1. PURPOSE
1.1. The purpose of this performance based non-personal services contract is to provide, Roman Catholic Priest services for the 92 ARW, Fairchild AFB, WA.
2. SCOPE
2.1. The contractor shall provide religious and spiritual leadership to meet the needs of
Catholic military personnel, their family members, and other authorized personnel at 92 ARW, Fairchild AFB will support individual rights under the First Amendment to the US Constitution and provide an opportunity for the free exercise of their religious beliefs, as outlined in Appendix A, within the 92 ARW area of responsibility which includes Fairchild AFB.
3. DESCRIPTION OF SERVICES
3.1. The contractor shall provide services for the Department of Air Force (DAF) 92 ARW
Installation Ministries Roman Catholic support requirements. The contractor shall coordinate efforts with Chaplain Corps personnel and work in a multi-denominational and pluralistic religious environment to include, but not limited to, Catholic, Protestant, Jewish, Orthodox, and Islamic faiths. The contractor shall provide services in accordance with (IAW) the Roman Catholic Archdiocese for the Military Services (AMS) faith requirements. This contract also covers emergency ministry services. Emergency ministry services are defined as: Last rights and after hour counseling.
3.2. DAILY SCHEDULE. The contract is for a 40-hour work week per the attached monthly schedule. The contractor will often be required to work weekends and on holidays to complete Catholic religious support services for Fairchild AFB. The nature of this contract may require contractor to provide some services outside of normal duty hours and in excess of the hours of a normal duty day. Contractor shall communicate these excess hours to the Contracting Officer Representative (COR) within 24 hours of the event. The contractor shall be time-compensated for overtime hours for pre-determined contingencies, for other ministries approved by the COR, and or for emergency ministry services/requirements.
3.2.1. Contractor shall submit electronic after-action reports for emergency ministry requirements within 72 hours to the COR’s email address.
3.2.2. Contractor shall complete the monthly access user file for Air Force Chaplain Corps Activity Reporting System (AFCCARS) within 1 business day after the services have been provided. The AFCCARS file shall include daily entries within 1 business day of performed services. In the event the AFCCARS system is down, the contractor shall provide all updates within 1 business day after the system is operational again.
3.3. MONTHLY SCHEDULE. The COR shall provide the contractor a monthly written schedule showing items of service for that month. This schedule shall be given to the contractor at least fifteen (15) calendar days prior to the month the schedule is to begin. Additionally, the contractor may work on an on-call basis for emergencies as pre-determined by the COR, who approves the additional work in a time compensatory manner at the end of each month. When an unforeseen facility closure, inclement weather, holiday, family day, installation “down day” or other event is deemed to be an acceptable delay (or absence) by the COR, which precludes completion of services on a regularly scheduled workday (or is rendered redundant), the contractor shall reschedule the work (if required by the government) to be performed and notify

the COR of the new time.
3.4. PERIODIC PROGRESS MEETINGS. Quarterly meetings shall be established after contract award. The COR and the contractor shall periodically meet to discuss the contractor’s performance. At a minimum, the following issues shall be discussed:
opportunities to improve the contract deliverables; any modifications required to the contract;
unsatisfactory inspections and trends regarding each performance objective observed, and positive performance and steps taken by the contractor to prevent unsatisfactory occurrences in the future. The COR shall provide a summation of unsatisfactory inspections, customer complaints and provide insight into any identified trends. The minutes of these meetings will be recorded and signed by the COR and distributed to the Contracting Officer (CO), other applicable government personnel and the contractor. Should the contractor not concur with the minutes, the contractor shall provide a written notification to the CO identifying areas of non-concurrence for resolution within five (5) business days.
4. DESCRIPTION OF SPECIFIC SERVICES
4.1. QUALIFICATION REQUIREMENTS
4.1.1. Contractor shall be a Roman Catholic Priest in good standing with his home diocese and the Archdiocese for the Military Services (AMS) and have a completed DD Form 2088, Statement of Ecclesiastical Endorsement, with faculties provided for any sub-contracted clergy. Final approval and award of this contract is contingent on endorsement of contractor and any subcontractors by AMS and receipt of the aforementioned documents. If the endorsement is lost after award of the contract, a stop-work will be issued, and the contractor shall have 30 calendar days to regain it. During a stop-work, the contractor will not be paid as services will not be provided.
4.1.2. Roman Catholic clergy who are contractors are not chaplains and will not use the duty title of “chaplain”. The contractor shall not use the duty title of “chaplain” while performing services, to include but not limited to printed documents and electronic communications. The contractor shall be referred to as “Father” on all printed documents and electronic communications.
4.1.3. The contractor must meet the same educational and experience requirements outlined in DoDI 1304.28 The Appointment and Service of Chaplain.
4.1.4. Reserved
4.1.5. English is the operational language of the Air Force. The contractor shall be able to read, write, speak and understand English fluently. English shall be the operative language used with regard to this contract for written correspondence, discussions and other business transactions. Other languages may be used for pastoral support when more relevant to parishioners receiving individual pastoral care. Use of other languages shall not be to the disadvantage of the English-speaking community.
4.1.6. IAW DODI 1402.05 Background Checks on Individuals in DOD Child Care Services Programs E3.2.c., each individual working with children under the age of 18 in conjunction with this contract shall successfully complete a Tier 1 background check. The results of a Federal Bureau of Investigation (FBI) advanced fingerprint check must be obtained prior to a person’s employment under Line of Site Supervision (LOSS). Additionally, IAW DODI 1402.5, Background Checks on Individuals in DOD Child Care Services Programs, E4.f., all individuals working under LOSS must wear conspicuous identification such as distinctive

