PERFORMANCE WORK STATEMENT (PWS)

B 721

ABATE FLOORING

VLSB 20-2034

Part 1

General Information

1. GENERAL: This is a non-personnel services contract to provide the ABATEMENT OF FLOORING at B 721.

The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.

1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform the abatement of approx. 1800 Sq. Ft. of flooring at Building 721 as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract.

1.2 Background: Existing mastic and vinyl tile has tested positive for asbestos and requires abatement prior to renovation of the facility.

1.3 Objectives: Abatement of existing flooring (vinyl tile and mastic) and providing Clearance Air Samples for the project.

1.4 Scope: To remove the existing floor tile and mastic in the facility. Services include the proper abatement of the facility as described by SCDHEC Reg. 61-86.1. The contractor shall accomplish the abatement activities and provide Clearance Sample results.

1.5 Period of Performance: 14 days after issuance of NTP.

1.6 General Information

1.6.2 Quality Assurance: The Government shall monitor and evaluate the Contractor’s performance to ensure services are received IAW requirements set forth in this PWS. The method of surveillance will be spelled out in the Governments’ Quality Assurance Surveillance Plan. The COR will periodically inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, or otherwise inspecting the task or its result to determine whether or not the performance meets the standards contained in this PWS. The COR may also inspect tasks not specifically mentioned in the Services Summary. The COR will document the results of the surveillance and this documentation will become the official Air Force (AF) record of the Contractor’s performance. When a performance threshold has not been met or Contractor performance has not been accomplished, the COR will initiate and provide a report to the CO for action concerning the Contractor. The Contractor shall respond to the CO within ten (10) calendar days of receipt. The surveillance results shall be maintained as past performance information of the Contractor’s overall compliance with contract terms and conditions. The Government will neither supervise contractor employees nor control the method by which the contractor performs the required tasks. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer immediately.

The COR will be identified by separate letter. The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, FA480320QB091

designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

1.6.3 Recognized Holidays: [Contractor is not required to work recognized holidays but may request to in advance.]

1.6.4 Hours of Operation: The contractor is responsible for conducting business, between the hours of 0730 – 1630 hours, Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.

1.6.5 Base Closure: The Government will notify the contractor of anticipated closure of the facility due to inclement weather or any other authorized closure. In the event of an unplanned closure of the facility the contractor will be notified using the contact information provided. The Contractor is not authorized to bill the Government for those hours unless the Contractor can perform productive duties from an alternate worksite. Work scheduled but not accomplished because of base closure, will be accomplished as soon as possible after reopening the base.

1.6.6 Place of Performance: The work to be performed under this contract will be performed at Building 721 Shaw

AFB SC 29152.

1.6.7 Type of Contract: The government will award a FIXED FEE CONTRACT.

1.6.8 SECURITY REQUIREMENTS.

1.6.8.4. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and level of security clearance (if needed). The list shall be validated and signed by the company representative and provided to the Sponsoring Agency Security Manager. An updated listing shall be provided when an employee's status or information changes. Sending a “Visit Request” through the Joint Personnel Adjudicative System (JPAS) can also fulfill this requirement.

1.6.8.5. Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.6.8.8. Security Training. The Contractor ensures all Contractor employees receive initial and recurring security education training from the sponsoring agency's security manager. The security manager shall provide employees with training required by DODM 5200.01, DoD Information Security Program, Volume 3, and AFI 16-1404, "Air Force Information Security Program". The Contractor will also provide initial and follow-on training to contractor personnel who work in Air Force controlled/restricted areas. Air Force restricted areas and controlled areas are explained in AFI 31-101, Integrated Defense.

1.6.8.6 Pass and Identification Items: The Contractor shall comply with all requirements and procedures in accordance with (IAW) Air Force Federal Acquisition Regulations System (AFFARS) 5352.242-9000, Contractor Access to Air Force Installations. The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees. All contractor employees shall undergo a background check conducted by

20th Security Forces Squadron prior to issuance of base access credentials. Until 1 Oct 2020 Shaw AFB will accept identification cards or driver's licenses from all states and Territories except ones marked "NOT VALID FOR FEDERAL IDENTIFICATION or FERERAL LIMITS MAY APPLY or any other words to that effect." Effective 1 Oct 2020 only REAL ID identification cards or driver's licenses will be accepted. For a full list of REAL ID Act and compliant and non-complaint states, please visit https://www.dhs.gov/current-status-states-territories. Contact 20th Security Forces Squadron for alternative forms of acceptable identification to conduct background checks. All documentation shall be submitted for base passes or base identification per Base Security Requirements. Contractor personnel shall obtain and wear specific Contractor base passes. Contractor personnel who have personal Department of Defense (DoD) retiree or dependent identification cards (ID), shall not use those personal ID cards for base access when performing contract work. All Contractor personnel who operate a vehicle in performance of this contract shall possess a valid driver’s license, current registration and insurance.

