SECTION C – SCOPE OF WORK

FY2016 SITE PREPARATION AND RELEASE PROJECT

EL YUNQUE NATIONAL FOREST SERVICE

C.1 DESCRIPTION OF WORK

The Contractor shall provide Water system operation services for El Portal water system and El Yunque Recreation Area water system at El Yunque National Forest (EYNF). The work must be performed by a water operator licensed in Puerto

Rico, lic. Level 2.

C.2 PROJECT LOCATION

El Portal water system consists of a slow-sand filtration plant with a surface water source which supplies potable water for

El Yunque Headquarters office building and El Portal Visitor’s Center building. The filtration system operates entirely by gravity. The plant is located at the intersection of road PR 191 with PR988. The water intake is located inside of the forest and is accessed by a service trail.

El Yunque Recreation Areas system operates two deep-wells and pump stations. This system supplies recreation area at

El Yunque NF, road PR191. The system located along road PR191, starting from yokahu tower (Km.8.9 approx.) and ending approximately at Km 12.7. The system has two water reserve tanks and distribution lines that are located along forest trails.

Government Furnished Property. The Forest Service shall supply chlorine and any other water disinfection products needed and other materials as described in 2.2 C.

Contractor Responsibility

a) The Contractor is responsible for transportation to the worksite each day and for the prompt reporting to work and attendance.

b) The contractor shall access the work areas using his/her own vehicle. The contractor must ensure that the vehicle used is capable of reaching all the work areas within the scope of this contract. The government will not provide government’s vehicles to the contractor.

c) The Contractor is responsible for all Federal and State tax liabilities. The Government will not withhold any money from Contractor's worker pay and will not provide any benefits.

d) The Contractor is responsible for obtaining all necessary licenses, permits, and for complying with any applicable Federal, State, and municipal laws, codes, and regulations in connection with the execution of the work.

e) The Contractor is responsible for adhering to all safety requirements of the Forest Service. This includes, but is not limited to wearing seatbelts while in Government vehicles. The Contractor will be required to drive

Forest Service Government Fleet vehicle in performing the duties as describe in the contract. The Contractor shall take proper safety and health precautions to protect the workers, the public, and property. The

Contractor may be held financially responsible for any damage to persons or property that occur as a result of his/her fault or negligence.

f) The Contractor is responsible for recording all time worked, and the submission of completed time reports and Invoices for Payment.

C.3 TECHNICAL SPECIFICATIONS

ITEM 01:

Occupational Description. The Contractor will perform a combination of the following tasks which are typical, but not necessarily all inclusive,

2.1 Conduct daily (7 days a week) sampling and testing (turbidity, chlorine, ph, etc.) and maintain daily records of sampling results and water meter readings, as required by Federal and PR Dept. of Health laws and regulations.

2.2 Prepare and submit certified reports to the Puerto Rico Health Department, as required by state and federal laws and regulations.

2.3 Maintain and operate a slow-sand filter treatment plan following the Operation and Maintenance manual.

2.4 Maintain and operate the well-pump system to ensure uninterrupted water service to recreation areas.

2.5 Ensure adequate chlorination levels in drinking water. Add chlorine tablets or other chlorination products as necessary.

2.6 Perform routine maintenance on distribution system, cleaning and repairs to pipelines, valves, strainers, fixtures, pumps, meters, motors, and chlorination systems.

2.7 Perform routine inspection of water system equipment, valves and distribution lines Coordinate and perform minor repairs. Inform the Forest Service of major maintenance, repairs, or replacement of equipment that might be required.

2.8 Perform scheduled inspection and maintenance on the surface water intake of the El Portal water plant. The intake is located inside of the forest and is accessed by a service trail which runs along the raw water line from the intake.

2.9 Perform scheduled water line flushing on raw water and potable water lines in El Portal and El Yunque water systems.

2.10 Inspect bathrooms at recreation areas to ensure proper operation of faucets and toilets. Replace faucets, gaskets, toilet diaphragms and other fixtures as needed (Forest Service will supply fixtures).

2.11 Operate electrical generator to supply power to well pumps in case of electrical power outages. Operate generator and water tank to maintain water supply to facilities in case water supply interruptions by damages in the supply line, maintenance or repairs work.

