About this file
This is a Cost Proposal template (Attachment D) for OregonBuys Bid #S-58100-00012591 for AI/AN Special Education Guidance and Resources. The project requires vendors to provide deliverables including a Toolkit, Mini Guides, Video Learning Modules, and Infographics. The work is broken down into five main tasks: attending a kickoff meeting, participating in four project check-in meetings, submitting first drafts, submitting second drafts incorporating Agency feedback, and submitting final drafts incorporating both Agency and AI/AN Advisory Committee feedback.
The cost proposal must be all-inclusive, covering preparation of materials, travel, software, labor, overhead, and profit. Pricing must be submitted as total costs per task rather than hourly rates. The Source Selection Committee will evaluate costs using a point system where the lowest cost proposal receives 15 points, the second lowest receives 10 points, the third lowest receives 5 points, and all others receive zero points. Failure to submit this cost proposal form results in disqualification.
