Attachment #2

ACTIVITY I SCENARIO/S: Emergency Hazardous Materials Release Response For each of the illegal dumping/spill scenarios provided below, prepare and submit a work plan that provides a description of how contractor would address the incident. The work plan needs to include and address the following:

i. Description of the work to be performed.

ii. Discussion of any specific exclusions/assumptions the contractor may have regarding their response to the scenario in question.

iii. Anticipated contractor response time.

iv. Conceptual listing of personnel, including subcontractor(s) that would be utilized for the response.

v. Conceptual listing of equipment and supplies to be utilized for the response.

vi. Safety & Health Plan and procedures the contractor will follow.

vii. Emergency Response Plan

viii. Container sampling and waste identification protocols that would be utilized.

ix. Hazardous material/waste management plan and protocols for material/waste handling, transport and disposal.

x. Provide the estimated cost for the response. Breaking down cost items into key categories (work plan development, mobilization, performance of work, disposal cost, and final report preparation).

1) Scenario 1

On November 10th, the Contractor’s emergency response line has received a call from the Forest Service about an illegal hazardous material disposal site which has been found on a national forest in California. The contractor is being provided with the following information:

· An illegal hazardous material dump site was discovered on the Lassen National Forest. The material was found approximately 2 miles east of the Bogard Work Center on Forest Service Road 32N07 near the spur with Forest Service Road 32N98Y. The Bogard Work Center is located in Lassen County on Highway 44, approximately 29 miles northwest of Susanville. Forest Service Road 32N07 is accessible by 2wd vehicles. See map below for the location of the Bogard Work Center.

· Initial field inspection by Forest Service personnel identified the following materials:

· Ten (10) partially full rusting 1 gallon metal cans. Labels on the containers have deteriorated, but several indicate they contain oil based wood stain.

· Three (3) partially filled 30 gallon metal containers. Containers are marked "stock #1556 and UN 1263, automotive refinishing product high efficiency thinner for refinishing lacquer”.

· Six (6) full 1 gallon plastic antifreeze containers. Containers appear to have used oil.

· One (1) open and partially full plastic antifreeze container with what appears to be used oil. Container is on its side and material has spilled out on the soil.

· Three (3) partially filled 5 gallon plastic containers. Tops are off the containers and they have what appears to be a black tar like material.

· Five (5) partially filled 5 gallon plastic containers. Containers have no labels and appear to have liquids. Two of the containers are cracked and have leaked on the ground staining an area approximately 2’ x 5’ in size with what appears to be oil.

· Three (3) automotive lead acid batteries.

· Rain is forecast for the next several days.

2) Scenario 2

The Contractor’s emergency response line has received a call from the Forest Service about a vehicle accident and hazardous material spill which has occurred on a national forest in California. The contractor is being provided with the following information:

· An individual towing a trailer carrying poly tanks with diesel fuel overturned on the Inyo National Forest. The accident occurred on North Oak Creek Road approximately 4.5 miles northwest of the town of Independence, California.

· The driver was transporting diesel fuel in 1100 gallon water poly tank which was secured to the trailer with cargo straps. The vehicle and trailer over turned and the tank broke free and rolled approximately 40 feet down a slight embankment and ruptured.

· Based on observations from Forest Service personnel, the majority of the 1100 gallons of diesel fuel were released and has flowed approximately 180 feet down a slight grade to within 40 feet of North Oak Creek. The width of the spill path appears to be approximately 12 inches wide and there was several areas where the fuel had pooled as it flowed downslope. The depth is unknown. The soil is silty and depth to ground water is unknown.
