Attachment #3
Submission Requirements. Offerors shall provide the following submissions for each course being proposed.
(a) Technical, Past Performance and Price Submissions. Offerors shall submit an original (1) and two (2) hard copies of complete proposals, PLUS one (1) electronic copy of the complete proposal on CD/DVD(s).
Electronic copies may not be submitted on flash drives/thumb drives, as their use is prohibited on the government’s computers. Hard copy submissions shall be provided in binders, with a separate divider for each Volume, and binder/divider/tab titles as indicated in the table below. CD/DVD(s) shall include folders for each Volume; with folders/sub-folders/file names as indicated in the second column in the table below. All information that is included within the hard copy binders MUST also be included on the CD/DVD(s). Offers shall clearly indicate the solicitation number on all volumes of the proposals. Failure to provide complete proposals in both hard copies and electronic copy will render the proposal non-responsive and it may not be considered for award.

Volume I: Course Content Course Content General Note: Proposals shall:
1a Course Overview, Performance
Objectives, and Training Approach
Narrative of course overview, performance objectives;
and training approach, to include, but not be limited to, the number of instructors provided for each iteration, and method(s) of instruction (lecture, practical exercise, etc.)
to be utilized in conducting the course.
1b Course Agenda Course Agenda shall be broken down by day and hour.
1c Topic Overview A brief overview of what will be covered for each topic listed in the course agenda.
1d Slide Sets (if used) Slide sets shall be numbered in order of presentation.
Presentation order should correspond with Course Agenda. (MCTC logo will be required on each slide if selected for award; however the logo is not required on the slides submitted with the proposal.)
Slide sets may be printed OR provided on a CD/DVD (NOT flash drives) placed in the appropriate location within the hard copy binders.
1e Videos (if used) Provide electronic copies of videos and identify where they are used within the course agenda (or indicate which slides they’ll be embedded in). All videos must be viewable using Windows Media Player™. Videos may be provided on a CD/DVD (NOT flash drives) placed in the appropriate location within the binder.
1f Practical Exercises (if used) If Practical Exercises will be used, offeror shall provide a list of the exercises, identify the course topic(s)/learning objectives exercised, and a brief explanation of how each exercise will be conducted.
1g Pre-test and post-test, if required by the PWS
Pre-tests and post-tests may be written, practical exercise, or a combination of the two. Provide a copy of all written tests or segments thereof. Provide a brief explanation of how practical tests will be conducted (if used). If graded, indicate the minimum passing score.
1h Student handbook (required) and other reference materials (optional)
Student handbook must be specific to this course.
Offeror shall list any commercially-published references to be provided as part of student take-home materials, including ISBN #, Title, Publisher (hard copies of commercially-published references are not required with the proposal). For each reference, indicate if it will be provided to students in hard copy or electronic format.
1i Contractor-provided materials/equipment
Offeror shall provide list of materials and equipment they will provide to facilitate the course – do NOT merely indicate that “materials listed in PWS will be provided” or similar statement.
1j Student-provided materials/equipment
Offeror shall provide list of materials and equipment that students must bring to participate in the course, if applicable.

Volume II: Offeror/Instructor Qualifications
Volume II of the proposal shall identify the offeror (prime contractor and subcontractors/teaming partners)
2a1: Offeror Qualifications Prime Contractor Qualifications Provide a narrative of the Prime Contractor’s qualifications to provide the training as described within the PWS. If PWS indicates a specific certification, license, or other formal accreditation is required, provide a copy of that certificate, license, or accreditation document.
2a2: Offeror Qualifications Subcontractor/Teaming Partner Qualifications
Provide a List of Subcontractors and/or Teaming Partners, and the roles/responsibilities they will be responsible for. Provide narrative to support their qualifications to perform their areas of responsibility.
2b1: Instructor Qualifications
List of proposed instructors Offerors shall submit only instructors who meet all mandatory qualification requirements and who they plan to actually use to instruct the course for consideration.
2b2: Instructor Qualifications List of Qualifications Offerors shall submit a detailed list of qualifications (not a resume) that are pertinent to the PWS requirements (i.e., if PWS requires supervisory, or other specialized experience or specific tenure in a certain position, clearly indicate such experience in the list of qualifications; do not assume the Government knows the duties/responsibilities associated with specific ranks or titles). NOTE: Please do NOT submit resumes; they will not be read or considered.
2b3: Instructor Qualifications
List of Courses Taught (for each proposed instructor)
Provide a detailed course list to demonstrate your capability and capacity to teach this particular course.
List only courses that are relevant (similar length, content, target audience) to the course being proposed.
List shall include course titles, dates, locations, and hosting agencies/organizations.
2b4: Instructor Qualifications Qualification Documents (for each proposed instructor)
Provide copies of document that supports the qualification indicated (i.e. if a specific certification, license, or other formal accreditation is required, provide a copy of the document specified.) If a substantiation document is listed in the PWS, a copy must be provided in order for the instructor to be credited with the qualification in the evaluation.

Volume III: Past Performance Offeror’s Past Performance
References Volume III of the proposal submission shall include a list of all companies/agencies that offeror has requested to submit PPQs, including:
a. Company/Agency Name (Offeror)
b. Point of Contact (POC) Name (Offeror)
c. Company/Agency address (Offeror)
d. Company/Agency phone number and POC direct number or extension, if applicable (Offeror)
e. POC email address
f. Brief description of services (courses) provided by Offeror to the Company/Agency Of
g. Date(s) of services provided by Offeror to the Company/Agency
h. Indicate whether Offeror was prime contractor or subcontractor for referenced services
The Government must receive a minimum of three (3) but no more than five (5) Past Performance Questionnaires (PPQs) to evaluate past performance.
Offerors shall provide PPQs to companies/agencies for whom they have performed relevant (similar training content, audience type) services within the past three (3) years. Offeror shall complete the “Offeror” information on the PPQs before sending to their references. Although completed PPQs must be submitted by the referring company/agency directly to the Contract Specialist, offeror is responsible for following up to ensure that the minimum number of PPQs have been submitted prior to the proposal due date/time.
Volume IV: Price Proposal 5a: Price Proposal Price Proposal Offeror shall provide a Firm Fixed Price (FFP), per course iteration, for each Contract Line Item (CLIN) of this solicitation. The FFP shall be inclusive of all labor, materials, equipment, mailing/shipping, and travel required to perform the training. Contractor Manpower Reporting (CMR) CLINs may be priced or identified as “not separately priced (NSP).”
5b: Representations and Certifications
Representations and Certifications
Offeror shall complete all representations and certifications within their System for Award Management (SAM) profile at http://www.sam.gov. Any provisions listed within the “Full Text Provisions and Clauses” document which are not completed within SAM must be printed, completed and included with the proposal.

(b) Technical proposals shall clearly document, in the offeror’s own words, how the offeror will perform the services/training required by the PWS. Proposals that merely mimic language from the RFP/PWS will result in a negative technical rating.
(c) Offeror is responsible for ensuring that the USPFO P&C has received all proposal materials (including Past Performance Questionnaires) prior to proposal due date/time. Recommendation:
Proposals should be shipped using a trackable delivery method, and tracking number(s) emailed to the Contract Specialist prior to the proposal due date.
(d) Proposal materials will NOT be returned.