Failure to turn in a base credential when requested can result in a non-recommend from Security Forces for future base access. The Contractor is responsible for employee inability to obtain base access required for contract performance. The contractor will be responsible for ensuring their employees are added to the Security Forces Entry Authorization List (EAL) to facilitate their servicing within the flight line perimeter.

1.6.8.7 Background Checks. All Contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Each Contractor personnel must submit their Social Security Number, Date of Birth, and Driver's License state and number (if applicable). Failure to submit required information can result in denial or delay of issuance of credentials. Background checks are only good for the company that requested them. Processing background checks takes, at a minimum, 7-10 days from time of submission.

1.6.8.8 Federal Investigations Requirements. Contractor employees requiring initial issuance of a Common Access Card (CAC), Access to Restricted Areas, Use of Government Automated Information Systems/Networks, Child Care positions, or designated Positions of Trust shall complete submission of a Tier 1 investigation to the Office of Personnel Management (OPM), or a DoD-determined equivalent investigation. When required, the Government (20 FW Information Protection Office) will submit Tier 1 investigations to OPM at no cost to the Contractor.

1.6.8.9 Retrieving Identification Media. The Contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc.

1.6.8.10 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.

1.6.8.11 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw AFB SC.

1.6.8.12 For Official Use Only (FOUO). The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

1.6.8.13 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

1.6.8.14 Controlled/Restricted Areas. The Contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where Contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed Tier 1 investigation before receiving a Restricted Area Badge. Interim access can be granted IAW AFI 31-501, Personnel Security Program Management.

1.6.8.15. Photography Restrictions. No one is authorized to take any pictures or video recordings of Air Force

operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, photographs taken from personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities. Any Contractor requiring pictures or videotape of Shaw's restricted/controlled areas to support their contract must request and possess written authorization prior entering the area. Written authorization letters to photograph or video the flightline must be signed by one of the following: 20 Maintenance Group Commander or Deputy; Aircraft Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; 20 Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations; or 20 FW/CVN. Written authorization letters to photograph or video MQ-9 operational areas must be signed by the 50 ATKS or 25 ATKG commander. Written authorization letters to photograph or video the Missile Defense Agency area must be signed by the 609 ACOMS commander.

1.6.9 Special Qualifications: All personnel are required to have a valid SCDHEC license for asbestos operations and a current fit test for the type of respirators in use.

1.6.10 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced.

Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government. Other topics may include (as applicable): opportunities to improve the contract, any modifications required of the contract, unsatisfactory performance inspections results, and valid customer complaints against each performance objective observed, and the results of corrective and preventive action measures. The Contractor will also notify the CO in writing, of work being requested or performed, if any, that the Contractor considers over and above the requirements of the contract. Appropriate action shall be taken to resolve outstanding issues. The Government will provide the Base Environmental Plan at this meeting. The minutes of these meetings will be documented in writing, signed by the CO and any other signatures as deemed appropriate, and distributed to the COR functional area and the Contractor. Should the Contractor not concur with the minutes, the Contractor will set out in writing to the CO any areas of non-concurrence. These meetings shall be held as needed, but not less than bi-annually after the contract is awarded. There will also be an initial meeting to take place 30 days after the contract has been awarded.

1.6.11 Key Personnel: The follow personnel are considered key personnel by the government: Mr. Charles Whittington, 20 CES Planning. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the contracting officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager or alternate shall be available between 0730 – 1630 hours, Monday thru Friday except Federal holidays or when the government facility is closed for administrative reasons. Qualifications for all key personnel are listed below:

1.6.12 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should identify personnel easily and quickly for reasons of safety and personal protection. Contractor personnel shall display distinguishing badges or other visible identification. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed.

1.6.14 Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to or participate in the development of proprietary or source selection information

(e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational Conflict of Interests (OCI) as defined in FAR Subpart 9.5. The Contractor shall notify the Contracting Officer immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor’s mitigation plan will be determined to be acceptable solely at the discretion of the Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may effect other remedies as he or she deems necessary, including prohibiting the Contractor from participation in subsequent contracted requirements which may be affected by the OCI.

