AMENDMENT 0001 - QUESTIONS AND ANSWERS
RFP# 75F40123R00019
1. Can the Government advise whether this is an existing contract or a new requirement? If this is an existing contract, can the incumbent and contract number be provided? Yes, there is an existing Contract with American Biomedical Group, Inc. under Contract No.: HHSF223201710045I.
2. The first sentence states "All Contractor personnel are required to conduct coordination of repair and maintenance services at the Contractor’s site.". The first sentence of the next paragraph states "Contractor shall arrange for repairs to be performed by the Original Equipment Manufacturer (OEM) and OEM-qualified third-party repair providers at the following FDA facilities:". Can the Government confirm contractor work will need to be performed at both FDA Centers/facilities and at the contractor's location? The Contractor shall be required to perform coordination of the repairs (calling the repair provider, arranging for a purchase order, confirming completion of the repair, and paying the repair provider, etc) at their own facility. The repairs, whether conducted by OEMs, OEM-qualified third-party providers or the Contractors own OEM-qualified repair personnel, shall be performed at FDA locations. The only time the Contractor would perform repairs at their own site would be if they were OEM-qualified to perform depot repair on certain types of equipment.
3. Can the Government clarify how it would like to see Sections 1 through 5 within Volume 1 as separate "tabs" within the Volume? The sections 1 through 5 reference the following which shall be addressed under separate tabs in Volume 1: Factor 1: Technical Approach to IDIQ, Factor 2:
Project Management Approach, Factor 3A: Relevant Experience, Factor 3B Past Performance, and Factor 4: Key Personnel.
4. Will the Government accept headers or footers that include "Proposal - RFP 75F40123R00019" text or will this count against the 1" margin on all sides? Yes, headers and footers are acceptable and will not count towards the 1” margin.
5. Will the Government consider excluding signed Contractor Teaming agreements from the proposal response's 50 page limit? Yes.
6. Can the Government confirm that Relevant Experience projects submitted by the Prime offeror for this effort can include projects on which the Offeror served as a sub-contractor so long as the project is similar in size, scope, etc.? Please reference Section E.3.3 in the solicitation as stated:
The prior experience of a sub-contractor may not be used as the Offeror’s relevant experience unless the sub-contractor is also listed in the submitted Contractor teaming agreement.
7. The RFP reads "For all contracts a Past Performance Questionnaire, Attachment D-13_ Past Performance Questionnaire shall be completed and submitted" - is the PPQ limited to the contracts used for the Relevant Experience above in Factor 3A? Please reference Section F.2.3 in the solicitation as stated: The past performance factor considers each Offeror’s demonstrated recent and relevant record of performance in providing services that meet contract requirements.
8. Is the Government expecting a text with completed SF1449 file as well as the Attachment D-7 xls (i.e. 2 files) as the complete Business Proposal Volume? Yes.

9. Where would the Government like to see the Reps and Certs, i.e. combined with the Technical Volume or as a separate file? If combined with the Technical Volume, will this count against the 50 page maximum? No, Offeror Representations and Certifications-Commercial Products and Commercial Services will not be counted against the solicitation page count.
10. In reference to the solicitation document ii 2. and iii. seem to have a bit of competing language. Provided there is an escalation path to the OEM, if needed, would OEM “qualified” technicians be appropriate? Based on the question as stated, the FDA does not understand what is being asked. The reference to document ii 2. and iii. are not a part of the current solicitation attachment labels.
11. Can you please provide the incumbent information to include the awarded vendor, contract award amount, award ID, and awarded date? Please see response to question number one.
