PERFORMANCE WORK STATEMENT

25 AUGUST 2024

VANDENBERG SFB, CA

1. OBJECTIVES. The Crucible Innovation Lab requires commercial internet services to support high performance computers & software, computer programming, 3D design & modeling, additive/subtractive manufacturing processes & equipment, and teleconferencing.

2. SCOPE. Provide Commercial Internet Services (CIS) for Crucible Innovation Lab users. Internet Service Provider (ISP), hereby referred to as “The Contractor”, shall provide installation, operations, and maintenance of security appliances, distribution nodes, switches, routers, and wireless access points. ISP shall provide dedicated 1 Gb (upload/download) commercial internet access for the Crucible Innovation Lab, Bldg. 10363, on 720 Community Loop, Vandenberg SFB, CA 93437.

3. DESCRIPTION OF WORK.

3.1. The Contractor shall provide establish commercial internet services and provide all associated commercial internet services operations and maintenance for the Crucible Innovation Lab, Bldg. 10363.

3.1.1. The Crucible Commercial Internet consists of: Comment by NGIRASWEI, LUCILLE D CIV USSF SSC 30 CONS/PKP: Edit text with your company’s proposed equipment list.

3.1.1.1. One (1) Gb of Commercial Internet Service (CIS).

3.1.1.2. One (1) Meraki MX250 Security Appliance

3.1.1.3. [X number] (X) Cisco Fiber Distribution Nodes

3.1.1.4. [X number] (X) Cisco 3750 or similar Cisco Switches

3.1.1.5. [X number] (X) Meraki MR36, MR56, or similar Meraki Wireless Access Points

3.2. The Contractor shall remove any contractor owned equipment from Bldg. 10363 within 60 days of contract completion or termination of contract.

3.3. The Government shall provide fiber transport pending availability and government approval IAW AFMAN 17-2101.

3.4. The Government shall pay the annual Meraki Licensing fee.

3.5. The Contractor shall coordinate with the Bldg. 10363 Facility Manager to schedule CIS service cutover and equipment relocation to be completed within 30 days from start of Period of Performance (PoP).

3.6. The Contractor shall perform all work using industry standards, to include ANSI/TIA/EIA 568-B Commercial Building Telecommunications Cabling Standard; UFC 3-580-01 Telecommunications Interior Infrastructure Planning and Design, with Change 1; as well as NEC, MIL-HDBKS, STDS, Unified Facility Guide Specifications (UFGS), Unified Facilities Criteria (UFC), Caltrans Standard Specifications and Caltrans Standard Drawings.

4. Hours of Operation.

4.1. Normal Hours. The Contractor is responsible for conducting business, during normal hours of operation between the hours of 7:00 A.M. and 4:00 P.M. Pacific Time, Monday through Friday excluding weekends and recognized federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor is required to perform services during normal hours of operation (standard hours) unless provided specific written authorization by the Contracting Officer to work Special Hours. The Contractor must submit a request and justification for working non-standard hours to the Contracting Officer at least three (3) working days in advance. Inclement weather, operational delays, and security related delays might affect the contractor’s work periods. The Contractor may be required to temporarily stop work for operational requirements, special events, installation exercises, or while Distinguished Visitor (DV) familiarization visits/tours are conducted. Notification to the Contractor to shall be provided by the Contracting Officer as required. The Contractor must make allowances for these events and resulting lost time will not be reimbursable.

4.2. Recognized Federal Holidays. The Contractor is not required to perform services on recognized federal holidays unless provided specific written authorization by the Contracting Officer to work Special Hours. The following is a list of recognized federal holidays:

5. Environmental Requirements.

5.1. The Contractor shall adhere to all environmental requirements in addition to any other environmental coordination and laws applicable. Refer to PWS Attachment 1 - Vandenberg SFB Environmental Specifications Section 01 57 20.

5.2. All materials to be removed and/or disposed of by the Contractor shall remain the property of the Government.

5.2.1. The Contractor shall make every effort to recycle materials generated by this effort to include but not limited to woods and metals. The Contractor shall contact the Vandenberg Recycling Center (805-605-1143/0102) to coordinate disposal of Government Office Furniture, Wood, and Scrap Metal.

5.2.2. The Contractors shall coordinate with DLA Disposition Services (previously “DRMO”) at 805-606-9852 for proper disposal of items noted in PWS Attachment 2 - 30 CES GovStuff Instructions.

6. Hazardous Material Requirements.

6.1. The contractor shall obtain approval from the government's hazardous material supply system (known as HAZMART), by processing an Air Force Form 3952, Chemical/Hazardous Material Request Authorization, for all of their hazardous materials with the HAZMART, 30 CES/CEIEC Environmental Support (Bldg. 5500 Bay C Rm 10), (805) 605-3870 / 7573, prior to using any hazardous materials on VSFB.