clothing, badges, wristbands, etc. that clearly identify the allowed scope of involvement with children. Additionally, posters or other printed information must be readily visible informing children’s guardians of what the conspicuous identification means. Contractor shall satisfactorily complete, maintain, and present compliance of the criminal history background check requirements listed above. The background check process must be initiated by the contractor and the chapel staff no later than thirty days after the award of the contract. In addition, child protection policies of the Catholic Church within the priest’s local diocese are prerequisites.
4.1.7. Annual Responsibilities to Maintain Currency Required For Contract
4.1.7.1. Contractor shall maintain dialogue with his Ordinary or Religious Superior and provide copies of his quarterly reports to the AMS.
4.1.7.1.1. Contractor may be required to complete bi-annual credentialing (Convocation) with the AMS, with reimbursement on cost basis not to exceed Joint Travel Regulations (JTR) reimbursement rates. Travel is contingent on government approval/availability of funds. Contractor shall be given up to seven days consecutive days approved absence (or alternate duty location), to include travel days, to attend AMS mandatory training Convocations.
4.1.7.1.2 Contractor, per AMS and Catholic Church requirements, is to participate in a Canonical Spiritual Retreat annually, no less than 5 and generally 7 consecutive days to include travel days. Such time is considered an approved absence (or alternate duty location). Travel is contingent on government approval/availability of funds and in accordance with Joint Travel Regulation (JTR).
4.1.7.2. Chapel Staff shall annually provide training which includes, but is not limited to:
a. Privileged communication training IAW AFI 52-101, Chaplain – Planning and Organizing.
b. IAW DODI 1402.5, Background Checks on Individuals in DOD Child Care Services Programs.
c. IAW AFI 40-301, Family Advocacy Program.
d.IAW DAFPD90-60, Sexual Assault Prevention and Response (SAPR) Program.
4.2. SPECIFIC RESPONSIBILITIES
4.2.1. Core Elements for Catholic Personnel Required For A Catholic Program
4.2.1.1. Contractor personnel given permission/faculties by the AMS to serve Catholic personnel are to ensure implementation and support of these elements in the interest of Free Exercise of Religion:
4.2.1.1.1. Sunday, Holy Day and daily Mass.
4.2.1.1.2. Comprehensive Religious Education and Sacramental preparation, with an emphasis on character formation, individual moral development, and military family cohesion and readiness.
4.2.1.1.3. Comprehensive Sacramental Ministry. This includes the sacraments of Baptism, First Eucharist, Confirmation, Penance, Marriage and the Sacrament of the Sick.
4.2.1.1.4. Rite of Christian Initiation for Adults (RCIA).
4.2.1.1.5. Young Adult Ministry focused on spiritual readiness for active duty (18-29 years of age).
4.2.1.1.6. Spiritual Enrichment programs (e.g., faith formation retreats, marriage retreats, etc.).

