About this file

This document provides the results of a hazardous materials assessment and limited asbestos and lead inspection for the Create Elevator Tower & Stairwell, Building 23 project at the Jefferson Barracks VA Medical Center in St. Louis, Missouri. The assessment identified asbestos-containing materials throughout the project areas, including floor tiles and mastics, thermal system insulation on pipes, stucco remnants under exterior walls, and window caulks. Lead-based paint was also found on walls, ceilings, windows, and other surfaces. The report recommends developing an asbestos abatement design specification and removing asbestos materials that may be impacted by construction. Any demolition or renovation of identified asbestos-containing or lead-based materials should follow regulated abatement procedures.

HAZARDOUS MATERIALS DESIGN NARRATIVE

VA Project Number: 657-19-128JB

Project Name: Create Elevator Tower & Stairwell, Building 23

Subject: Hazardous Materials Design Narrative

Date: May 21, 2020

1.0 Introduction

Riverfront Safety & Health, LC (Riverfront), a Service-Disabled Veteran Owned Small Business, has performed a limited asbestos & lead identification survey for the 95% Site Investigation phase of the Elevator Tower & Stairwell Project proposed at Building 23 (VA Project 657-19-128JB) at the Jefferson Barracks VA Medical Center in St. Louis, MO (The Project). The principal contacts for this project were Jacob Hunt of Spur Design and Brad Petrovic of the VA Medical Center. The following report is provided for your review and for future design considerations related to asbestos, lead, and mold issues associated with the proposed new elevator installation near

Building 23.

Justin Rhyneer and Gerald Reimer attended the VA sponsored project “kickoff” meeting held on

July 31, 2019 at Jefferson Barracks VA Medical Center. Three locations for the new elevator and associated vestibule/lobby and stairway were suggested by the VA. The proposed locations will be referred to as Location South (at tunnel corridor adjacent to and immediately south of Building

23), Location Central (at tunnel corridor north of Building 23 and adjacent to existing handicap entrance ramp), and Location North (area north of intersection of ground level tunnel corridors and located within area to be soon demolished as part of major projects East Demolition. The partial corridor and Building 4 are scheduled for demolition in late August allowing for the

Location North elevator option. The Project was described as facilitating customer access from the existing prosthetics upper parking lot to levels 1 and 2 of North/South corridors near Building 23 by way of new elevator. The proposed elevator with vestibule and associated stair tower were anticipated to be new structures connected to the east and west faces of corridors respectively with passageways into existing corridors. Additional areas where work may be performed and not reflected by the 65% drawings include Building 23 rooms G17, G07, and Building 1 Room GC-

12(I.T. Room), and associated corridors to provide for fire protection and communications.

The 65% design review proposed a new hybrid siting for proposed project. New construction area is to include original proposal option North and substantial portions of the North/South Corridors

(X001 & X104) from Building 23 north to building 4 demolition limits and ground level East/West

Corridor (X105A). The 95% DD review will likely include a temporary bypass tunnel which will result in less aggressive abatement schedules and allow for other “general” demolition options.

On July 31st, August 1-2, November 22, 2019 and January 2, 2020, the limited asbestos, lead, and mold survey was conducted by Missouri-licensed Asbestos Inspectors Justin Rhyneer, Gerald

Reimer, Zach Haselhorst, and Dennis Ida. The lead survey was conducted by Zach Haselhorst, a

Missouri-licensed Lead Risk Assessor. The survey was conducted with the following limitations:

• The Elevator North Location was unavailable for inspection as abatement/demolition activities were ongoing in adjacent Building 4 as part of the JB Major Project - East

Demolition. The above grade corridor X104, northern portion of Corridor x001, and stairwell were unavailable for inspection due to hard wall barriers restricting access.

• The 95% project scope is advancing. Locations of tie ins and associated routing of fire protection and low voltage communications/signaling is presented on drawings.

Assessment for material impact has been generally defined.

• The limited mold inspection consisted of visual observations only.

• Sampling collection was limited to non-destructive techniques. The possibility of a bituminous water proofing material exists within cavities of exterior walls. This material application exists at some locations on campus and has been occasionally detect for asbestos. Asbestos containing caulks and lead-based painted wood associated with window system may be located under other materials and has been addressed by Abatement

Drawings.

• Unknown quantities of asbestos pipe insulation and mudded fittings may exist above or inside plaster walls, ceilings, and chases. Access to Stairwell located at intersection of

North/South and East/West Tunnels has been boarded shut. It has been assumed asbestos pipe insulation on steam piping exists inside wall chase starting at lower level tunnel and supplying flush mounted heat convectors on walls of stairwell at tunnel level and Level 1.

