Deadline for completed Base Pass request is 25 Apr 2022 by COB. All participants are to report to Building 932 located at 4380 Alabama Road, Moody AFB, GA. ****INSTRUCTIONS TO OFFERORS SECTION****

Addendum to FAR 52.212-1 Instructions to Offerors - Commercial Products and Commercial Services.

A. Quotations shall contain the following information: RFQ Number; Time Specified for Receipt of Quotations;

Name, Address, and Telephone Number of Quoter; System for Award Management (SAM) DUNS Number and CAGE Code; Name and Email address of Representative authorized to discuss quote; Any Discount Terms; and Acknowledgement of all Solicitation Amendments (if applicable). Quotations shall also include the following required documentation:

B. Submission of Offers. The contractor(s) are responsible for submitting a quote in response to this solicitation and shall be completed and sent to the following email addresses; victor.houston.1@us.af.mil, mary.twitty.1@us.

af.mil and 23CONS.LGCB@us.af.mil. The subject line of the email will contain the solicitation number (FA483022Q0007) and your company name. If it is necessary to send more than one email, the subject line of the email will contain the solicitation number, the company name, and the sequential order of the email, and the total number of emails to be received. For example, an offeror who would be sending two emails in response to the request for quote would have the following subject lines.

C. It is the responsibility of the offeror to ensure the quote and all required attachments must be received at all the above email addresses prior to the close date and time of the solicitation. Amendments must be acknowledged in writing or the proposal may be ineligible for award. It is the offeror's responsibility to check SAM.gov for amendments issued against this solicitation and respond accordingly.

1. Price. The offeror shall submit proposed pricing in Attachment 2 Pricing Worksheet. Offerors rejecting the terms and conditions may be excluded from consideration.

Note: Wage Determination 2015-4493 Rev 17, dated 12/27/2021, at Attachment 5, applies.

2. Past Performance Information (PPI). PPI will utilize past performance questionnaires (Attachment 6).

Additionally, the government may use data independently obtained from other government and commercial sources.

(a) The offeror shall submit evidence of recent and relevant contracts performed for custodial service in the last five years. A maximum of three (3) contracts may be submitted. The offeror must use the Past Performance Information Form (Attachment 8) to submit past performance information. Refer to "Evaluation Factors for Award Section" for determining if the contracts you propose to submit meet the definitions of recent and relevant.

(b) Offerors are required to thoroughly explain what aspects of the contracts are deemed relevant to the proposed Base Custodial Services. Specifically relate them to sections and paragraphs within the Performance Work Statement.

(c) Offerors are required to thoroughly explain corrective actions taken in the past, if any, for substandard performance and any current performance problems.

3. Technical. The offeror must submit a technical approach that clearly demonstrates a complete and realistic plan to meet the requirements in accordance with (IAW) the Performance Work Statement (PWS) (See Attachment 1). The entire technical approach must include the following documents:

(a). Scheduling Plan (b). Resource Allocation Plan (c). Essential Services Plan for Level 4 Facilities

D. Period for acceptance of offerors. The offeror agrees to hold the prices in its quote firm for 90 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

1. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

2. Late submissions, modifications, revisions, and withdrawals of offers. Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation.

Evaluation Factors for Award

****EVALUTION FACTORS FOR AWARD SECTION****

Addendum to FAR 52.212-2: Paragraph (a) is tailored as follows:

Evaluation:

IAW FAR 52.212-2, Evaluation-Commercial Products and Commercial Services. The Government will award a contract to the quoter whose quotation is judged to represent the best value to the Government based on the following factors, they are:

1. Price

2. Past Performance

3. Technical

BASIS FOR CONTRACT AWARD: This is a competitive, best value continuum using the Past Performance Tradeoff (PPT) source selection process in which competing offerors past performance history will be evaluated on a basis significantly more important than price. Technical elements will be evaluated as acceptable or unacceptable for each proposal. Qualifications will be clear and objectives will be outlined in the RFQ. If after evaluating all offers and no offeror has a substantial confidence rating, the CO may award to the first Offer that received a Satisfactory Confidence and Acceptable Technical Evaluation rating representing the best value to the Government. The evaluation process shall proceed as indicated below. Evaluation Approach:

Step 1: Initially, offers shall be evaluated for acceptability, completeness and timeliness IAW Addendum FAR 52.212-1.

Step 2: Offerors will be ranked by price from the lowest to highest and then proceed to next step.

Step 3: Assess past performance confidence for the two lowest priced offers. If determined to have Substantial

Confidence, proceed to next step. If any of the two offeror's past performance is less than Substantial Confidence, the next offeror will be evaluated and so on until one Substantial Confidence offer is determined.

Step 4: Technical capability will be evaluated on an acceptable or unacceptable basis for offerors that successfully make it through steps 1-3 above. If after evaluating all offers and no offeror has a substantial confidence rating, the CO may award to the first Offer that received a Satisfactory Confidence and acceptable Technical Evaluation rating representing the best value to the Government.

Step 5: The Government will make an integrated assessment to determine best value and award decision where past performance is significantly more important than price. After this integrated assessment an award decision will be made.

