STATEMENT OF WORK (SOW)

FOR

HUSH HOUSE SITE SURVEY

BLDG 1208/3008

MISAWA AIR BASE, JAPAN

1. OBJECTIVE

The purpose of this Statement of Work is to produce a report containing the inspection and diagnosis of the Misawa Air Base Hush Houses, Buildings 1208 and 3008. This report shall be used in future efforts to repair these Hush Houses, and the report shall not include any proposed solutions for any issues identified in the report.

2. SCOPE

The contractor shall provide an inspection plan that incorporates a forecasted schedule with the proper personnel and equipment needed for each phase of this site survey.

The contractor shall provide all manpower and equipment necessary to diagnose any issues identified by the contractor in the Hush Houses to the extent possible without degrading operational capacity of either Hush Houses. Issues that are identified that require further diagnosis beyond the extent described in the sentence prior, shall be annotated by the contractor in the report using “INCOMPLETE DIAGNOSIS” to indicate that the issue could not be fully diagnosed. Incomplete diagnosis items shall contain as much information as the contractor is able to provide given the limitations of this SOW.

All issues shall include pictures of the damage that clearly show any external damage and, if possible, any internal damage, the diagnosis of the issue, and a diagram of where the issue is located. The contractor may use one “Master Diagram” for each building to display all issues as long as the location of each issue is clearly marked and can be found using that diagram alone. Multiple diagrams of subsections of each building may be required if a Master Diagram is unable to meet this requirement.

In regards to identifying issues, the standard for this SOW is that if an issue can be identified upon close inspection without additional equipment, is causing any components of the Hush House to not operate at an optimal capacity, or presents a structural weakness for any portion of the Hush House, the contractor shall identify that issue in the report.

3.REQUIREMENTS

The contractor shall Coordinate with Propulsion Flight leadership and/or Engine Test Cell leadership for access to the buildings during work hours: Monday through Friday 0700-2300.

Excluding federal holidays and PACAF family days. These inspections shall be required to be coordinated at times when engine testing is not being performed.

The contractor is responsible for obtaining identification cards, base passes, and/or flightline Entry Authority List (EAL) for all employees and vehicles utilized during the survey. Upon termination of employment, identification cards and passes shall be returned to Security Forces within 24 hours.

The Hush Houses, buildings 1208 and 3008 are located inside the Entry Control Point to the flightline and the contractor will need an escort to accompany your team through the gate if EAL passes are not obtained.

The contractor shall Develop a Work Breakdown Structure (WBS) for the manning, estimated shift times, and labor costs for each day to perform the tasks.

The contractor shall Provide effective and qualified personnel to accomplish the tasks according to the inspection plan, at minimum personnel must be proficient in structural, electrical and plumbing.

The contractor shall provide a project manager who can read, write, speak and understand English to discuss any contract related matters with the leadership team.

The contractor shall secure all tools and equipment, along with leaving the site each day in a clean and orderly condition, a lay dawn area will be provided in the Hush house being inspected and building 3008 will serve as an auxiliary location if needed.

The final report shall include all issues identified per paragraphs 2 and 4 of this SOW. All findings shall be ordered in sequence by location and all information, pictures, and diagrams, if necessary, shall be displayed with each issue prior to moving on to the next issue

The contractor shall provide a detailed milstone schedule for inspection flow. Schedule provide below is sole for example purposes.

4. SCOPE OF INSPECTION

The following items are specific items that are expected to be identified in the final report. Keep in mind the standard for issue identification outlined in paragraph 2 of the SOW shall take precedence.

Main Building Structure

• Foundation. Inspect for missing material, cracks, breakage or loose fragments, and overall security of the structure.

• Exterior Structures. Inspect all sidewall and roofing structures for missing material, cracking, breakage or loose fragments, security and integrity of support beams and attaching hardware. See Figure 2 for layout reference.

• Piping Systems. Inspect all water, fuel, air, and electrical support piping for corrosion, missing material, security of welds and attaching hardware, leakage around joints and valves, flanges for loose or worn gaskets, and overall integrity of piping structures.

• Lighting Structures. Inspect all external and internal lighting for functionality and effectiveness, cracking, missing material, and overall security of attaching hardware.

• Overall facility structure for integrity/longevity of the building.

Misc Interior Rooms

• Indoor Ventilating Fans and Air Conditioner. Inspect for proper operation and air flow.

• Check for corrosion, breakage, dew condensation of the units or ducting, broken insulation, abnormal sound during operation.

• Generator Room/Electric Room

• Generators. Inspect generators for serviceable operating conditions looking for abnormal indications and readings, and abnormal sounds during operation.

• Electrical Panels. Check electrical panels for missing or broken levers, deteriorated or burned wires, and security of attaching hardware for doors, etc.

• Air Compressor Unit. Inspect for corrosion, breakage, leaks, and proper operation of valves and unit.

Check electrical parts for burn damage, disconnection, and deterioration.

Engine Test Bay

• Concrete floor, wall, and ceiling surfaces. Check for cracks, breakage or any loose fragments. Check floor heater functionality.