1.6.16 STANDARDS: The Government is authorized to restrict employment under this contract of any Contractor personnel or prospective Contractor personnel, who is identified as a potential threat to the health, safety, security, general well-being, or the operational mission of the installation (to include annexes such as Poinsett and Wateree) and its population. A determination may also remove Contractor personnel for, but is not limited to, incidents involving most immediately identifiable types of misconduct or delinquency such as: disorderly conduct, use of abusive or offensive language, intimidation by words or actions, fighting, participation in disruptive activities which interferes with the normal operations or brings discredit to the Government, theft, vandalism, sexual harassment, or any criminal actions. This also includes selling, consuming, or being under the influence of intoxicants, drugs, or substances which produce similar effects while in or on Federally-controlled property. Contractor personnel shall only smoke and/or use tobacco products in designated and approved smoking/tobacco use areas. If the CO directs removal of Contractor personnel for violating the above provisions, the Contractor shall immediately remove the individual(s) at no cost to the Government. Removal of employees for misconduct does not alleviate the Contractor from providing services. Per Shaw AFB Plan 31- 101, the installation commander has authority to debar any individual from the installation who commits a crime or offense prejudicial to good order and discipline. All disputes to the CO’s decision must be filed within ten (10) days of the decision to the Base Operations Support Flight Chief. The removed Contractor personnel may not resume work unless/until a decision is made by the Government CO overturning the removal decision.

1.6.17 SUBCONTRACTOR COMPLIANCE: The Contractor shall be responsible for ensuring Subcontractors satisfy the requirements set forth in the contract. The Contractor shall include a provision in all subcontracts to require Subcontractor compliance with terms and conditions of this contract.

1.6.18 EMPLOYING GOVERNMENT PERSONNEL: The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest.

Further, the Contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DODD 5500.7R, Joint Ethics Regulation. In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Industrial Labor Relations Activities. All questions of “conflict of interest” shall be referred to the CO. The Contractor is cautioned that off duty active military personnel hired under this contract are always subject to Permanent Change of Station (PCS), change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the Contractor's ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.

1.6.19 COMPLIANCE WITH LABOR LAWS: The Contractor shall comply with all applicable laws pertaining to wages, worker’s compensation, equal opportunity, Service Contract Act, and so forth, as implemented by Air Force directives and required by law.

1.6.20 Continuation of Essential Contractor Services: This requirement is not considered mission essential, and services will be suspended during a declared emergency. Contractor personnel will not be required or expected to perform maintenance in such an event as a hurricane or when travel is deemed “unsafe” by local authorities.

Inspections and/or maintenance actions due during this time will be completed as soon as the condition warranting the delay is lifted. This maintenance will not be withheld until the next service interval and contractor personnel are expected to perform the maintenance that was to be performed prior to the delay within 5 days.

1.6.21 Information Management: All records, files, documents, regardless of media (e.g., paper, electronic, etc.) as described in the PWS that are the responsibility of or generated by the Contractor, are the property of the Government, and shall remain so upon termination or completion of the contract. The Contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, paragraph 10, Air Force Manual (AFMAN) 33-363, paragraph 6.4, AFI 33-364, paragraph 2.10, and the Air Force Records Disposition Schedule available on the Air Force Portal link. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Shaw AFB Records Management Office (20 CS/SCOKR), 895-1630, will provide assistance and training to the Contractor to ensure compliance with Federal record keeping requirements.

1.6.21.2 Records: The Contractor shall be responsible for creating, maintaining, and disposing of only those Government required records that are specifically cited in this PWS. If requested by the Government, the Contractor shall provide the original record or a reproducible copy of any such record within five working days of receipt of the request.

1.6.22 Purchase of Recycled Products: The Contractor shall comply with the DoD Green Procurement Program for Recovered Materials.

1.6.23 Publications: The Contractor shall ensure compliance with all applicable publications, regulations, and operating instructions related to the tasks set forth in this PWS. The Contractor is required to acquire and abide by the latest version of all publications. See Part 6 for a list of applicable publications.

1.6.24 Safety Requirements: The Contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements and applicable Department of Labor, Department of Defense and Air Force safety standards.

The Contractor shall provide a verbal report to the CO as soon as possible following an occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of the required Accident Investigation Report to the CO within five (5) calendar days of each occurrence. The Accident Investigation Report shall be prepared and submitted IAW AFI 91-204.

1.6.24.1 Mishaps: In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.

1.6.24.2 Investigations: If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.

1.6.24.3 Lock Out Tag Out (LOTO): The Contractor will follow all LOTO procedures IAW OSHA regulation

1910.147 and AFI 91-203. The Contractor will also provide proof of an approved LOTO process to the COR upon contract approval.