Evaluation Factors for Award:

Price Foreword: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

1. PRICE:

a.) Initially, offerors shall be ranked from lowest to highest according to the total evaluated price (TEP). In Attachment 3, Pricing Worksheet, the Contract Line Item Number (CLIN) price for each CLIN (with the exception of CLIN's 0010, 1010, 2010, 3010, or 4010 which will not be evaluated for price) will be determined by multiplying the CLIN Unit Price by the CLIN Quantity. The CLIN prices for each period of performance, including the basic period and each option period, will be added together to determine a price for each period of performance. These annual prices will then be added to arrive at a total evaluated price.

b) Price: 52.217-5 Evaluation of options applies to this evaluation. A potential six-month extension IAW FAR 52.217-8 will be evaluated at the pricing of the final option year divided by two. In the event of an extension under this clause, it will be exercised at the prices of the prior contract year. Evaluation of options will not obligate the Government to exercise the option(s).

c) The total evaluated price will be evaluated to determine whether it is reasonable. A reasonable price/cost represents a price/cost that provides best value to the government, taking into account market prices, the technical and functional capabilities of the offeror, and risk. Additionally, the government reserves the right to, but is not required to, conduct a price realism analysis on offers that appear to be exceptionally low and not supported by the offeror's technical approach. In instances where an offeror's proposed price(s) appear exceptionally low, offerors may be requested to address this disparity by providing evidence of their capability and/or performance at similar price(s). The burden of proof as to price credibility rests with the offeror. An exceptionally or unrealistically low offer that's left unexplained may pose an unacceptable risk to the government and may be a reason to reject an offeror's proposal.

d) The government anticipates adequate price competition and intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest.

2. PAST PERFORMANCE:

a) The Government will seek recent and relevant performance information based on (1) the past efforts provided in the proposal and (2) data independently obtained from other Government or commercial sources in order to determine the offeror's Performance Confidence Assessment, which is comprised of three factors: Recency, Relevancy, and Quality.

b) An assessment of the past performance information will be made to determine if it is recent. Recent contracts are: (a) contracts which have been performed within the last 5 years from the date of issuance of this solicitation, or (b) contracts that are currently being performed with a performance record of at least 6 months from the date of issuance of this solicitation.

c) Relevant performance includes performance of efforts involving Custodial Services that involved similar scope/complexity, magnitude of effort, contract type and contract environment this solicitation requires, as identified in Table 2. Scope/complexity is more important than magnitude; magnitude is more important than contract type and contract environment and contract type are equal. In evaluating past performance, the government reserves the right to give greater consideration to information on those contracts deemed most relevant to the effort described in this solicitation.

d) The Government will consider the performance quality of relevant efforts that are recent. The quality assessment will consist of an evaluation of the past performance questionnaire responses, PPIRS information, and CPARS information. It may include interviews with Government customers, interviews with commercial clients, and interviews or other sources known to the Government. The quality assessment may result in positive or adverse findings.

Offeror's may be asked to clarify certain aspects of their proposal (for example, the relevance of past performance information) or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system.

f.) Next, the Performance Confidence Assessment Rating will be assigned in accordance with Table 5 ****See Attachment 9 - Evaluation Factor Tables (Look for Table 5: Performance Confidence Assessments)

3. TECHNICAL:

a) The Government will evaluate the technical proposal, on an acceptable or unacceptable basis, of the total evaluated priced offers. If after evaluating all offers and no offeror has a substantial confidence rating, the CO may award to the first Offer that received a Satisfactory Confidence and Acceptable Technical Evaluation rating representing the best value to the Government.

Offerors must submit a Technical Approach that clearly demonstrates a complete and realistic plan to meet the requirements of the solicitation. The technical approach must include:

b) Scheduling Plan: Attachment 9, Scheduling Plan, must be completed and submitted as part of the offerors technical approach. (Not to exceed 20 pages. Any additional pages will not be evaluated)

c) Resource Allocation Plan: The offeror must submit a Resource Allocation Plan as part of the offerors technical approach. The Resource Allocation Plan must include a list of all vehicles, equipment for cleaning high windows and dusting rafters, and electromechanical floor maintenance equipment that will be used in performance of the requirements of the PWS.

(Not to exceed 10 pages. Any additional pages will not be evaluated)

d) Essential Services Plan for Level 4 Facilities: The offeror must submit an Essential Services Plan as part of the offerors technical approach. Custodial Services provided to the Child Development Center (CDC), the Youth Center, and the Fitness Centers are designated as an Essential Service in accordance with (IAW) the clause DFARS 252.237-7023. The offeror shall therefore include an Essential Services Plan, IAW DFARS clause 252.237-7024, explaining how the offeror will undertake steps to maintain the service in the event of a qualifying event or emergency. The plan must address; (Not to exceed 10 pages - Any additional pages will not be evaluated).

i. Challenges associated with maintaining essential contractor services during an extended event, such as a pandemic that occurs in repeated waves;

ii. The time lapse associated with the initiation of the acquisition of essential personnel and resources and their actual availability on site;

iii. The components, processes, and requirements for the identification, training, and preparedness of personnel who are capable of relocating to alternate facilities or performing work from home;

iv. Any established alert and notification procedures for mobilizing identified "essential contractor service" personnel; and

v. The approach for communicating expectations to contractor employees regarding their roles and responsibilities during a crisis.