• Sound Proof Door (Hangar Door). Check gaskets, hinges, handles, lock pins, etc., for corrosion, breakage, peeled paint, opening and closing performance.

• Inlet screen/splitter inside suction tower. Check for security and serviceability of screen and hardware. Check splitter structures, handrails, and surfaces for cracks, corrosion, and general damage.

• Sound absorption panels. Inspect for cracks, damage in steel frames, loosening and breakage of bolts, nuts, rivets, bundle wires, etc.

• Exhaust Sliding Doors: Inspect for cracks, corrosion, missing material, and smooth proper operation.

• Sound Insulating Door: Inspect for missing material, cracks, corrosion, peeled paint, bottom door seals, and abnormal sounds during operation.

Exhaust Room(Fig 1.)

• Floor Surface: Inspect for cracking, chipping, deterioration and flaws

• Steel Stairs: Inspect for missing material, cracks, corrosion, security of welds and hardware.

• Guide Vanes: Inspect for missing material, cracks, corrosion, security of welds and

• hardware. Inspect wall inboard of guide vanes for missing material, cracks, corrosion, security of handles and hardware on access covers.

• Secondary Air Splitter: Inspect for missing material, cracks, corrosion, security of rivets and hardware.

• Secondary Air Passage: Inspect guide vanes and stairs for missing material, cracks, corrosion, security of hardware.

• Soundproof Panels: Inspect for material dropout, security of rivets and hardware, cracks, corrosion, dents, and general damage.

• Air Deflector Rack: Inspect for missing material, cracks corrosion, security of welds and hardware.

• Exhaust Silencer (vertical stack): Inspect support beams for missing material, cracks, corrosion, security of welds and hardware. Check Soundproof paneling for material dropout, cracks, corrosion, security of hardware, dents, and general damage

• Exhaust Silencer (Splitter): Inspect all areas for missing material, cracks, corrosion, dents, general damage, and security of hardware.

5. GENERAL INFORMATION:

5.1. HOURS OF OPERATION.

The hours of operation for custodial cleaning tasks are shown in the remark’s column on Appendix A, Estimated Workload Data.

5.2. FEDERAL HOLIDAYS.

5.2.1. If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the contractor will be responsible for rescheduling services for the first day past the holiday observance.

5.3. BASE CLOSURES.

Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

5.4. SPECIAL SECURITY REQUIREMNTS.

The contractor shall comply with all Misawa AB security requirements. The government will provide escorts for the contractor, as required.

5.5. RANDOM ANTITERRORISM MEASURES (RAMS).

RAMS are conducted on Misawa Air Base at varying times a 24-hour period and may consist of measures such as spot checks of identification credentials, random vehicle searches, and site security sweeps. The contractor is subject to participating and cooperating with RAMS as directed by competent authority.

Anticipate increased RAMS during implementation of elevated Force Protection Conditions (FPCONs).

5.6. REPORTING SUSPICIOUS ACTIVITY.

It is the responsibility of all personnel working and residing on Misawa Air Base to be alert for detecting and reporting suspicious criminal or terrorist activity. The contractor shall immediately report any on-base observed suspicious activity to the 35 SFS/BDOC, Bldg. 646. All contractor- employed personnel shall be familiar with suspicious activity reporting numbers and procedures. The primary number for reporting suspicious activity on Misawa Air Base is 911 from a base phone or 0176-53-1911 from a mobile phone or off-base.

5.7. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY

OR OVERSEAS COMBATANT COMMANDER.

This service is not determined to be essential for performance during crisis as declared by the National Command Authority or Overseas Combatant Commander. In the event of crisis, the contractor will be notified by the CO of the need to discontinue services due to base closure or other causes. If notification to discontinue services is not received, the contractor will be expected to continue performance in accordance with this PWS.

5.8. LELVEL I ANTITERRORISM AWARENESS TRAINING FOR CONTRACTORS (JAN 2023)

(a) Definition. As used in this clause—

“Military installation” means a base, camp, post, station, yard, center, or other activity under the jurisdiction of the Secretary of a military department or, in the case of an activity in a foreign country, under the operational control of the Secretary of a military department or the Secretary of Defense (see 10 U.S.C.

2801(c)(4)).

(b) Training. Contractor personnel who require routine physical access to a Federally- controlled facility or military installation shall complete Level I antiterrorism awareness training within 30 days of requiring access and annually thereafter. In accordance with Department of Defense Instruction O-2000.16 Volume 1, DoD Antiterrorism (AT)

Program Implementation: DoD AT Standards, Level I antiterrorism awareness training shall be completed—

(1) Through a DoD-sponsored and certified computer or web-based distance learning instruction for Level I antiterrorism awareness; or

(2) Under the instruction of a Level I antiterrorism awareness instructor

(c) Additional information. Information and guidance pertaining to DoD antiterrorism awareness training is available at https://jko.jten.mil/ or as otherwise identified in the performance work statement.

(d) Subcontracts. The Contractor shall include the substance of this clause, including this paragraph (d), in subcontracts, including subcontracts for commercial products and commercial services, when subcontractor performance requires routine physical access to a Federally-controlled facility or military installation.