4.2.1.1.7. Comprehensive Catholic Pastoral Care.
4.2.2. AUTHORITY AND GOVERNANCE
4.2.2.1. The contractor shall be responsible for comprehensive pastoral care of the Catholic Community and serves as the subject matter expert for Roman Catholic accommodation as an integral member of the chapel staff at Fairchild AFB. The contractor must work collegially with the chapel staff and other contractors, particularly those serving the 92 ARW Catholic Community, in the interest of the common good of the Catholic community.
4.2.2.2. Contractor serves as the definitive advisor to the senior chaplain and is a vital member of the chapel staff on all matters regarding Catholicism and the Catholic program.
4.2.2.3. Contractor shall form and utilize a Pastoral Advisory Council to assist him in providing for those in the priest’s care. Such councils are consultative and not deliberative.
Given that the contractor is contracted to care for the active-duty population and their dependents, the priest may restrict retiree representation on the pastoral council.
4.2.3. THE BLESSED SACRAMENT
4.2.3.1. The contractor shall ensure the following:
4.2.3.1.1. A Blessed Sacrament Room must be reserved on all installations that have Roman Catholic Priest overseeing and supporting a Roman Catholic program. The Blessed Sacrament Room shall be for the exclusive use of the Roman Catholic Community and the reservation of the Blessed Sacrament under the care of the assigned priest(s), including the contractor.
4.2.3.1.2. The responsibility for safeguarding the Blessed Sacrament Room and the reservation of the Blessed Sacrament must never be entrusted to any other person under any condition than the assigned priest. The contractor shall provide guidance to any Extraordinary Ministers of Holy Communion (EMHC) who are trained and appointed to assist the priest in this responsibility.
4.2.3.1.3. Only Catholics approved by the AMS may function as Lay Leaders (LL), Catholic Representatives (CR) and EMHC. Additionally, the CR and LL must be active duty in the grade of E5 or higher. Authorizations are specific as to duration as well as to location. There is a mandatory training requirement for each of these three functions and accountability to the
AMS.
4.2.4. SUNDAY AND DAILY MASSES
4.2.4.1.. At the Survival School, Daily Mass may only be conducted when requested by the government. Otherwise, the weekday Mass may be offered at the Main Chapel.
4.2.4.2. Non-Fairchild AFB services are not included in this contract unless to the benefit of military members or families (such as emergency ministrations and hospital visitation) or upon approval from the COR.
4.2.4.3. Contractor shall be at the respective places of worship and prepared for each service no later than 15 minutes prior to the designated starting time of any service, and no later than 15 minutes prior to the designated starting time of any Special Services for preparations.
4.2.4.4. The celebration of Sunday Mass is critical to the spiritual life of the community and must be available at Fairchild AFB whenever possible. This celebration must conform to the General Instruction on the Roman Missal (GIRM). The location of the GIRM can be provided upon request by the COR.
4.2.4.5. Unnecessary multiplication of Sunday Masses will be avoided. Vigil Masses (and other Masses) scheduled as mere celebrations of convenience, or are of poor quality, or are divisive to the Catholic Community shall be recommended to the COR for removal from the

