About this file
This is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky under KRS 45A.110 and 45A.115. The affidavit is a mandatory document that all bidders, offerors, and contractors must submit to be awarded a contract or for contract renewal. An authorized representative of the contracting party must complete the attestation, obtain notarization, and return the completed affidavit to the Commonwealth. The affidavit is effective for one year from the date of execution. There are no specific project deliverables, quantities, response dates, or award dates detailed in this document, as it serves as a standardized compliance requirement rather than a project-specific solicitation.
The affidavit requires the authorized representative to swear under penalty of perjury that the contractor has not knowingly violated Kentucky campaign finance laws and that contract award will not violate any provision of those laws. "Knowingly" is defined as awareness or reasonable awareness of a violation's existence. The document requires the bidder or offeror to provide their legal name, address, and Commonwealth of Kentucky Vendor Code if available. The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. The attestation must be notarized before a notary public, with the notary providing their signature, commission expiration date, and the date and location of notarization.