No access to above plaster ceiling located in Level 1 N/S Corridor X104 was available. It is assumed no historic steam, chilled or domestic watering piping exists above plaster ceiling. This presumption was visually verified that pipes are not emanating from Building

23 into the north tunnel direction (Level 1).

• Access to the Level 1 Corridor and Stairwell Core have been sealed by partitions. Based on observations and historic information it has been assumed that the wall mounted heat convectors are supplied from steam piping in corridor at grade passing through floor and are uninsulated laterals with no residual asbestos in pipe sleeves. Steam pipe in tunnel below has been mostly abated.

• Estimated units of abatement include asbestos flooring, asbestos and lead window and door assemblies, lead plaster ceiling, roofing, asbestos stucco, and lead stabilization on concrete ceiling in Level 1 Corridor section from Building 21 to Building 4 demolition limits.

Prior to the field survey, the 1994 Roth Asbestos and Environmental Consultants, 2008 Altec

Environmental Consultants & The 2018 Riverfront Safety and Health Campus Surveys were reviewed for any previously identified asbestos-containing materials (ACM). Based on the 2008

Altec Environmental Consultants & 2018 Riverfront Safety and Health Campus Surveys and project area inspection, pipe insulation and associated mudded pipe fittings, 9” x 9” floor tiles, remnant black mastic on concrete floor, fire doors, stucco siding, and window systems were anticipated and verified as existing and assumed to be asbestos containing materials(ACM). It should be noted that a recent destructive window inspection project performed by others suggests that ACM caulking materials may exist under window sills or on frame components of the window such as “seal beads” within the walls. This report will assume that non-observed ACM caulk exists at window installations underlying other building materials.

Based on the age of the facility, prior survey results, and potential methodology of contract work, anticipated suspect materials included TSI and mudded fittings, floor tiles, remnant black mastic, fire doors, interior window caulks, unseen window framing caulks, structural framing caulks, glazing, ceiling tiles, waterproofing, asphalt roofing products, cove base/mastics, and Lead Based

Paint (LBP) on window components, walls, and ceilings.

This report summarizes the asbestos and lead-related issues identified with existing conditions for this project. The sampling results have been summarized in Tables 1 and 3, with the laboratory analytical report and sample chain of custody contained in Appendix A and B.

2.0 Scope of Work

Riverfront provided an AHERA/EPA trained Asbestos and Lead Inspector/Risk Assessor to perform the following:

- Perform an initial non-destructive survey to identify the presence, location, quantity, appearance, and condition of all suspect asbestos, lead, and mold within the Jefferson Barracks VA Medical Center in St. Louis, MO identified by the VA as likely to be impacted during installation of new elevator with lobby and associated stair tower near Building 23. Standard EPA/AHERA field sampling methods were employed to ensure representative sample collection during this initial assessment effort.

3.0 Limited Inspection Results

3.1 Asbestos Containing Materials - Inspection Results

All suspect asbestos materials sampled were classified as miscellaneous (e.g., floor tiles, mastic, ceiling tile, caulk, cove base, and glazing) or surfacing material in the cases of plaster, EFIS, and

Stucco Remnant. After classifying the suspect material, homogeneous materials (areas of uniform color, texture, and general appearance) were delineated. The number of samples collected from each area varied based on the classification of the material and increased as the potential for a non-uniform mixture of asbestos in the material increased. Typically, three samples per homogeneous area were collected, with less collected in the event of limited access to homogenous material areas. Sampling locations were chosen to be representative of the accessible materials and collected randomly from all accessible areas of the building where material was available without destructive collection procedures. All sampling procedures were conducted using guidelines established by the EPA. Samples were analyzed using polarized light microscopy (PLM) analysis with dispersion staining (DS) according to U.S. Environmental

Protection Agency (EPA) Method 600 IR-93/116 which is presently recommended by the EPA.

Asbestos Containing Materials are defined by the EPA as having greater than 1% asbestos as measured by PLM methods.

Riverfront collected a total of 71 representative samples of suspected asbestos-containing materials related to The Project. All samples were transferred via written chain-of-custody, to

EMSL, an AIHA & NVLAP accredited laboratory for analysis. Sampled indoor and outdoor building materials included plaster, EFIS, stucco remnant, floor tile, mastic, window glazing, caulk, fibrous expansion board, roof flashing caulk, roofing system, and ceiling tile. The suspect materials, which were similar in general appearance and age of construction, were grouped into homogeneous material sets and sampled.

