STATEMENT OF OBJECTIVE (SOO) FOR:
Sidewalk and Ramp Concrete Replacement

Sidewalk and Ramp Concrete Replacement Purpose The purpose for the concrete replacement project is to provide an even concrete sidewalk at the FRC building’s east side and under the east awning, repairs/replacement of loading dock at shipping receiving bay and replacement of the ramp at the east shed. These existing areas will need to be replaced with concrete equivalent to or better than the existing sidewalk or ramps (See photo on page 6). The existing concrete is damaged and is a tripping hazard. The east shed ramp is too steep to use a forklift or pallet jack.
Scope The Scope of this effort includes removing damaged concrete, and installing new concrete, disposition of all debris waste, and installation of a new sidewalk, shipping/receiving ramp, and east shed ramp using concrete as needed for the concrete replacement project. The contractor is responsible for forming and using appropriate rebar size, on appropriate centers, pour of concrete thick enough for pedestrians on the sidewalk, forklifts on the ramps and use appropriate PSI to avoid degrading of the surface with a broom finish.
Period of Performance The period of performance for this task order will be 30 days from date of award for one (1) base period.
Place of Performance The place of performance is FEMA Region 6, 800 N. Loop 288, Denton, Texas 76209 Background The Federal Regional Center (FRC) is the Headquarters office building for Region 6. This is a complex that houses multiple buildings, offices, and parking areas. The current sidewalk is showing signs of wear and tear and has several patches that need to be replaced. The Shipping/Receiving ramp is worn out, uneven and cracked. And the East Shed ramp is way too steep to use a forklift or a pallet jack. Throughout the years, we have performed some repairs around the area which required resurfacing. The constant patching of the concrete created an uneven surface which is affecting on-foot traffic and has become a safety issue.
Performance of Objectives Design Review
a. The purpose of the review is to validate constructability of the conceptual design and ensure it will meet FEMA’s needs, will comply with local, state and ADA building and concrete regulations. The design will be reviewed by the COR and on-site Contractor supervisor prior to starting demolition.
Demolition: Demolish blocks of damaged sidewalk on east side of FRC building, shipping/receiving ramp and east shed ramp (see attachment 1).
b. All demolition/waste materials, because of this project, are the sole responsibility/ownership of the selected contractor and must be removed from the job site upon completion. Final site conditions shall be left in as good or better condition as when work commences. All waste material/debris will be disposed of. The preferred method is to divert from landfill and recycle/reuse.
Installation of new concrete at all areas that were demolished.
c. The sidewalk and two ramps will be built – e.g., concrete pouring, compacting and finish, Sub-base, and base to be appropriately designed in accordance local, state and/or Federal regulations/guidelines, International Building Code (IBC).
Inspection at project completion
d. The selected contractor is responsible for obtaining all necessary permits and inspections required by local, state, or federal offices. Upon project completion, the FEMA COR is responsible to approve completion of project.
Operating Constraints Time constraints and unforeseen weather delays. Any activation of our Regional Response Coordination Center will increase, foot traffic may complicate the project.
Safety This section is applicable to all work covered by this contract. Contractor employees working onsite must follow all Federal agency workplace safety protocols.
1. Safety Meeting
2. Representatives of the Contractor shall meet with the Contracting Officer and representative(s) prior to the start of work under this contract for the purpose of reviewing the Contractor's safety and health programs and discussing implementation of all safety and health provisions pertinent to the work to be performed under the contract. The Contractor shall be prepared to discuss in detail the measures he/she intends to take to control any unsafe or unhealthy conditions associated with the work to be performed under the contract. If directed by the Contracting Officer, this meeting may be held in conjunction with other meetings which are scheduled to take place prior to start work under this contract.
3. Safety Submittals
a. Accident Reporting: A copy of each accident report, which the Contractor or subcontractors submit to their insurance carriers, shall be forwarded through the Contractor Supervisor to the Contracting Officer Representative as soon as possible, but in no event later than seven (7) calendar days after the day the accident occurred.
b. Site Safety Plan: Contractor to provide and submit prior to start of demolition
c. Contractor Liability: The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of work and shall hold the Government harmless for any action on his/her part or that of his/her employees or subcontractors, which results in illness, injury, or death.
Protection
a. The Contractor shall take all necessary precautions to prevent injury to the public, building occupants, or damage to property of others. To this contract, the public or building occupants shall include all persons not employed by the Contractor or a subcontractor working under his/her direction.
b. Storing, positioning or use of equipment, tools, materials, scraps, and trash in a manner likely to present a hazard to the public or building occupants by its accidental shifting, ignition, or other hazardous qualities is prohibited.
c. Wherever practicable, the work area shall be fenced, barricaded, or otherwise blocked off from the public or building occupants to prevent unauthorized entry into the work area.
d. When work is to be performed over a public thoroughfare such as a sidewalk, lobby, or corridor, the thoroughfare shall be closed, if possible, or other precautions taken such as the installation of screens or barricades. When the exposure to heaving falling objects exists, as during the erection of building walls or during demolition, special protection of the type detailed in 29 CFR 1910/1926 shall be provided.
e. Fences and barricades shall be removed upon completion of the project, in accordance with local ordinance and to the satisfaction of the Contracting Officer or his/her representative(s).
Security General Requirements
This section includes all items related to security clearances, security badge passes, security guards and escorts.
1. Security Requirements and Regulations
4. Clearance checks will be performed by the Government.
1. Contractor shall provide Contracting Officer information of all staffers that will need to access to the site to include First/Last name, Birthdate, and last four of SSN for everyone accessing federal facilities and be cleared by the NCIC. Contractors must submit all personal information one week prior to start of project.
4. Permission to enter the site is revocable at any time during the project by the Regional Security Officer.
4. Contractors are not to direct sales representatives, individuals seeking employment or others to the project site.
4. Temporary Security Badges serve to authorize the individual issued the badge access to the federal property with an escort.
4. Temporary Security Badges shall be always clearly visible when on the work site.
4. Do not enter the building without security badges.
4. If the clearance is revoked for any reason, the security badge shall be returned to the Regional Security Officer immediately.
4. Temporary badges will be issued by the Regional 6 Security.
