SECTION C – SPECIAL CONTRACT REQUIREMENTS

GENERAL

Scope of Work:

The work shall consist of what is shown on the plans, stated in the specifications, and the major items are summarized below:

• Coordinate with the phone company to locate their facility as required.

• Establish temporary traffic control signing as required for construction of the detour.

• Install temporary erosion control measures.

• Remove trees and clear and grub as required.

• Construct roadway detour and switch traffic.

• Adjust temporary traffic control signing as necessary.

• Remove the existing bridge.

• Excavate for footings.

• Construct the footings.

• Construct the abutments and wingwalls.

• Backfill the abutments to just below the bearing seat elevation.

• Install riprap adjacent to the new structure.

• Grade portions of the channel banks under the new bridge.

• Install the precast deck panels.

• Complete abutment backwall.

• Construct the remaining backfill and the approach roadway embankments.

• Install the bridge rails.

• Install the approach rails.

• Perform all work required to construct asphalt surface treatment on bridge approaches.

• Install permanent traffic control signing.

• Adjust temporary traffic control signing as necessary.

• Open the road and new bridge to traffic.

• Obliterate the detour road and haul the material from the site.

• Complete the riprap installation along the stream.

• Prepare the finished grading and apply seeding.

• Remove temporary traffic control signing.

Project Location:

The bridge carries FSR 150 over East Indian Creek. The bridge is located 81 miles northeast of Silver City, New Mexico in the Gila National Forest.

Work Procedure:

All work related to this project shall be done in accordance with all applicable OSHA and State of New Mexico regulations.

Final construction staging areas shall be designated by the CO. Security of the staging area shall be the responsibility of the Contractor.

If the contractor will be camping near the site, a permit must first be obtained. The permit request shall be made through the CO.

The road to the site becomes very muddy and difficult to travel over when it is wet.

101.04 Definitions.

Bid Schedule--The Schedule of Items.

Bridge--No definition.

Contractor--The individual or legal entity contracting with the Government for performance of prescribed work. In a timber sale contract, the contractor is the “purchaser”.

Culvert--No definition.

Right-of-Way--A general term denoting (1) the privilege to pass over land in some particular line (including easement, lease, permit, or license to occupy, use, or traverse public or private lands), or (2) Real property necessary for the project, including roadway, buffer areas, access, and drainage areas.

Add the following:

Adjustment in Contract Price--“Equitable adjustment,” as used in the Federal Acquisition Regulations, or “construction cost adjustment,” as used in the Timber Sale Contract, as applicable.

Pioneer Road--Temporary construction access built along the route of the project.

Purchaser--The individual, partnership, joint venture, or corporation contracting with the Government under the terms of a Timber Sale Contract and acting independently or through agents, employees, or subcontractors.

Protected Streamcourse--A drainage shown on the plans or timber sale area map that requires designated mitigation measures.

Road Order--An order affecting and controlling traffic on roads under Forest Service jurisdiction. Road Orders are issued by a designated Forest Officer under the authorities of 36 CFR, part 260.

Schedule of Items--A schedule in the contract that contains a listing and description of construction items, quantities, units of measure, methods of measurement, unit price, and amount.

Utilization Standards--The minimum size and percent soundness of trees described in the specifications to determine merchantable timber.

Add the following:

(c) As-Built-Plans. Furnish one set of as built plans. The Government will provide one set of contract plans to be used exclusively for recording the as-built details of the project. Use red pencil or red ink to record the information on the as-built plans.

Note all additions or revisions to the location, character, and dimensions of the prescribed work shown on the contract plans. Line out all details shown that are not applicable to the completed work. Check off details shown that were incorporated into the completed work without change.

Retain the plans at the project site and, as work progresses, continually update them to reflect the as-built details. Upon request, make the plans available to the CO to review for compliance with these specifications.

Show the following types of changes on the as-built plans:

Furnish the as-built working plans to the CO before the final inspection. Correct all details found during the final inspection that are not shown on the as-built plans and return to the CO within 5 days.

104.04 Coordination of Contract Documents.

Replace with the following:

The special contract requirements, standard specifications, and plans are contract documents. A requirement in one document is binding as though occurring in all contract documents. The contract documents are intended to be complementary and to describe and provide for a complete contract. In case of discrepancy, calculated and shown dimensions govern over scaled dimensions. The contract documents govern in the following order:

(a) Special contract requirements;

(b) Standard specifications; and,

(c) Plans.

104.06 Use of Roads by Contractor.

The Contractor is authorized to use roads under the jurisdiction of the Forest Service for all activities necessary to complete this contract, subject to the limitations and authorizations designated in the Road Order(s) or described in the contract, when such use will not damage the roads or national forest resources, and when traffic can be accommodated safely.