schedule.
4.2.4.6. The contractor shall conduct Masses as shown in the table in Appendix A.
4.2.5. SACRAMENT OF PENANCE
4.2.5.1. Catholics in the military will have access to a weekly scheduled celebration of the Sacrament of Penance. The scheduled times must be separated from Mass start time by at least 15 minutes and will be as shown in Appendix A.
4.2.6. AVAILABILITY OF THE PRIEST FOR SPIRITUAL CARE AND
FORMATION OF CHRISTIAN COMMUNITY
4.2.6.1. The principal responsibility of the contractor shall provide for the spiritual care of Catholic military personnel, their families and other authorized personnel. Such spiritual care includes the moral formation and spiritual readiness of Catholics.
4.2.6.2. Consistent with operational contingencies and mission essential requirements of the base, the contractor must be freely available for active-duty Catholics, dependents and other authorized personnel seeking his counsel. Since this is a primary mission responsibility, the contractor’s access to an office or workspace conducive to confidential counsel or confession is necessary. Military personnel and their family members have a right to confidential privileged communication. The contractor shall provide spiritual counseling sessions IAW DoD guidelines these conversations are confidential and privileged communication. The contractor shall complete annual training on privileged communication provided by the chapel staff.
4.2.6.3. Contractor shall consult the AMS regarding potential volunteer usage of Deacons within the Catholic Program.
4.2.6.4. In keeping with the essential requirements named above, the contractor must ensure that suitable lay ministers are properly selected, trained, commissioned and certified to AMS standards, ensuring comprehensive religious education and normative liturgical celebrations.
4.2.7. CONTRACTOR’S RESPONSIBILITIES VIS-À-VIS RELIGIOUS
EDUCATION COORDINATORS, AND OTHERS ASSISTING IN THE CATHOLIC
PROGRAM
4.2.7.1. When the contractor shall be responsible for the Catholic Program on an installation, he is also the Director of Religious Education (DRE) and has the vested responsibility to provide services within his area of responsibility IAW the directive of the AMS, USA (AMS Priests’ Manual, para 3.1).
4.2.7.2. The Coordinator of Religious Education (CRE) is that contractor, or in some instances a volunteer, who manages the day-to-day operation of the religious education program.
Together with other instructors, they are one of the means by which religious education is presented. This must be done in coordination with AMS and the contractor (AMS, Priests’ Manual, para 3.1.2.). The services of the CRE, when contracted, are considered Government-furnished. The contractor shall not provide CRE services.
4.2.7.3. All final authority for the content of Catholic religious instruction belong to the contractor (AMS, Priests’ Manual, para 3). Instructional materials used in religious education must be contained in the Conformity Listing maintained by the United States Conference of Catholic Bishops (USCCB).
4.2.7.4. The Catholic Program Lay Coordinator (CPLC) is a contractor or volunteer who administratively and logistically coordinates the functioning and activities of the program and the role and needs of the contractor. The contractor shall provide guidance to the CPLC. The

contractor shall be the subject matter expert for CPLC qualifications. The contractor shall provide programming guidance to the CPLC. The services of the CPLC are considered government-furnished.
4.2.7.5. Contractor shall become familiar with Chapel Readiness and Contingency Planning requirements and educate and train the Catholic Community accordingly.
4.2.8. ECUMENICAL AND INTER-RELIGIOUS OPPORTUNITIES FOR
UNDERSTANDING AND UNITY
4.2.8.1. DODI 1300.17, Religious Liberty in the Military Services states “the DoD places a high value on the rights of members of the Military Services to observe the tenets of their respective religions or to observe no religion at all.” The DAF is a pluralistic environment within which contractor supports and helps create a spiritual climate conducive to the health and well-being of DAF personnel and families, with respect for all faith expressions and persons, and with freedom to express his own without seeking to convert others. As appropriate, contractor shall guide, and as needed, appropriately refer individuals and families in the direction of their sought religious expression IAW current and forthcoming DoD and DAF policy guidance.
4.2.8.2. Contractor is encouraged to take advantage of the rich opportunities for collegial exchange inherent during Ecumenical and Interfaith events and dialogues.
5. GENERAL REQUIREMENTS
5.1. CONTROL AND REPORTING
5.1.1. CONTRACTING OFFICER’S REPRESENTATIVE (COR). The CO will appoint a (COR) for management of the day-to-day activities of the contract. The identity, title, and authority of this representative will be provided in writing to the contractor after contact award. The contractor shall meet with the COR to ensure understanding of Standard Operating Procedures (SOPs), Operating Instructions (OIs) and procedures, as well as understanding and control of chapel facilities.
5.2. ADMINISTRATION
5.2.1. Contractor shall provide advertising and promotional bulletin announcements, articles, flyers and materials to the COR for quality, language, and information purposes. This includes publication in chapel bulletins, installation newspapers, and bulletin boards.
5.2.2. Provide an annual review of the program, providing a summary of activity, trends and progress during the preceding year. Include information and suggestions regarding future program direction.
5.2.3. A monthly schedule shall be provided to the contractor by the COR on a quarterly basis at least fifteen days prior to the schedule’s start date.
5.2.4. Provide written after-action reports on all special events, retreats, emergency calls, etc. as applicable. These reports are to include suggested changes of direction, trends, and problem areas and are to be submitted to the COR within one week of completion of the event.
5.2.5. Provide input for youth ministry and all other program requirements in the annual ministry plan and include their projected costs.
5.2.6. Reserved
5.2.7. Provide the COR purchasing information in the authorized format for supplies and resources to support the Catholic Program.
5.2.8. Attend HC weekly staff meeting and update the Catholic data on the weekly Chaplain Corps (HC) “Staff Meeting Slides,” if applicable.

