About this file
This is a Past Performance Questionnaire used by the 28th Contracting Squadron of the Air Force Global Strike Command to evaluate potential contractors for a School Bus Services contract (FA4690-26-Q-0017). The questionnaire is designed to be completed by organizations with direct experience working with offerors to provide reference information for source selection purposes.
Respondents are requested to submit completed questionnaires by May 9, 2026, to allow sufficient time for analysis prior to negotiations. The form can be completed electronically and submitted via email to christian.depollar.2@us.af.mil and cc: jayna.clements@us.af.mil, with questions directed to 2d Lt Christian Depollar at (605) 385-1725. The questionnaire is divided into four main sections: contract identification (contractor information, contract number, type, program title, period of performance, and dollar value), customer or agency identification (customer name and geographic scope of work), respondent identification (contact information and role), and performance information (detailed evaluation criteria). The performance evaluation section contains 24 sample questions addressing contractor capabilities in areas such as management and supervision, personnel hiring and maintenance, quality control, schedule adherence, problem resolution, cost proposal accuracy, and cooperation with government personnel. Respondents rate performance using a three-tier scale: Satisfactory (meets contractual requirements with minor issues), Unsatisfactory (does not meet most requirements with ineffective corrective actions), or Not Applicable (insufficient information). The document notes that evaluation teams must tailor the questionnaire to seek key information tied to significant aspects of the specific acquisition, avoiding simple yes/no questions in favor of probing and direct questions. Completed questionnaires become part of official source selection records and are marked as Source Selection Information per FAR 2.101 and 3.104.

Past Performance Questionnaire FA4690-26-Q-0017 Attachment 3
The 28th Contracting Squadron of the Air Force Global Strike Command is in the process of competitively selecting a source for School Bus Services. The required effort will be the operation and maintenance of school buses used to transport students residing on base.
One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 09 May 2026. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to christian.depollar.2@us.af.mil and cc: jayna.clements@us.af.mil. Please call 2d Lt Christian Depollar at (605) 385-1725 if you have any questions. Your completed questionnaire will become a part of the official source selection records.
Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
