About this file

This is a budget template spreadsheet (Annex 1) designed for applicants to submit detailed cost proposals for the USAID SOROKA program in Madagascar. The template requires a comprehensive breakdown of costs across multiple categories including personnel, fringe benefits, travel, equipment, supplies, contractual services, construction, and other direct costs, with separate worksheets for prime contractors and subawardees. The template must be completed for each year of the 5-year performance period.

The related grant opportunity (72068724RFA00008) is for SOROKA (Surveillance, Operational Research, and Optimization of Knowledge-sharing for Action), a $7.5 million cooperative agreement running from October 2024 to September 2029. Administered by USAID Madagascar under CFDA 98.001, the program aims to improve public health through enhanced data collection, management and capacity building within Madagascar's Ministry of Public Health. Focus areas include malaria, maternal/child health, water/sanitation/hygiene (WASH), and global health security, with emphasis on surveillance, emergency response, and research activities.