5.2.9. Contractor shall abide by AMS dress and appearance standards.
5.3. VOLUNTEERS AND SUPPORT COMMITTEE
5.3.1. The contractor shall recruit, organize, and train volunteers from the Catholic Community to assist in the conduct of the program. All volunteers who work with children under the age of 18 will be appropriately trained, with approved volunteer qualifications before working with children and will wear appropriate approved identifiers. Contractor ensures that a volunteer roster is kept and includes name, address, phone number, military affiliation, and role, where applicable, is secured, maintained, and updated with the 92 ARW Chaplain Corps Security Manager. It is the contractor’s responsibility to ensure that the moral character and good standing of all volunteers is consistent with AMS guidance and that personal information protections are applied IAW Privacy Act requirements.
5.3.2. The contractor shall ensure training is accomplished to teach and train volunteers in all aspects of the Catholic program. He shall help volunteers become comfortable in performing their duties, teaching them the skills and confidence to be effective volunteers.
5.4. GOVERNMENT FURNISHED PROPERTY AND SERVICES
5.4.1. The contractor shall ensure facilities are secured after services are provided, such as rehearsals, meetings, and classes (close and lock windows, turn-off lights, secure equipment, lock doors). The contractor shall safeguard keys to avoid unauthorized access to government-owned facilities and property. Facility keys and any other provided equipment belonging to the government shall be returned at the end of the contract period. the appropriate personnel.
5.4.2. All items required to complete assigned duties will be provided to include, private office space, computer, office supplies, printing and copier, and are considered incidental to the performance of this contract.
5.4.2.1. Limited use of office equipment will be provided to include, but not limited to projector, television, media player systems, copy machine, computer with network support and email account. Contractor shall complete all required training for access to the installation network.
5.4.3. Limited use of military transportation vehicles for the transportation of youth for specified events may be available. Transportation requests will be made through the COR or designated Chapel staff member. All requests are subject to approval and dependent upon availability of vehicles and authorized drivers. The contractor shall not drive government vehicles.
5.4.4. Contractor shall be liable for loss, damage, or theft of Chapel property due to the contractor’s carelessness and/or negligence. Negligence will be determined by appropriate investigative or report of survey findings.
5.4.5. Contractor shall coordinate reservation, set up, clean up, and lock up of the facilities needed as well as safeguard all funds (when applicable), property, equipment and supplies before, during and after all activities. Contractor shall be accountable for security and returning facilities to standard after utilization. The contractor coordinates all support requirements with the chapel staff.
5.4.6. The contractor may use additional personal items of equipment or music as part of his performance of duties with approval of the COR. The government will not be liable in any way for personal equipment including loss, damage, theft, or maintenance.
5.4.7. The contractor shall be liable for repair/replacement for any government or private

property damaged or lost due to negligence, to include vehicles, and equipment. The contractor shall immediately advise the COR of any losses or damages.
5.5. RECORDS MANAGEMENT.
5.5.1. The contractor shall provide all data created in the performance of this contract to the Chapel staff.
5.5.2. The contractor will provide sacramental records to the chapel staff who will manage the records IAW the instructions provided in the AMS Priest’s Manual found at http://www.milarch.org/ams-priests-manual/.
5.6. AIR FORCE ANTI-TERRORISM (AT) PROGRAM. The AT program seeks to deter or limit the effects of terrorist acts against the DAF by giving guidance on collecting and disseminating timely threat information, providing training to all DAF members, developing comprehensive plans to deter and counter terrorist incidents, allocating funds and personnel and implementing AT measures. DODI, 2000.12, DOD Antiterrorism (AT) Program, contractors shall be offered Level 1 - AT Awareness Training. AT Awareness Training may be accomplished by either a Level I - qualified instructor, or by completing the "Force Protection" computer-based training (CBT) course on MyLearning (https://lms-jets.cce.af.mil/moodle/). Compliance with the training will be monitored by the COR.
5.7. SAFETY. The contractor shall perform all work in a safe manner as required by Safety and Health Requirements and follow all DAF safety regulations and reporting procedures.
5.8. FRAUD, WASTE, AND ABUSE. The contractor shall be responsible for operating under conditions that prevent the waste, fraud, and abuse of resources, including utilities.
Contractor shall be encouraged to be alert for and report any suspected situations of fraud, waste, and abuse, or other dishonest conduct against the government observed during, or in the performance of this contract.
5.9. SMOKING. The contractor shall comply with DODI 1010.10, Health Promotion and Disease Prevention, which outlines uniform policies governing use of tobacco products in government facilities.
5.10. PERMITS, LICENSES AND CERTIFICATIONS. Unless otherwise specified in this contract, the contractor shall obtain all necessary permits, licenses, and certifications; give all required notices; and comply with applicable federal, DOD, DAF, state, county, and local laws, codes, and regulations in performance of the requirements of this contract. The contractor will maintain records of such requirements and properly make the documentation available when needed.
5.10.1. Contractor should take advantage of potential resources available for Archdiocese for the Military Services required training for Catholic Youth and Religious Education coordinators in support of current chapel programs.
5.11. GOVERNMENT QUALITY ASSURANCE. The contractor shall be responsible for quality control for all work accomplished during the performance of this contract. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the Service Delivery Summary.