Two material groups tested positive as asbestos-containing materials (ACM) during the surveys and will be affected by the project scope. The material groups are as follows:

• 12” x 12” White Vinyl Floor Tile and Associated Brown Mastic bulk sample results were both positive, each containing 4% Chrysotile asbestos. The estimated quantity of known affected materials was approximately 10 square feet each and located in the proposed Central Elevator location on Level 2. This floor tile and associated brown mastic was in damaged condition and located within corridor and adjacent to ramp access doors.

• Original Stucco Siding Under New EFIS System bulk sample results were positive, containing 7% Chrysotile asbestos. The estimated quantity of known affected material is approximately 2,000sf. This condition of the Stucco material is unknown and existing under the exterior wall EFIS system of the corridor structures. Stucco present on the soldier walls was in good condition and could be a different surfacing material. Due to non-destructive inspection restraints, two locations were observed at Building 23, each location with no visible underlying Stucco, however it is assumed that Asbestos Stucco is existing at Building 23. Additional inspection (possibly destructive) at Building 23 exterior EFIS is required to fully assess the extents of underlying asbestos containing Stucco. However, in three separate locations of N/S Corridor, Stucco samples were collected. The Stucco was present at all three locations (where EFIS covering was damaged and allowed for sample collection), therefore it is assumed Stucco substantially exists throughout elevation.

• Black Fibrous Expansion Board bulk sample results were positive, containing 18%

Chrysotile asbestos. The estimated quantity of known affected material was approximately

10sf. This condition of the material is good and existing between the base slab of the

Ground Level N/S Corridor and exterior footing of Building 23.

• Flashing Composite Roof bulk sample results were positive, containing 11% Chrysotile asbestos within a layer. The areas of flashing are sloped and located along the outer 18” on each side of the three roof lines. Selective locations for samples were chosen to protect the integrity of the roof. Areas beneath the edge capping system were not sampled. Since the roofs are narrow, less than 8’ wide, and the layer locations are undetermined, it will be assumed the asbestos layer may exist throughout. The condition of the material is good and existing beneath or within asphaltic built up roofing materials, flashing, and capping system.

During this inspection, seven material groups were presumed positive as asbestos-containing materials (ACM) based on a significant number of historic building samples from 1994 Roth

Asbestos and Environmental Consultants, 2008 Altec Environmental Consultants & 2018

Riverfront Safety and Health Campus Surveys have shown these materials to be asbestos containing and will likely be affected by the project work in some fashion. The material groups are presented in Table 2 and summarized as follows:

• 9” x 9” Asbestos Floor Tile is an ACM material (>1% Asbestos). This material was in good condition and 160sf is located at South Elevator position Level 2, 140sf at Central

Elevator position Level 2, 80sf is estimated at North Elevator position Level 2 (allow for

10 lf of slope adjustment of corridor floor), and 750 sf at Hybrid Option 4 underlying 12” x 12” non asbestos floor tiles.

• Remnant Asbestos Black Mastic Underlying Floor Tile is an ACM material (>1%

Asbestos). The estimated quantity of known affected material was approximately 320 square feet (20’x8’ @ 2 levels) at each proposed elevator locations South and Central and estimated 160sf at Elevator North. Black mastic is existing under tiles in all Corridor locations at proposed Hybrid 4 Elevator Option with an estimated quantity of known affected material of 1,800 square feet. This black mastic was is in good condition and underlies 9” x 9” and/or 12” x 12” floor tile groups in corridors. Locations of potential work in Building 1 and Building 23 such as floor/ceiling penetrations where/if necessary will require further assessment of impacted floor tiles and associated mastics where/if identified during design progression. Hidden Asbestos Caulks Associated with

Windows is a suspected ACM material that is likely concealed in locations such as on bottom aluminum window trim, frame caulk on aluminum to wood interior seams, caulk on the interior wood sill seams, or exterior seams (where wood/block meet) and should be assumed as existing on all perimeter windows. An estimated 60lf of hidden asbestos caulk was assumed for each window located in exterior wall.

• Fire Door and Frames is a suspected ACM material. They are in good condition with 3 fire doors located at the South Elevator position, 4 fire doors at the Central Elevator position, and no fire doors assumed at the North Elevator position.

• Asbestos Pipe Insulation (Air Cell & Cal/Mag) and/or Fiberglass Insulation with

Asbestos Mudded Fittings are presumed ACM materials (>1% Asbestos) based on review of previous sampling effort results as well as asbestos labeling on pipes observed in the field. All observed asbestos pipe insulation and mudded fittings were in good condition.