5.9. CONTRACTOR/EMPLOYEE BASE PASS AND IDENTIFICATION, SPECIAL CLEARANCES AND VEHICLE

PASSES.

The contractor shall obtain the following pass and identification items:

A. Prior to the start of on-site work, the contractor shall submit applications for base passes (listed below) to the Contracting Squadron, 35 CONS/PKB, Bldg. 656, for all employees, including subcontractor employees requiring access to Misawa Air Base in order to perform under this contract.

(1) 5 AF Form 99, Application for Base Pass, for employees.

B. Upon completion of this contract, the contractor shall return all employee passes to the Visitor Control Section, 35 SFS/S-5, Bldg. 301, and obtain a Certificate of Receipt. Final payment will be withheld until the contractor has accomplished required clearances.

5.11. CONTRACTOR EQUIPMENT.

5.11.1. Vehicles.

The contractor shall operate vehicles to comply with Japanese and base traffic regulations. All contractor vehicles shall comply with Japan Licensing, Registration, and Vehicle Safety Inspection requirements.

5.11.1.1. Required Insurance.

When contractor’s owned vehicles are being operated on Misawa Air Base,

(a) The contractor shall procure and maintain during the entire period of his/her performance under this contract, the following minimum vehicle insurance:

Property Damage – ¥2,000,000

(b) Prior to the commencement of work hereunder, the contractor shall furnish to the CO a certificate or written statement of the above required insurance. The contractor is required to notify the CO immediately upon cancellation or of any material change in the policies adversely affecting the interests of the government.

c) The contractor agrees to insert the substance of this clause including this paragraph c, in all subcontracts hereunder.

5.12. MISAWA AIR BASE FIRE PREVENTION.

The contractor shall be responsible for fire prevention in all contracted area and be free from any potential fire hazards. The contractor shall comply with the following fire prevention guidance.

a. Fire Reporting:

1) The primary emergency reporting number on Misawa Air Base is 911 or 0176-53- 1911 from off base. Fire Department operations take place out of Bldg. 947 and Bldg. 3003.

2) On-base telephones in the warehouse(s) shall have “911” stickers.

3) All contractor-employed personnel shall be familiar with fire reporting numbers and procedures, and shall to put the number “0176-53-1911” into his her mobile phone.

b. Vehicles:

1) Access for fire fighting vehicles must be maintained at all working sites.

2) Contractor’s vehicles shall not be parked within 15 feet of any fire hydrant of Fire Department connection.

3) Contractor’s vehicles shall give the right-of-way to responding fire/emergency vehicles.

4) If contractor use the truck, it shall carry a fire extinguisher (1 litter or more) for the performance of this contract.

c. Electrical Hazards:

1) All of the contractor’s electrical tools and equipment shall be in good (original) working condition and its maintenance shall be done properly in compliance with manufacturer’s maintenance instructions.

2) All extension cords used shall be equipped with circuit breakers and plugged directly in the outlets (not other extension cords).

3) Building utility panels shall not be blocked by contractor tools/supplies.

d. Storage:

1) Flammable liquids or any other safety hazards shall not be stored inside the warehouses next to the buildings or near exit ways.

2) Fire protection system shall not be obstructed at any time. Carpet storage shall be neat in a manner which does not present fire/safety hazards and not to block the access to where fire-extinguishers are mounted.

5.13. CONTROLLED UNCLASSFIED INFORMATION (CUI)

The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding CUI material.

5.14. REPORTING REQUIREMENTS.

Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information.

5.15. PHYSICAL SECURITY.

The contractor shall be responsible for safeguarding all government property and controlling forms provided for contractor use. At the end of each work, all government facilities, equipment, and materials shall be secured.

5.16. CONTRACTOR PERSONNEL.

Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and the personal protection.

The government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general wellbeing, or operational mission of the installation and its population.

5.17. Safety Requirements.

In performing work under this contract, the contractor shall:

A. Comply with all applicable Misawa AB Directives, USFJ and DoD Instructions, OSHA standards, and the General Duty Clause of the OSH Act.

B. Record and report promptly (within one hour) to the contracting officer, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

C. Take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

D. Cooperate fully with the investigation and assist government officials until the investigation is completed.

6. APPENDICES:

The government will make all publications, forms, references and report formats listed available. Publications can be accessed on-line at http://www.e-publishing.af.mil/. Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract. The contractor shall immediately implement those changes in publications, which result in a decrease, or no change in the contract price. Prior to implementing any such revision, supplement, or amendment that will result in an increase in contract price, the contractor shall submit to the CO a price proposal and obtain prior approval.

Price proposals shall be submitted within 15 calendar days from the date the contractor receives notice of the revision, supplement, or amendment giving rise to the increase in cost of performance. Changes in the contract price due to supplements and amendments shall be considered under the FAR 52.212-4, Contract Terms and Conditions - Commercial Items clause. Failure of the contractor to submit a price proposal, shall entitle the government to performance in accordance with such change, at no increase in contract price.