2.12 Coordinate with the Forest Service contracted water treatment laboratory for the additional sampling and testing required to meet State Health Department requirements.

ITEM 2:

3.1 Additional requirements.

A. Sampling, testing and water system operation work is required daily, seven days a week.

B. The sampling, testing, certification and submittal of tests to the Puerto Rico Health

Department shall be done by a water systems operator licensed in Puerto Rico.

C. The contractor is responsible for all equipment, tools, materials and personal protective equipment needed to complete all the work. The Forest Service will supply fixtures, pipes, valves, faucets, gaskets, and other parts needed for repair and maintenance of the water system. The Forest Service will supply chlorine tablets or any other water disinfection products needed.

D. The contractor is responsible for the safety of his personnel. The contractor must comply with Federal and State safety regulations. The COR will stop work immediately in case personnel safety is compromised.

E. Contractor is responsible for all bonds, liability insurance and worker’s compensation related to this contract.

3.2 Supervision. The Contractor shall follow instructions and directions issued by the Contracting Officer’s

Representative (COR). The COR will give detailed written or oral instructions when needed, but will not provide constant supervision. The COR will inspect the Contractor’s work while in progress and after completion.

3.3 Rejection of Contractor or Contractor’s Employees. The Government reserves the right to reject, at any time and without previous notice, any contractor who, in the opinion of the Contracting Officer, does not meet the Skill and Knowledge or Contractor Responsibility requirements described herein.

C.4 PERIOD OF PERFORMANCE

Estimated Start Date: February 19, 2016

Contract Duration: 12 Months

Completion Date: February 18, 2017

*** This solicitation shall result in a base plus three year options fixed price contract.

SECTION F- PERFORMANCE OF SERVICES

F.1 Quality Control

The Contractor shall provide adequate internal controls and review procedures to eliminate conflicts, errors, and omissions, and to insure the technical accuracy of all services and products. All submittals shall be reviewed for contract compliance prior to delivery.

(B) Site Visits, Inspections, and Investigations:

The Contractor shall notify the Government of all project site visits, inspections, meetings, or conferences.

The Contractor shall visit and inspect/investigate the identified project site(s) as necessary and as required during the preparation and accomplishment of the work.

Periodic meetings shall be held, whenever requested by the Government or the Contractor, to resolve questions and problems relating to the work required under each delivery order.

The Contractor shall be required to attend and participate in all conferences. These conferences may be located at the project site, Contractor’s Office, a Forest Service Office, or other locations.

(D) Coordination and Execution of the Work:

The Contractor shall furnish sufficient technical, supervisory, and administrative personnel to insure execution of the work in accordance with the schedule in the contract.

During the execution of the work, the Contractor shall keep in close liaison with the Government Project leader.

Work beyond the original scope of work shall be accomplished only at the direction of the Contracting Officer.

At all times during performance of this contract and until the work is completed and accepted, the Contractor shall directly superintend the work or assign and have on the worksite a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor.

SECTION G – CONTRACT ADMINISTRATION DATA

AGAR 452.215-73 Post Award Conference (NOV 1996)

A post award conference with the successful offeror is required. It will be scheduled within 30 days after the date of contract award. The conference will be held at a place to be determined.

G.2 GOVERNMENT-FURNISHED PROPERTY

The Government will provide the following item(s) Maps-- of Government property to the Contractor for use in the performance of this contract. This property shall be used and maintained by the Contractor in accordance with the provisions of the "Government Property" FAR clause contained elsewhere in the contract.

G.3 CONTRACTOR INVOICING

The "Submit Invoice-to" address for USDA orders is the Department of Treasury's Invoice Processing Platform

(IPP). The contractor must follow the instructions on how to register and submit invoices via IPP as prescribed in the previous communications from USDA and Treasury. All invoices are to be submitted via the electronic

SECTION H - SPECIAL CONTRACT REQUIREMENTS

H.1 . FSAR 4G52.222-701 Employment of Eligible Workers (DEC 1999)

Section 274A of the Immigration and Nationality Act (8 USC 1324a) makes it unlawful for an employer to hire unauthorized aliens. The Immigration and Naturalization Service (INS) has established the Form I-9, Employment Eligibility Verification Form, as the document to be used for employment eligibility verification

(see 8 CFR 274a).