5.11.1. The CO will periodically evaluate the contractor’s performance by appointing a COR to monitor performance to ensure services are received. The COR will evaluate the contractor’s performance IAW the Quality Assurance Surveillance Plan (QASP) for this requirement.
5.12. HOLIDAYS OBSERVED. Because of the nature of services being provided, services may be required on civic holidays and Holy Days associated with the Catholic faith.
Federal holidays:
New Year's Day, January 1 Martin Luther King Jr Day, the third Monday in January Presidents’ Day, the third Monday in February Memorial Day, the last Monday in May Juneteenth, June 19 Independence Day, July 4 Labor Day, the first Monday in September Columbus Day, the second Monday in October Veterans’ Day, November 11 Thanksgiving Day, the fourth Thursday in November Christmas Day, December 25 Any other day designated by Federal statute, executive order, or Presidential proclamation.
5.13. SECURITY REQUIREMENTS
5.13.1. Contractor. Since the contractor has regular and reoccurring contact with children, the Tier 1 background check, State Criminal History Checks with fingerprints will be conducted.
In the event the DoD CAF cannot make a suitability determination due to derogatory information, the suitability determination authority levels are identified as FSS/CC or MSG/CC (AFI 34-144, Child and Youth Programs, para 9.6.11.3.).
5.13.2. Pass and Identification Items. The contractor shall ensure the pass, identification, and computer access cards items required for contract performance are initiated according to Chapel guidelines and obtained for sub-contractors and non-government owned vehicles, if applicable.
5.13.3. Retrieving Identification Media. The contractor shall ensure all identification media, including vehicle passes are surrendered to the government upon departure for any reason before the contract expires, e.g. termination for convenience, cause, retirement, etc.
5.13.4. Traffic Laws. The contractor shall comply with base traffic regulations.
5.13.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately- owned vehicle while on military installations.
5.13.6. Controlled Unclassified Information (CUI). The contractor shall comply DODM 5400.07, Freedom of Information (FOIA) Program, AFMAN33-302, Communications and Information, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding CUI material.
5.13.7. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and/or classified or unclassified

defense information. The COR shall ensure the contractor has been briefed on all general security policies.
5.13.8. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
5.13.9. Controlled/Restricted Areas. Contractors shall be escorted by an individual who meets entry requirements to controlled/restricted areas.
5.13.10. Key Control/Access Badge Control. The contractor shall establish and implement methods of making sure all keys/badges issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.
5.13.11. Lost Keys. The contractor shall immediately report to the COR any occurrences of lost or duplicated keys.
5.13.12. Prohibited Use. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor. The contractor shall not provide access to other contractors for government facilities without COR approval.
5.13.13. Lock Combinations. The contractor shall control access to all government provided lock combinations to preclude unauthorized entry (as needed). The contractor shall not record lock combinations without written approval by the government Program Manager.
Records with written combinations to authorized secure storage containers, secure storage rooms, or certified vaults, shall be marked and safeguarded.
5.13.14. Automatic Data Processing Equipment (ADPE). The contractor shall establish and implement procedures to ensure all ADPE, to include laptops and other data processing devices, issued to the contractor by the government are not lost or misplaced, are not used by unauthorized persons or are not subject to unauthorized external devices. The contractor shall immediately report to the CO and COR any occurrences of lost ADPE within two (2) hours of discovery of occurrence. In the event that ADPE is lost, the contractor may be required, upon written direction of the CO, reimburse the government for the cost to replace the equipment. The government may replace the equipment and deduct the cost of such from the monthly payment.

