Deliverables include nine tasks: a kick-off meeting summary, quarterly progress updates, interim evaluation plans and reports, a primary formative research plan, briefs on branding/marketing/recruitment strategy and shipping logistics, a list of recommended project sites, and a final comprehensive formative research report synthesizing epidemiologic findings and implementation recommendations. The contract is firm-fixed-price, with the vendor proposing a milestone payment schedule. The period of performance runs August 1, 2026 through July 31, 2027, with performance at the contractor's facility. The NAICS code is 541720 (Research and Development in Social Sciences and Humanities) with a small business size standard of $28.0 million; the PSC code is B599 (Special Studies/Analysis – Other). The RFQ is unrestricted competition. Quoters must be registered in the System for Award Management at time of submission. Questions are due July 28, 2026 at 4:00 p.m. ET, and quotes are due July 29, 2026 at 10:00 a.m. ET. Award will be made to the responsible quoter submitting the lowest-priced quotation conforming to all material requirements. Quoters must demonstrate experience implementing multi-site evaluations with community-based organizations in rural settings and must disclose any planned or potential artificial intelligence use with a compliance and risk management plan if applicable.

Project Requirements – Statement of Work

Project Title: Procurement of formative research assessment data for expansion of the rural outreach testing evaluation

SECTION 1 – BACKGROUND

Self-collected blood specimens for viral load monitoring have the potential to improve retention across the HIV prevention and care continuum and directly support national goals under the Ending the HIV Epidemic (EHE) initiative. The TrueLevel Feasibility Study demonstrated both operational viability and community acceptance of self-collected specimens for viral load monitoring across piloted sites in the Southeast. Building on these findings, this scope of work proposes a multi-jurisdictional formative research study to support expansion of self-collection-based viral load monitoring into additional EHE priority states.

SECTION 2 – PURPOSE

The purpose of this formative research is to identify site-specific implementation requirements, assess operational and community readiness, and establish conditions for rapid pilot activation following completion of formative activities for geographically expanding the evaluation of the self-collection protocol. The study will use a rapid formative-to-implementation bridge model, enabling sites that demonstrate readiness to transition directly into in-field evaluation of the collection method.

SECTION 3 – SCOPE OF WORK

The vendor’s work will be guided by the Social Cognitive Theory (SCT) to assess how individual cognition, community influence, and structural factors a*ect adoption of self-collection behaviors. The research will build specifically on the outcomes of the field implementation component of the existing TrueLevel Feasibility Study conducted in South

Carolina, Mississippi, Missouri, and Alabama, with emphasis on reported specimen collection self-e*icacy as well as environmental facilitators and barriers, including digital enrollment access, mail-based specimen transport infrastructure, and availability of trusted community health intermediaries. The study will also assess feasibility in priority expansion states, such as South Carolina (new sites), Kentucky, Oklahoma, Missouri (new sites), and Arkansas.

SECTION 4 – TASKS TO BE PERFORMED

The vendor will employ a mixed-methods approach to integrate quantitative data analysis, qualitative research, operational assessments, and site visits across expansion states.

Operational readiness assessments will examine specimen transport logistics, including shipping workflows, drop-o* feasibility, in-person versus remote collection models, regional supply chain capacity, and Community-Based Organization (CBO) readiness for

HubSpot platform integration.

The Vendor will perform community research to assess usability perceptions of self-collection, and structural determinants such as digital access and health literacy in the proposed expansion areas. One community leader per potential site will be engaged for the formative research process for up to a total of 5 persons maximum being interviewed. The discussion topics will include clinical referral pathway mapping, digital platform usability testing, and community-informed recruitment and education messaging testing. These activities will support co-development of implementation strategies and strengthen community trust.

The ultimate deliverable will be a comprehensive formative research report synthesizing epidemiologic findings, site readiness assessments, and implementation activation recommendations created by vendor sta*. The report will also include multi-site comparative analyses to identify scalable implementation strategies and shared structural and community barriers.

The contractor shall perform the following tasks:

Task (1) Develop agenda and convene kick o* meeting, and provide a summary report of kick o* meeting

The NDA also requires contractor’s employees properly identify themselves as employees of a contractor when communicating or interacting with CDC employees, employees of other governmental entities, and members of the public (when communication or interaction relates to the contractor’s work with the CDC). The Federal Acquisition Regulation (FAR) 37.114 (c), states “All contractor personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public or Congress that they are Government o*icials, unless, in the judgment of the agency, no harm can come from failing to identify themselves. Therefore, if an employee participates in any discussions relating to procurement-sensitive information, assists in developing any procurement-sensitive information, or otherwise obtains any procurement-sensitive information while performing duties at the CDC, contractor understands and agrees that contractor may be excluded from competing for any acquisition(s) resulting from this information.