The estimated quantity of known affected asbestos TSI on pipe was approximately 40

LF/Aircell, 30 LF Cal/Mag-Block, and 8 mudded fittings at South Elevator Location level

2. This ACM is related to steam pipe supply of ceiling level heat convector. At level 2

Central Elevator Location, no asbestos pipe insulation was observed as upper corridor has a plaster ceiling. All level 1 project locations have steam, condensate, or chilled water supply (loop) pipe with asbestos insulation and mudded fittings and/or fiberglass insulation with asbestos mudded fittings mounted near ceiling level. Unknown chases with asbestos pipe insulation may exist in all locations and more so at the Elevator North Position as records suggest wall mounted heat convectors near Level 2 North Elevator position. It is estimated that 46lf total of asbestos pipe insulation and 22 mudded fittings will be impacted by Hybrid Elevator Option 4 at intersection of Gound level Tunnel (X001 and building 23) and stairwell chase located in Ground Level Tunnel X105A and Stairwell Level 1 landing.

• Black Waterproofing on foundation footing is a suspected ACM material. It is in good condition and exists at and below grade on the Building 23 foundation. This material will be treated as a presumed asbestos containing and will be impacted by rebar dowels during new foundation placement.

During this course of design development, other known or potential asbestos containing material groups, all related to utilities, exist below grade. The material groups are summarized as follows:

• Asbestos Chilled Water and domestic water lines are known to exist below grade in the area of new foundation placement. Spur has informed that these utilities exist at adequate depth below new foundation and will not be impacted by project work and/or not within the project scope of work. Based on site experience, the Chilled Water lines are known to contain asbestos and the domestic water line could possibly be impacted by an asbestos black tar coating. An approximate 50ft section of historic metal water piping will be removed and is assumed to have an outer coating of asbestos containing black tar.

• Vitrified Clay Pipe related to sewer or storm water will be removed due to placement of project foundations. These utility systems are not known to be impacted by asbestos in this area of the facility and will be assumed as non-asbestos containing for this project work.

Concrete, brick, brick mortar, ceramic tile mortar, vitrified clay blocks, pavement caulks, fiber expansion boards, red penetration caulk, plastic handrail and exterior window and expansion caulks were not sampled as a significant number of historic building samples from 1994 Roth

Asbestos and Environmental Consultants, 2008 Altec Environmental Consultants & 2018

Riverfront Safety and Health Campus Surveys have shown these materials to be non-detect for asbestos. Black bituminous waterproofing was observed on foundation of Building 23 and will be an assumed asbestos containing material.

A summary of asbestos sampling efforts, including sample numbers, descriptions, locations, quantities, category (friable or non-friable) and analytical results are presented in the following

Table 1. The survey and condition/risk assessment did not include an investigation of potentially buried piping within or in the vicinity of the structures. The full laboratory analytical reports are included in Attachment A. A summary of assumed ACM based on the 2008 Altec Environmental

Consultants & 2018 Riverfront Safety and Health Campus Surveys is presented in the following

3.2 Lead Based Painted Components & Potential Associated Impacted

Material Results

A targeted lead inspection was performed to identify lead-based paint and/or lead-based glazing on primary substrates potentially impacted by project work. The pathway for the new utilities from nearby locations in Buildings 1 & 23 is not known. A few sample readings were performed in level 1 corridor locations, where other utility routing was observed, to imply a general lead condition for potential penetrations through walls or attachment to corridor ceiling.

Riverfront collected lead readings from potentially impacted materials and targeted surfaces to identify the presence of lead-based paint (LBP), which is defined by the EPA as greater than or equal to 1.0 mg/cm2 lead by surface loading (as tested by an XRF instrument). EPA/HUD compliant XRF (X-Ray Fluorescence) sampling methods, which are non-destructive, were employed for this project assessment. Note that lead glazing finishes on ceramic tiles and blocks have been tested in the same manner as painted surfaces, with results held to the same standard of lead mass loading of 1.0 mg/cm2.

It is stressed that this sampling was intended to identify the presence of lead-based paint and/or lead glazed tiles and blocks within readily accessible areas of the building likely to become disturbed during The Project and associated with this particular project design, and not necessarily an EPA-HUD type “surface-by- surface” inspection. Readings were collected by an

EPA-HUD trained Lead Inspector/Risk Assessor.

Based on the 2008 Altec Environmental Consultants & 2018 Riverfront Safety and Health

Campus Surveys, it was anticipated that the level 1 areas within Corridors, Building 23, and

Building 1 would have lead-based paint on corridor walls, ceilings, any wood trim associated with external windows, and Stucco where present. It was anticipated that the ceilings and walls of the level 2 corridors would have spot locations with lead base painted surfaces.