The contractor is required to (1) have all employees complete and sign the I-9 Form to certify that they are eligible for employment; (2) examine documents presented by the employee and ensure the documents appear to be genuine and related to the individual; (3) record information about the documents on the form, and complete the certification portion of the form; (4) retain the form for three years, or one year past the end of employment of the individual, whichever is longer.

It is illegal to discriminate against any individual (other than a citizen of another country who is not authorized to work in the United States) in hiring, discharging, or recruiting because of that individual's national origin or citizenship status.

Compliance with Section 274A of the Immigration and Nationality Act (8 U.S.C. 1324a) is a material condition of the contract. If the contractor employs unauthorized workers during contract performance in violation of section 274A, the Government may terminate the contract, in addition to other remedies or penalties prescribed by law.

For further information on the requirements of the Act, contractors should contact the Employer and Labor

Relations Officer of their local INS office.

H.2. 4G52.222-704 PERSONAL PROTECTIVE EQUIPMENT (FEBRUARY 2007)

The contractor shall train workers in the safe operation and use of equipment that the worker may use before the worker begins using such equipment.

Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards or processes encountered which may cause injury or impairment in the function of any part of the body.

Defective or damaged personal protective equipment shall not be used.

The contractor shall provide chain saw chaps to each employee who operates a chain saw, at no cost to the employee. These chaps must be approved by an Underwriters Laboratory or meet Forest Service specification

6170-47. The chaps shall cover the full length of the thigh and shall extend to the top of the boot on each leg.

The contractor shall assure that each employee wears foot protection that provides adequate traction and ankle support. Employees operating chain saws shall wear foot protection that is constructed with cut-resistant material which will protect the employee against contact with a running chain saw.

In any area where the worker is exposed to the potential for flying or falling objects, the contractor shall provide a hard hat, at no cost to the employee, and the contractor shall assure that the employee wears the hard hat. The

General maintenance services hard hat must meet the minimum requirements of American National Standards Institute (ANSI) standard

Z89.1-1997.

The contractor shall provide, at no cost to the employee, eye protection where there is potential for eye injury due to flying objects. This eye protection must meet the minimum requirements of ANSI standard Z87.1-2003.

The contractor shall provide, at no cost to the employee, hearing protection where there is a potential for hearing loss due to high intensity noise for example, chain saw operation.

I.2 52.204-7 System for Award Management. (JUL 2013)

As prescribed in 4.1105(a)(1), use the following provision:

SYSTEM FOR AWARD MANAGEMENT (JUL 2013)

(a) Definitions. As used in this provision—

“Data Universal Numbering System (DUNS) number” means the 9-digit number assigned by Dun and

Bradstreet, Inc. (D&B) to identify unique business entities.“Data Universal Numbering System +4 (DUNS+4) number”means the DUNS number assigned by D&B plus a 4-character suffix that may be assigned by a business concern.(D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional System for Award Management records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at subpart 32.11) for the same concern. “Registered in the System for Award Management (SAM) database” means that—(1) The offeror has entered all mandatory information, including the DUNS number or the DUNS+4 number, the Contractor and

Government Entity (CAGE) code, as well as

General maintenance services data required by the Federal Funding Accountability and Transparency Act of 2006 (see subpart 4.14) into the

SAM database; (2) The offeror has completed the Core, Assertions, and Representations and Certifications, and

Points of Contact sections of the registration in the SAM database; (3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification

Number (TIN) with the Internal Revenue Service (IRS). The offeror will be required to provide consent for TIN validation to the Government as a part of the SAM registration process; and (4) The government has marked the record “Active”. (b)(1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation.(2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation “DUNS” or “DUNS +4” followed by the DUNS or DUNS +4 number that identifies the offeror’s name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database.(c) If the offeror does not have a DUNS number, it should contact Dun and

(i) Competitively within a time frame providing for compliance with the contract performance schedule;
