About this file

This is a Required Affidavit for Bidders, Offerors and Contractors issued by the Commonwealth of Kentucky pursuant to Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit is effective for one year from the date of execution and must be completed and notarized by an authorized representative of any bidding or contracting party seeking to be awarded a contract or the renewal of an existing contract with the Commonwealth. The form requires the authorized representative to attest under penalty of perjury that the contractor has not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any provision of those laws. The term "knowingly" is defined as awareness or should-have-been-aware status regarding any violation.

The affidavit requires completion of contractor identifying information including name, address, and Commonwealth vendor code, along with the signature, printed name, and title of the authorized representative and the execution date. The document must be notarized before a notary public, with the notary attesting to the state, county, date of notarization, and commission expiration date. The Commonwealth retains the right to request an updated affidavit at any time during the contract relationship. This document serves as a compliance requirement for participation in state contracting opportunities rather than a solicitation for specific goods or services.
