About this file
This is an Attachment A form detailing Minority & Women's Business Enterprises (MWBE) subcontractor commitment requirements for a State of Indiana Request for Proposal. The document establishes guidelines under Indiana Code 4-13-16.5 and 25 IAC 5 for proposing certified MWBE subcontractors as part of RFP responses. Respondents have the option to propose MWBE subcontractor commitments, which must be submitted with their proposals if selected. All proposed subcontractors must be listed on the State of Indiana Certified M/W/IVOSB directory at the time of proposal submission. If subcontractors are proposed, respondents must provide detailed scope of work descriptions, explain whether products or services will be utilized directly by the contractor or the state, describe the application process, define deliverable requirements, identify applicable UNSPSC codes, and specify all associated costs. The total bid amount entered on the MWBE Subcontractor Commitment Form must match the amount in Attachment D's Bid Cost Template.
Prime contractors proposing MWBE subcontractors must ensure each proposed firm meets strict criteria, including current state certification, submission of certification letters from the Indiana Department of Administration (IDOA), classification as only one type (MBE, WBE, or IVOSB), and provision of goods or services exclusively within their certified industry area. Certified prime contractors who are themselves MBE or WBE firms cannot count their own workforce toward subcontractor goals. Each subcontractor must serve a valuable scope contribution and provide only industry-specific services germane to the contract. Accompanying each commitment form must be signed letters on company letterhead from proposed MBE and/or WBE subcontractors acknowledging the subcontract amount, describing products and services, and specifying the anticipated work period. Subcontractor commitments apply to the entire contract life, including periods beyond the initial term. Failure to meet MWBE participation goals does not affect proposal evaluation. Questions should be directed to the Division of Supplier Diversity at (317) 232-3061 or via the MWBE website.
