Final acceptance occurs after successful installation, operational testing, staff training completion, and submission of all required documentation. The contractor shall provide manufacturer standard warranties for all installed equipment and workmanship.

STATEMENT OF WORK (SOW)
Project Title:
Central Sterile Room (CSR) Processing Department Equipment Replacement and Installation Facility:
Hopi Health Care Center Polacca, Arizona
1. PURPOSE
The purpose of this procurement is to provide all labor, materials, equipment, supervision, transportation, installation, testing, training, and related services necessary to replace existing Central Sterile Room Department (CSR) equipment at Hopi Health Care Center. The project will support continued compliance with healthcare sterilization, infection prevention, and patient safety requirements.
2. SCOPE OF WORK
The Contractor shall furnish and install the following equipment and associated components:
a. Two (2) AMSCO 400 Sterilizers
• Model: 20X20X38 SFPP Single Sliding Cabinet Electric
• Electrical Requirements: 480V
b. One (1) AMSCO 53 Reprocessing Sink
• 120-inch Triple Bay configuration
• Stainless steel construction suitable for sterile processing operations
c. One (1) Pass-Through Window Assembly
d. One (1) AMSCO 7052HP Washer/Disinfector
• Electric Heated
• 460-480V, 3 Phase, 60 Hz
3. CONTRACTOR RESPONSIBILITIES
The Contractor shall:
3.1 Provide all labor, supervision, tools, materials, equipment, transportation, permits, and incidentals necessary to complete the work.
3.2 Remove and dispose of existing sterilization and reprocessing equipment in accordance with all applicable federal, state, local, environmental, and healthcare regulations.
3.3 Coordinate all delivery and installation activities with the Hopi Health Care Center Point of Contact (POC).
3.4 Install all equipment in accordance with manufacturer specifications and applicable healthcare facility standards.
3.5 Provide all required utility connections, including but not limited to:
• Electrical
• Plumbing
• Steam
• Drainage
• Ventilation connections as required
3.6 Install pass-through window assembly in designated wall opening and ensure proper fit, finish, and operation.
3.7 Perform startup services, calibration, operational testing, and system verification for all installed equipment.
3.8 Provide manufacturer-authorized installation services for all STERIS equipment.
3.9 Conduct on-site operational and basic maintenance training for hospital personnel.
3.10 Provide all operating manuals, warranty documentation, maintenance documentation, and closeout paperwork upon project completion.
4. GOVERNMENT-FURNISHED PROPERTY
The Government shall provide reasonable access to the worksite and designate a Point of Contact for project coordination.
5. PERIOD OF PERFORMANCE
The Contractor shall complete all work within the timeframe established in the awarded contract after receipt of Notice to Proceed (NTP).
6. PLACE OF PERFORMANCE
Hopi Health Care Center Polacca, Arizona
7. HOURS OF WORK
All work shall be coordinated with the Government Point of Contact. Work shall be performed during normal business hours unless otherwise approved by the HHCC or COR.
8. INSPECTION AND ACCEPTANCE
The Government reserves the right to inspect all work performed under this contract. Final acceptance shall occur after:
• Successful installation
• Operational testing
• Staff training completion
• Submission of all required documentation
9. SAFETY AND COMPLIANCE
The Contractor shall comply with:
• OSHA requirements
• Infection control requirements
• Hospital safety policies
• Applicable building and electrical codes
• Manufacturer installation standards
• Federal, Tribal, State, and local regulations
10. WARRANTY
The Contractor shall provide manufacturer standard warranties for all installed equipment and workmanship. Warranty documentation shall be provided to the Government at project completion.
11. DELIVERABLES
The Contractor shall provide:
• Fully installed and operational equipment
• Startup and testing reports
• Training documentation
• Warranty information
• Final closeout documentation
12. POINT OF CONTACT
The Contracting Officer (CO) will oversee contract award and administration. A designated COR will monitor performance, coordinate delivery, and confirm acceptance of deliverables.
13. PROJECT COMPLETION
The contractor shall coordinate scheduling, delivery, installation, and commissioning with Hopi Health Care Center representatives to ensure successful project completion and operational readiness.
