About this file

This document is an Attachment E: Bidder Response Form for a state and local government contract opportunity issued by the Washington State Department of Health (DOH) for Care-a-Van Mobile Services. The contract requires the bidder to provide mobile preventative health services, including vaccine administration, in areas with significant needs and restricted access, as well as the ability to respond to public health emergencies and disasters. The bid submission requires the bidder to provide information on their qualifications, experience, technical capabilities, and pricing. Key requirements include the bidder's experience with community clinics, working with diverse populations, capacity to meet program objectives, trained staff, ability to operate a mobile health clinic, vaccine procurement and insurance billing, adherence to regulatory requirements, and reporting capabilities. Bidders must also certify that they do not require employees to sign mandatory arbitration clauses or class/collective action waivers. The bid is scored based on the bidder's qualifications (45 points), technical proposal (35 points), and pricing (20 points).

The contract opportunity is open to Washington Small Businesses and Certified Washington Veteran-Owned Businesses, who can earn additional points for their business status. Bidders must provide detailed pricing information, including administrative costs and the total proposed contract amount. The contract term is not specified, but the document indicates the ability to quickly respond to future public health emergencies and disasters, suggesting a longer-term or renewable contract.