104.07 Other Contracts.

The contractor shall schedule activities to coordinate and cooperate with other contracts, when applicable.

105.02 Material Sources.

105.02(a) Government Provided Sources.

Add the following:

Comply with the requirements of 30 CFR 56, subparts B and H. Use all suitable material for aggregate regardless of size unless otherwise designated. When required, re-establish vegetation in disturbed areas according to section 625.

105.02(b) Contractor-located sources.

Add the following:

All certifiable material (e.g., soil, gravel, seed, mulch etc.) transported onto National Forest System land or incorporated into the work will be weed-free. The Contracting Officer may request written documentation of methods used to determine the weed-free status of any and all materials furnished by the contractor. Contractor-provided expertise and methods to establish weed-free status must be appropriate for the weeds of concern in the local area.

All seed and mulch materials must be certified weed free.

105.05 Use of Material Found in the Work.

Delete 105.05 (a) and (b) and the last sentence of the second paragraph and substitute the following:

Materials produced or processed from Government lands in excess of the quantities required for performance of this contract are the property of the Government. The Government is not obligated to make reimbursement for the cost of producing these materials.

106 - Acceptance of Work

106.01 Conformity with Contract Requirements.

Delete Subsection 106.01 and substitute the following:

Follow the requirements of FAR Clause 52.246-12 Inspection of Construction.

References to standard test methods of AASHTO, ASTM, GSA, and other recognized standard authorities refer to the methods in effect on the date of solicitation for bids.

Perform all work to the lines, grades, cross-sections, dimensions, and processes or material requirements shown on the plans or specified in the contract.

Incorporate manufactured materials into the work according to the manufacturer’s recommendations or to these specifications, whichever is more restrictive.

Plan dimensions and contract specification values are the values to be strived for and complied with as the design values from which any deviations are allowed. Perform work and provide material that is uniform in character and reasonably close to the prescribed value or within the specified tolerance range.

The purpose of a tolerance range is to accommodate occasional minor variations from the median zone that are unavoidable for practical reasons.

When standard manufactured items are specified (such as fence, wire, plates, rolled shapes, pipe conduits, etc., that are identified by gauge, unit mass, section dimensions, etc.), the identification will be considered to be nominal masses or dimensions. Unless specific contract tolerances are noted, established manufacturing tolerances will be accepted.

The Government may inspect, sample, or test all work at any time before final acceptance of the project.

When the Government tests work, copies of test reports are furnished to the Contractor upon request.

Government tests may or may not be performed at the work site. If Contractor testing and inspection is verified by the Government, the Contractor’s results may be used by the Government to evaluate work for acceptance. Do not rely on the availability of Government test results for process control.

Acceptable work conforming to the contract will be paid for at the contract unit bid price. Three methods of determining conformity and accepting work are described in Subsections 106.02 to 106.04 inclusive. The primary method of acceptance is specified in each Section of work. However, work may be rejected at any time it is found by any of the methods not to comply with the contract.

Remove and replace work that does not conform to the contract, or to prevailing industry standards where no specific contract requirements are noted, at no cost to the Government.

(a) Disputing Government test results. If the accuracy of Government test results is disputed, promptly inform the CO. If the dispute is unresolved after reasonable steps are taken to resolve the dispute, further evaluation may be obtained by written request. Include a narrative describing the dispute and a proposed resolution protocol that addresses the following:

(1) Sampling method;

(2) Number of samples;

(3) Sample transport;

(4) Test procedures;

(5) Testing laboratories;

(6) Reporting;

(7) Estimated time and costs; and

(8) Validation process.

If the evaluation requires additional sampling or testing be performed, mutually agree with the Government on witnessing procedures and on sampling and testing by a third party laboratory.

Use a third party laboratory accredited by the AASHTO accreditation program. Provide proof of the laboratory’s accreditation for the test procedures to be used. Do not use the same laboratory that produced the disputed Government test results or that produced the test results used as a basis for the dispute.

The CO will review the proposed resolution protocol and may modify it before final approval and execution.

The Government will use the approved resolution protocol test results to determine the validity of the disputed testing. If the Government test results are validated, the Contractor will be responsible for all costs associated with developing and performing the resolution protocol. If the Government test results are not validated, the Government will be responsible for all costs associated with developing and performing the resolution protocol. If the validity of the Government test results cannot be determined, the Contractor and Government will equally share all costs associated with developing and carrying out the resolution protocol.

