About this file
This is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky under Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit is effective for one year from the date of execution and must be completed and notarized by an authorized representative of any entity seeking to be awarded a state contract or to renew an existing contract. The document serves as a certification that the contractor has not knowingly violated Kentucky's campaign finance laws and that contract award will not result in violations of such laws. The affidavit requires the bidder or offeror to provide company name, address, and Kentucky vendor code information, along with the authorized representative's signature, printed name, title, and execution date.
The affidavit must be notarized before a notary public, with the notary providing their signature, commission expiration date, and the date and location of notarization. The Commonwealth of Kentucky retains the right to request updated affidavits at any time during the contract period. For purposes of the attestation, "knowingly" is defined as the bidder or offeror being aware or should have been aware of a violation's existence. This document is a mandatory compliance requirement for all bidders and contractors doing business with the Commonwealth and must be submitted prior to contract award or renewal.
