About this file

This is a Privacy Act Statement form required by the Air Force Research Laboratory for federal grant applications and related research agreements.

The statement outlines the legal authority for collecting applicant information, citing multiple federal statutes and regulations including the Government Paperwork Elimination Act, the CHIPS and Science Act, National Defense Authorization Acts, and various Department of Defense directives. The collected information may be used for processing, investigating, and maintaining records related to Federal Grants, Cooperative Agreements, Contracts, Other Transaction Agreements, and other research-related agreements awarded by the Department of the Air Force. Routine uses of the information include sharing with contractors, grantees, experts, consultants, and others performing work on federal agreements, as well as disclosure to law enforcement authorities when records indicate a violation or potential violation of law. The statement specifies that providing information is technically voluntary; however, 42 U.S. Code § 6605 imposes mandatory disclosure requirements for federal research and development awards, with noncompliance subject to civil or criminal penalties, including rejection of applications.

The form requires signatures and printed names from both a covered individual and the institution's authorized representative, along with the proposal title and institution name. The document serves as an acknowledgment of consent to the privacy practices and information collection described herein.