(b) Alternatives to removing and replacing non-conforming work. As an alternative to removal and replacement, the Contractor may submit a written request to:

(1) Have the work accepted at a reduced price; or

(2) Be given permission to perform corrective measures to bring the work into conformity.

The request must contain supporting rationale and documentation. Include references or data justifying the proposal based on an evaluation of test results, effect on service life, value of material or work, quality, aesthetics, and other tangible engineering basis. The CO will determine disposition of the nonconforming work.

106.05 Statistical Evaluation of Work and Determination of Pay Factor (Value of Work).

Replace the entire section with the following:

Statistical evaluation of the work will not be required. All specifications referring to this Subsection shall instead be referred to Subsections 106.02 through 106.04 as appropriate for the work performed.

107 - Legal Relations and Responsibility to the Public

107.02 Protection and Restoration of Property and Landscape.

Add the following:

Work within the project limits shall be limited to 100 feet up and downstream of the existing bridge.

107.05 Responsibility for Damage Claims.

Delete subsection 107.05.

107.06 Contractor’s Responsibility for Work.

Delete the following:

“except as provided in Subsection 106.07”.

107.08 Sanitation, Health, and Safety.

Add the following:

Perform all operations in a prudent, conscientious, safe and professional manner. Ensure that all personnel involved in handling and packaging the hazardous waste are trained for the level of expertise required for the proper performance of the task and, in particular, in the areas of chemical incompatibility, general first aid procedures, and spills. Provide handling and personal protective equipment appropriate to ensure safe handling of the hazardous waste according to 29 CFR 1910.120).

Notify the Forest Service of all hazardous material that may be brought onto the National Forest.

107.09 Legal Relationship of the Parties.

107.10 Environmental Protection.

Add the following:

Design and locate equipment repair shops, stationary refueling sites, or other facilities to minimize the potential and impacts of hazardous material spills on Government land.

Before beginning any work, submit a Hazardous Spill Plan. List actions to be taken in the event of a spill. Incorporate preventive measures to be taken, such as the location of mobile refueling facilities, storage and handling of hazardous materials, and similar information. Immediately notify the CO of all hazardous material spills. In addition, report all spills immediately to the NMED as required by the New Mexico Water Quality Control Commission regulations (20.6.2.1203 NMAC). For non-emergencies during normal business hours, call 505-428-2500. For non-emergencies after hours, call 866-428-6535 or 505-428-6535 (voice mail, twenty-four hours a day). For emergencies only, call 505- 827-9329 twenty-four hours a day (NM Dept of Public Safety).

Provide a written narrative report form no later than 24 hours after the initial report and include the following:

• Description of the item spilled (including identity, quantity, manifest number, and other identifying information).

• Whether amount spilled is EPA or state reportable, and if so whether it was reported, and to whom.

• Exact time and location of spill including a description of the area involved.

• Containment procedures.

• Summary of any communications contractor had with news media, Federal, state and local regulatory agencies and officials, or Forest Service officials.

• Description of clean-up procedures employed or to be employed at the site including final disposition and disposal location of spill residue.

When available provide copies of all spill related clean up and closure documentation and correspondence from regulatory agencies.

The Contractor is solely responsible for all spills or leaks that occur during the performance of this contract. Clean up spills or leaks to the satisfaction of the CO and in a manner that complies with Federal, state, and local laws and regulations.

107.11 Protection of Forests, Parks, and Public Lands:

Fire Precautions. Specific fire precautionary measures listed below shall be applicable during Contractor’s Operations in “Total Fire Season” described as between April 1st and October 31st.

Contracting Officer may change the dates of Total Fire Season by advance written notice, if justified by unusual weather or other conditions. Required tools and equipment shall be kept in serviceable condition and immediately available for fire fighting at all times during Contractors Operations in Total Fire Season.

Substitute Precautions. Forest Service may authorize substitute measures or equipment, or waive specific requirements by written notice, if substitute measures or equipment will afford equal protection or some of the required measures and equipment are unnecessary.

Emergency Precautions. Forest Service may require the necessary shutting down of equipment on portions of Contractor’s Operations, as specified by the emergency fire precautions as listed herein.

GILA NATIONAL FOREST FIRE PLAN

The Contractor shall be responsible for the cost of suppressing all fires caused by his operation, and for all damage to any property of the USDA Forest Service or third parties. The USDA Forest Service reserves the right to participate in extinguishing any fires caused by the Contractor's operations and the Contractor shall reimburse the USDA Forest Service for the cost of all suppression, activities ordered or directed by the USDA Forest Service. The following requirements are a part of this solicitation and any resultant contract.

1. Fire Prevention - During periods of contract performance, fire requirements shall be met by the

Contractor.

