The Commonwealth reserves the right to request an updated affidavit at any time during the contract period. The completed form must include the bidder or offeror's name, address, Commonwealth of Kentucky vendor code if known, the authorized representative's signature, printed name, title, and execution date. The attestation must be notarized, with the notary providing their signature, commission expiration date, and identifying the state and county where the notarization occurred. This affidavit applies to all bidders and offerors regardless of contract value or type and serves as a prerequisite condition for contract eligibility.