6.2. When applicable, the Contractor shall report their monthly hazardous materials usage for their activities on VSFB to the HAZMART. By reporting all monthly usage through the HAZMART, the contractor shall meet VSFBs reporting requirements for air emissions, EPCRA Toxic Release Inventory (TRI), solvent usage, and hazardous materials.

7. Government Furnished Items & Services.

7.1. Equipment/Material. The Contractor shall provide all equipment, materials, and tools required to perform work.

7.2. Utilities. Electrical outlets are available in most areas. The Contractor shall not change or modify any utility system or component; or connect any Government property, equipment or system without prior Contracting Officer review and writing approval authorizing change/modification. The Contractor shall practice energy and resource conservation in its use of Government provided utilities.

7.3. Security, Fire, And Medical Response Services. The Government shall provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis. Emergency services may be obtained by calling 9-1-1 from a base phone or 805- 606-3911 on a cellular phone. The Government will provide initial medical treatment for life-threatening emergencies. It shall be the responsibility of the Contractor or Contractor employee to seek continued medical treatment if needed. The Contractor or employee shall reimburse the Government for all Government-provided medical services.

8. Applicable Documents/Permits. The Contractor shall identify and comply with all applicable federal, state, and local statutes; Air Force/ Military instructions, manuals, handbooks, regulations, guidance, and policy letters; Executive Orders; ANSI/TIA/EIA; National Fire Protection regulations; National Electrical Code; Occupational Safety and Health Administration (OSHA) regulations; Environmental Protection Agency regulations; Unified Facilities Code, and International Building Code including all changes and amendments in effect on the date of issuance of each contract. It is the Contractor’s responsibility to identify and comply with all applicable and relevant requirements. It is the responsibility of the Contractor to identify all permits, clearances and other documents required for this project.

9. Security Requirements.

9.1. Accountability. At all times, while on Vandenberg SFB (VSFB) premises, Contractor employees shall comply with VSFB security requirements imposed by the Installation Commander. The Contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at VSFB. The Contractor shall be responsible for accountability of all such media issued to Contractor personnel and these media shall be surrendered to the 30th Security Forces Squadron (30 SFS) Pass and Registration Section upon completion or termination of the contract or upon termination of an individual’s employment with the Contractor. The Contractor shall comply with all VSFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions. Contractor employees are authorized entry to the installation only for the purpose of work under this contract and are not authorized in other facilities or areas without prior Government authorization. Due to safety and security requirements, Contractor personnel performing work on VSFB shall be required to fluently speak, understand, read, and write English.

9.2. Main Base Access. Entry requirements are subject to change, without notice, due to installation and world situations. For base access all contractor employees are subject to a background check through the Federal Bureau of Investigation. Current security access restrictions at the Santa Maria Gate (Main Base Entry Gate) require all Contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter only through the Lompoc Gate (located at intersection of Santa Lucia Road and Pine Canyon Road). There is a vehicle check pull-off area at that gate and the Contractor will be required to submit to a vehicle search prior to gaining entry to the base. Contractor should plan and allow for delays resulting from in-place security procedures.

9.2.1. Facility Access. The Contractor shall coordinate access to facility with the Facility Manager(s).

9.2.1.1. The Contractor shall brief all personnel that they shall immediately leave any area upon notification by the Contracting Officer, constituted authority, or the Security Forces, when directed, as occasional operational requirements, real world activities, or emergencies dictate.

9.2.1.2. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the 30th Security Forces Squadron (30 SFS) as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Due to safety and security, all Contractor personnel performing work on VSFB shall be able to fluently speak, read, and write English.

9.2.2. Contractor Vehicles. Each Contractor vehicle shall have the name of the Contractor exhibited on each side of the vehicle. The Contractor shall provide company identification tags and vehicle identification at no additional cost to the Government.

9.2.3. Physical Security. The Contractor shall be responsible for safeguarding all Government equipment, information and property provided for Contractor use.

10. Contractor Personnel.

10.1. Contractor personnel shall present a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company name and employee name.

10.2. Contractor personnel shall comply, and ensure that Sub-Contractor personnel comply, with current federal, state, local, Department of Defense (DoD), and Air Force regulations, plans, and policies including all applicable parts of the Air Force Occupational Safety and Health Standard (AFOSH) and United States Department of Labor Occupational Safety and Health Administration (OSHA).

10.3. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with (IAW) DoD 5500.7-R, "Joint Ethics Regulation (JER)", "Standards of Conduct," Dated August1993.

11. Invoicing.

11.1. In accordance with the contract clause at DFARS 252.232-7006, Wide Area WorkFlow Payment Instructions, the Contractor shall invoice and complete receiving reports electronically in the Wide Area WorkFlow (WAWF) application on the Procurement Integrated Enterprise Environment (PIEE) website (https://piee.eb.mil/). The Contractor shall notify the POC identified in the WAWF Acceptor upon submittal of invoice/receiving report for processing. The Contractor shall provide supplemental documentation (i.e. monthly billing statement) with the invoice/receiving report.