Elevator Location South

Testing locations and XRF results are contained in Table 3. The Elevator South Location included corridor locations on Level 1, Level 2, Building 23 Room G17, and portion of south wall Building 23. A total of 56 XRF measurements, including 9 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. Building 23 Room G17 is included with Elevator South

Location where windows in south wall of Building 23 may be removed by The Project. Of these measurements, 17 readings are considered lead-based paint or ceramic glaze. The lead-based paint condition on material was noted to be intact with the exception deteriorated beige paint on concrete ceiling at Level 2 (located above the ceiling grid, sample 1491). In summary, lead-based paint exists on concrete columns, ceilings, window systems and expansion joint plates on Level 2 and plaster walls/ceilings throughout Level 1 and will be impacted by project work. Underlying Stucco remnants, where existing, were not tested for lead based paint.

Elevator Location Central

Testing locations and XRF results are contained in Table 3. The Elevator Central Location included corridor on Level 1, Level 2 and small section of adjacent stair tower. A total of 43

XRF measurements, including 7 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. Of these measurements, 5 readings are considered lead-based paint. The lead-based paint condition on material was noted to be intact with the exception deteriorated white paint on window frame at Level 1 adjacent exterior wall of stair tower (sample 1881). In summary, lead-based paint exists on plaster ceiling of corridor Level 2 and window systems at all levels will be impacted by project work. Underlying Stucco remnants, where existing, were not tested for lead based paint, but based on historical knowledge of this material present on adjoining

Building 1 (2008 Stucco Removal Project), it should be presumed positive and handled accordingly.

Elevator Location North

Location North was not accessible for survey due to adjacent abatement/demolition work at

Building 4. Based on the 2008 Altec Environmental Consultants & 2018 Riverfront Safety and Health Campus Surveys, the XRF results at this location would be similar to Elevator

Location South. It is anticipated that the project interface locations of ceiling and walls would be the locations where lead-based paint surfaces are encountered by The Project.

Elevator Location Hybrid Option 4

Location North was not accessible for survey due to adjacent abatement/demolition work at

Building 4. Based on the 2008 Altec Environmental Consultants & 2018 Riverfront Safety and Health Campus Surveys, the XRF results at this location would be similar to Elevator

Location South. It is anticipated that the lead based paint exists on plaster ceiling as referenced by (Sample 1491) in Level 1 N/S Corridor X104. Based on inspection of south elevator location and experience at nearby Building 1, the structural concrete ceiling above the lead painted plaster ceiling will have delaminating lead based paint some of which has fallen to the plaster ceiling below. Window trim will be assumed as positive for lead paint.

Building 23, Room G07, G02 & G01

Testing locations and XRF results are contained in Table 3. A total of 9 XRF measurements were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. Measurements for QA/QC purposes were collected during the survey during power on and off intervals throughout sample collection and are accounted in other locations within report. Of these measurements, 5 readings are considered lead-based paint and it is noted to be intact. In summary, lead-based paint exists on ceilings where sampled and walls of Room G07 and may be impacted by project work. Any penetrations through walls or ceilings of Room G07 or attachments to other ceilings will disturb lead-based paint.

Building 23, East/West Corridor & North/South Corridor

Testing locations and XRF results are contained in Table 3. Fire protection, power and communications utilities may be installed near ceiling level of corridor where cable trays or overhead suspension is not available. A total of 6 XRF measurements were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. Measurements for QA/QC purposes were collected during the survey during power on and off intervals throughout sample collection process and are accounted in other locations within report. Of these measurements, 1 reading is considered lead-based paint and is noted to be intact. Any penetrations or attachments to the ceilings in the East/West corridor will disturb lead-based paint.

Stairwell

Area was not accessible for survey as door entrances are blocked at ground level and level 1.

As observed through ground level exterior window, no floor tile was observed on landings or risers. A wall mounted steam convector was observed. Conditions for Level 1 were assumed to be the same as observed at ground level.

Building 1, Level 1 Corridor and IT Room GC-12

Testing locations and XRF results are contained in Table 3. Communications utility may be installed at ceiling level of corridor where cable trays or overhead suspension is not available.

A total of 5 XRF measurements, including 3 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials possibly impacted by project work. Of these measurements, no readings are considered lead-based paint. The ceiling in this corridor was not accessible for testing due to height restrictions. It is anticipated that the ceiling where used for attachments would impact lead-based paint. Access into the I.T. Room was not available during the inspection. Based on the 2008 Altec Environmental Consultants

& 2018 Riverfront Safety and Health Campus Surveys, the walls and ceiling will have lead-based paint. Any penetrations or attachments to the surfaces will disturb the lead-based paint.

A summary of lead results for The Project is provided below:
