About this file
This is an Annual Required Affidavit for Bidders, Offerors and Contractors issued by the Commonwealth of Kentucky pursuant to Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit must be completed and notarized by an authorized representative of any bidder, offeror, or contractor seeking to be awarded a contract or renew an existing contract with the Commonwealth. The affidavit is effective for one year from the date of execution. Bidders and offerors are required to attest that the contracting party has not knowingly violated Kentucky's campaign finance laws and that awarding the contract will not violate any provision of those laws. "Knowingly" is defined as awareness or constructive awareness of a violation.
The affidavit requires the bidder or offeror's authorized representative to provide their signature, printed name, title, and date, along with the bidder or offeror's name, address, and Kentucky vendor code if known. The attestation must be subscribed and sworn before a notary public, who must provide their name, signature, title, the state and county of notarization, commission expiration date, and the date of notarization. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. This document serves as a compliance mechanism to ensure all contractors meet Kentucky's campaign finance law requirements.
