About this file

This is a Mandatory Requirements Attachment for RFP 26-86494 issued by the Indiana Department of Family and Human Services (DHHS) for the procurement of hearing aids, assistive listening devices, and related ear apparatus. Respondents must meet minimum requirements including providing at least a 46% discount on all products, establishing individual accounts per VRS Counselor or State Agency rather than a single state account, and demonstrating the ability to accept orders via email, fax, online portal, or other means. The vendor must maintain separate billing and shipping addresses, provide order confirmation and shipping notifications, and implement a wrong product RMA process that includes issuing prepaid return labels within two business days and shipping correct replacements within five business days at no cost to the State.

Device and service requirements include providing hearing aids with integrated ear apparatus at prices that incorporate at least a two-year repair warranty and one-year replacement warranty for loss or damage. Respondents must offer a 90-day full refund return policy for hearing aids and devices, a 60-day return policy for ear molds and accessories, and waive all interest, restocking, and shipping fees. Hearing aids must be delivered to dispensers within two weeks of receiving ear impressions and within five days of non-ear mold orders. Additional mandatory requirements include providing a staff audiologist for consultations, yearly device training, individual invoicing within five business days, online tracking capabilities, and maintaining a primary point of contact responsive to communications within 48-72 hours. The vendor must support Indiana's Claims Payment System for VR participant claims and coordinate billing directly with state agencies and the State Employee Hearing Aid Program.