1.7. Environmental Management System, Executive Summary For Contractors

1.7.1. Introduction. In accordance with Air Force Instruction 32-7001, Environmental Management, Shaw Air Force Base (SAFB) has implemented an ISO 14001 based Environmental Management System (EMS). The goal of the EMS is to improve the core business processes of SAFB, as they relate to the environment, seeking to improve and enhance performance in support of the overall mission. The EMS supports a transition from a compliance driven program to one that focuses pollution prevention efforts on managing both environmental and mission risks.

1.7.2. Environmental Policy. Central to the SAFB EMS is the base environmental policy set by Wing Leadership.

The policy establishes fence-to-fence applicability to include all 20th Fighter Wing organizations, all tenants, and contractors conducting business on SAFB property. It further extends applicability to the areas under SAFB control at Wateree Recreational Area and the Poinsett Electronic Combat Range. As indicated in the policy, SAFB leadership is committed to continual improvement, pollution prevention, regulatory compliance, the

conservation of natural resources, and the integration of environmental awareness into day-to-day operations.

Through such commitments, SAFB is able to set environmental objectives and targets with focus on enhancing the mission through environmental management.

1.7.3. What is EMS? An EMS is that part of an overall management system that includes organizational structure, planning activities, responsibilities, practices, procedures, processes, and resources for developing, implementing, achieving, reviewing, and maintaining the environmental program and achieving environmental goals based on continual-improvement. The EMS is intended to help SAFB balance its mission with maintaining regulatory compliance and developing sustainability strategies to secure this base’s ability to support current and future mission requirements.

1.7.4. Significant Aspects. An aspect is any interaction with the environment caused by activities on SAFB.

Through the EMS, these aspects have been systematically inventoried and evaluated based on the following ranking criteria.

Relative financial cost Risk to the environment Risk of regulatory non-compliance Potential mission degradation Community Concerns

These criteria are scored and weighted for each aspect as recommended by Wing Leadership to establish those environmental aspects deemed most critical to address.

1.7.5. What is My Role? As a contractor performing work on SAFB property you must understand that any activities that directly or indirectly impact the environment, especially the significant aspects listed above, must be communicated with the 20th Civil Engineer Squadron, and the greatest precaution should be taken to avoid negative impacts in these areas. Think of EMS the same way you think about safety. Just as you are aware of all safety hazards associated with your job, you must also be aware of all environmental impacts associated with your job and mitigate those to the extent possible.

DEFINITIONS & ACRONYMS

2. DEFINITIONS AND ACRONYMS:

2.1. DEFINITIONS

2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.

2.1.5. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.

2.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

2.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

2.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.

3. GOVERNMENT FURNISHED ITEMS AND SERVICES:

3.1. Services: N/A

3.2 Facilities: The Government will provide Building 721 for the abatement process.

3.3 Utilities: The Government will furnish sufficient quantities of electricity, water, sewage, that currently exists in the facility, if additional power is required the contractor shall provide generator power: The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment.

3.4 Equipment: N/A

3.5 Materials: N/A

Security Police: The Government will provide general on-base security police service. Use the following numbers as needed:

Security-Police and Fire Protection: Call phone number 911.

Emergency Medical Service: The Government will provide emergency medical treatment for Contractor personnel while they are performing duties associated with this contract. The Contractor shall reimburse the Government for the cost of medical treatment and patient transportation at the current inpatient or outpatient treatment rate as appropriate.

Forms and Publications: The Government will provide forms and publications to perform the work required by the contract. If network access is included in this contract, the Government will provide electronic access to required forms and publications. Publications can be accessed on-line at http://www.e-publishing.af.mil.

Base Civil Engineers: The Government will provide fire prevention inspections and maintenance of Government furnished fire systems.

Postal/Installation Distribution:. All commercial mail services are the responsibility of the Contractor.

Parking: Space is available for common vehicles at the parking lot adjacent to the personnel gate outside of Building 721.

PART 4

CONTRACTOR FURNISHED ITEMS AND SERVICES

5.1. Basic Services. The contractor shall provide services for the abatement of approx. 1800 Sq. Ft. of asbestos containing flooring in B 721

5.3. CONTRACTOR MANAGEMENT REPORTING (CMR): The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via secure data collection site. The contractor is required to complete all required data fields at the Air Force CMRA link http://www.ecmra.mil. Reporting inputs will be for the labor executed during the period of performance for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
