About this file

This is a Past Performance Questionnaire template used by the Federal Bureau of Investigation (FBI) Procurement Team to evaluate contractor performance on previous relevant contracts or projects.

The questionnaire is structured in six sections. Sections I and II are completed by the offeror and provided to the assessor (government or commercial personnel with experience on the contractor's previous contract), who verifies the information's accuracy. Section I requires identification of the customer/agency name, description, and geographic scope of services (single location, multiple locations, CONUS, or OCONUS). Section II collects current or historical contract identification details including contractor name, address, UEI, contract number, email, contract type, periods of performance, initial and current/final contract values, reasons for value differences, brief service descriptions, and contractor role (prime or subcontractor). Section III captures the assessor's information: name, title, phone/email, role (PM, PCO, ACO, Contract Specialist, COR, Quality Assurance Specialist, etc.), and years of experience on the contract.

Sections IV through VI are completed entirely by the assessor. Section IV provides standardized rating definitions: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N). Section V requires performance evaluations across multiple categories: Program Management (overall program management, management responsiveness, management of personnel); Technical Performance (quality of technical approach, understanding of requirements); Timeliness of Performance; Quality/Customer Satisfaction (quality of performance/customer satisfaction, business behavior, communication); and Cost Control. An overall rating is also required. Section VI provides space for general comments and requires the assessor's signature and date. Completed questionnaires must be submitted as PDFs to swalker4@fbi.gov and NRYUILLE@fbi.gov using the naming convention "Vol_1_PPQ_Company Name."
