disapproval.

11. General Information

11.1 Qualifications:

The contractor shall provide sufficient personnel who meet all contract requirements and who are competent, qualified, and adequately trained to perform services required in this SOW.

11.2 Warranty:

The contractor shall warranty all parts, equipment, and workmanship for a period of one year from the date of Government acceptance of work.

11.3 Waste Disposal:

The contractor shall dispose of all waste and excess materials from the job site. The contractor shall not dispose of any waste at Dallas CMOP or leave waste at the site overnight.

11.4 Work Areas:

The contractor shall, always, keep work areas free from accumulations of waste material, rubbish, tools, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition.

11.5 Safety:

The contractor shall comply with all local, state and federal laws and safety regulations.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor’s employees is exclusively the contractor’s obligation, and the Government will assume no liability or responsibility for the contractor’s compliance or non-compliance with such responsibilities. The contractors shall provide contractor employees with all necessary personal protective clothing and equipment.

11.6 Fire Prevention and Safety:

Contractor’s employees shall comply with the Occupational Safety and Health Act (OSHA), VA CMOP Fire Protection Program and the Fire Prevention and Health Program.

11.7 Federal Holidays:

The following Federal holidays are observed at Dallas CMOP. The contractor will not perform services on Federal holidays, or the actual day set aside for observation.

11.8 Contractor Employee Conduct:

The Government reserves the right to require removal from the site any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of VA CMOP security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.

11.9 Smoking In VA Facilities:

Contractors are advised that the Government has placed restrictions on the smoking of tobacco products in Government facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.

11.10 Illegal Substances:

Illegal substance possession, use, or distribution will not be tolerated on any Government installation, including VA Dallas CMOP. This includes prescribed medical marijuana. The contractor shall ensure that contractor employees who have access do not possess, use, or distribute illegal substances. All contractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation and subject to arrest and prosecution by local authorities.

11.11 Security Requirements:

The contractor shall obtain their security badges through Security located in the front office. Contractor’s personnel and all sub-contractors shall abide by all Dallas CMOP security requirements and regulations.

11.12 Weapons, Firearms, and Ammunition:

Contractor’s employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or on Government property while at VA Dallas CMOP.

12. BILLING and INVOICING:

12.1 Contractor must be registered in the Federal Government’s System for Award Management (SAM) in order to be awarded a contract with the U.S. Government.

The Federal Government utilizes a third-party contractor to process invoices.

Contractor must be registered with OB10 in order to submit e-invoices. The Federal Government DOES NOT accept, nor can they physically process paper invoices. Paper invoices WILL NOT be paid. Contractors may register in OB10 at:

13. QUALITY CONTROL/ASSURANCE

13.1 The contractor will be responsible for the quality control of all work performed. All work must be performed in accordance with industry standards, manufactures specifications, installation instructions/recommendations, related documents provided and in compliance with all applicable codes and regulations. NEC, IBC, State, County, and local authority. Perform all required tests on all work as specified herein and in accordance with code requirements and industry standards to ensure full and complete operation of all systems.

13.2 Performance Monitoring

Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract.

13.3 Direct Observation-(Can be performed periodically or through 100% surveillance.) The COR does not have the authority to modify or approve any alterations to the SOW or contract. Any changes or issues must be addressed through the Contracting Officer.

13.4 ACCEPTABLE Performance

The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action.

13.5 UNACCEPTABLE performance

When unacceptable performance occurs, the COR shall inform the contracting Officer.

This will normally be in writing unless circumstances necessitate verbal communication.

In any case the COR shall document the discussion and place it in the COR file.

When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR) and present it to the contractor's program manager for review before submission to the Contracting Officer.

The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR and Contracting Officer. The Government shall review the contractor's corrective action plan to determine acceptability.

Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO.

16. INSPECTIONS

16.1 The Government reserves the right to perform initial, follow-up, and final inspections and acceptance of all work. The Dallas CMOP engineering office shall inspect and accept all work and workmanship.