6.1. Performance Assessment: This method requires the COR to employ a “spot check” style of evaluation based on the contractor’s performance schedule. Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, at the government’s discretion, based on quality trends. The schedule or modified version thereof, shall be used for COR scheduled inspections and results documentation. Any unsatisfactory inspection (defect) result shall be recorded, and the contractor shall re-perform the service within the time parameters listed in the Services Summary. Exceeding the performance threshold for any of these performance objectives in any one-month period may result in a warning or letter of concern from the CO. Exceeding the performance threshold of these performance objectives, in any combination for any two, or more consecutive or non-consecutive months during a performance year may constitute justification to hold an immediate Progress Meeting with the Multi-functional Team.
6.2. Complaints: The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement between the COR and the contractor. Validated customer complaints shall be re-performed IAW the Services Summary.
6.3. Performance of Services During Crisis Declared by the National Command Authority or Overseas Combatant Commander: IAW DODI 1100.22 Policy and Procedures for Determining Workforce Mix, and any applicable AF implementation (unless otherwise directed by an authorized government representative), services under this PWS have been determined by the Functional Commander/Functional Director as being not mission essential during Force Protection Condition Delta; however, they will be required to continue services during Force Protection Conditions Charlie.
6.4. Non-Personal Services and/or Inherently Governmental Services: The government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services or give the perception of personal services. If the contractor determines any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the CO immediately. These services shall not be used to perform work of a policy/decision making or management nature, (e.g., inherently governmental functions). All decisions relative to programs supported by the contractor shall be the sole responsibility of the government.

APPENDIX A
PROJECTED SCHEDULE OF SERVICES
A.1. Services are held in any of the facilities on Fairchild AFB, WA, the contractor is expected to provide his own transportation to and from services. Contractor shall normally be notified 72 hours in advance of a projected move and/or time change.
A.1.1. Monthly schedule shall be provided by the COR to the contractor on a quarterly basis at least 15 days prior to the start date of the new schedule.
A.1.2. Primary Place of Performance of services will be Base Chapel, Building 4200 and other chapel buildings located on Fairchild AFB, WA.

A.2 Definitions A.2.1. Weekday Mass - Weekday services are normally held Monday-Friday.
A.2.2. Sunday Mass – To include services held either Saturday after 4PM and on Sunday.
A.2.3. Holy Day Mass/Special Service - Celebrate the Mass according to current AMS guidance for the specific Holy Day being celebrated.

A.2.8. Sacrament of Penance/Confessions: Celebrate the Sacrament of Penance which is usually performed in conjunction with the designated weekend Mass. To also celebrate the Sacrament for First Communion candidates, this shall be known as 1st Reconciliation.
A.2.9. *Sacraments of Initiation: Celebrate the below religious ceremonies in accordance with Roman Catholic doctrine.
-Confirmation -First Communion *Note: May only be administered to AD military, Guard and Reserve while on orders, authorized DoD civilian personnel and their family members.

E.2. Definitions
Contract Administrator/Specialist
The individual within the contracting office who performs the day-to-day administration of the contract. The contract administrator/specialist may also be the contracting officer.
Contracting Officer
The duly appointed government agent authorized to award or administer contracts. The Contracting Officer is the only person authorized to contractually obligate the government, make or approve of changes to the contract.
Corrective Action Report Used to document unacceptable performance by the contractor.
Defect Any non-conformance with requirements specified in the contract.
Defective Service A service output that does not meet the standard of performance specified in the contract for that service.
Emergency Sacrament
To include but not limited to Anointing of the Sick and Confession - conduct sacraments with Active Duty personal and their dependents requesting to see a Roman Catholic priest to perform emergency ministrations such as Anointing of the Sick or Confession.
Faculties
Authority, privilege, or permission to perform an act or function, Rights granted by the Holy See to bishops and by ordinaries to their priests to enable the latter to exercise their respective powers for the faithful under their jurisdiction.
The term is most commonly used in relation to the sacrament of penance, where faculties are needed for the priest to absolve not only licitly but even validly. However, faculties for licit administration are required for all sacraments, with varying degrees of obligation.
Fair Wear and Tear The deterioration of items attributed to normal usage.