2. Execution of Fire Plan

a) Forest Service personnel involved will, in all cases, coordinate through the Contracting

Officer's Representative (COR) except in cases of emergency or Forest closure.

b) The Contractor shall designate a fire plan representative who shall be responsible for executing and carrying out the fire requirements.

c) The Contractor shall include the fire requirements in all subcontracts.

d) The Contractor shall assure that all contract employees are informed of the existence and conditions of the fire requirements.

3. Compliance Inspection

a) Inspection by the Forest Service will be made to insure compliance with the fire requirements.

b) Should any deficiencies appear during the inspection, the Contractor will be informed and the deficiencies shall be corrected within 24 hours. Activities with the potential to start fires shall be halted until the deficiencies are corrected and the corrections approved by the COR.

c) The Contractor's fire plan representative shall make daily inspections to insure that the terms of the fire requirements are met at all times.

4. Fire Pre-Suppression Requirements

a) Fire Tool Box

1) During contract performance, the Contractor shall furnish and maintain, at the Contractor's expense, one fire toolbox at the campsite or some central location, as long as it is within 1/4 mile of the location where the Contractor is currently performing work.

2) The fire toolbox is to be maintained with tools in good condition in the following quantities for each five people in the Contractor's work force. The equipment is for the sole purpose of firefighting. Four shovels, two axes, one Pulaski and five each 1-gallon canteens. Where needed, brush hooks and McLeod tools may be substituted for shovels or axes.

3) The fire toolbox shall be replenished by the Contractor after each incidence of fire use.

b) Work Hours

1) The Contractor shall restrict operations in accordance with the following emergency fire precaution schedule unless the COR waives the requirement in writing.

2) Each day of operation when there is a predicted change in the fire precaution plan, the Forest Service will inform the Contractor between 3 and 6 p.m. MST (4 and 7 p.m. MDT).

Notification will be made of the alphabetical industrial fire precaution plan (Exhibit 1) to be followed the next day within the local operating area. No later than 9 a.m. MST (10 a.m.

MDT) the following day, the Forest Service will inform the Contractor of any change in the fire precaution schedule.

c) Blasting

1) Use of caps and safety fuse detonators for blasting is not permitted when fire precaution plan C or D is in effect.

2) A fire guard shall be required in accordance with the enclosed emergency fire precaution schedule.

3) This individual, when required, shall remain on duty for at least 1 hour after blasting is finished.

4) Minimum equipment shall consist of a round-pointed, size 0 or larger shovel and a 3 to 5 gallon backpack pump filled with water.

5) Blasting hours are restricted under fire precaution plan C, and blasting is prohibited under fire precaution plans D.

d) Welding

1) An area commensurate with the amount of welding to be accomplished shall be cleared before welding operations are started.

2) Written permission must be obtained from the COR during fire precaution plan B, C and D.

3) As a minimum, a 3 to 5 gallon backpack pump filled with water is to be available in addition to the vehicle fire extinguisher.

4) The area in the immediate vicinity of the operation shall be wet down before and after operations.

5) The area adjacent to the welding operations shall be thoroughly checked for fires for 1 hour after welding.

e) Fire Guard

1) The Contractor shall designate at least one representative to train and supervise each work crew in fire prevention, detection, and suppression.

2) To prevent, detect, and suppress fires, the Contractor shall provide a trained fire guard for projects involving internal combustion engines.

3) The fireguard shall perform guard duties during operating hours.

4) The fireguard may perform contract work in conjunction with fireguard duties.

5) Each fire guard shall report all fires and take suppression action on any fire that starts on the project area.

6) Each fire guard shall be equipped with a vehicle and a fire tool cache consisting of a fire cache box, two size 0 shovels, two Pulaski’s, two McLeod tools, and two 4 to 5 gallon back pack pumps filled with water.

7) All equipment and tools shall be maintained in serviceable condition.

5. Fire Suppression

a) The Contractor shall take independent action on the suppression of all fires in the work area or vicinity. Vicinity is defined as a 1/4 mile extension from the operations area boundary.

b) All Forest fires shall be reported to the District Ranger's Office as soon as possible. Even though the fire has been suppressed by the Contractor's crew, this report is required. The office and telephone number to which fires shall be reported will be furnished to the Contractor prior to start of contract work.

6. Equipment Requirements

a) All gasoline and diesel equipment (except turbocharged) used on the project shall be equipped with spark arresters approved in the Spark Arrester Guide, USDA Forest Service, June 1983.

b) All internal combustion engines must be inspected before use on the job and periodically thereafter by the Forest Service. Equipment not passing inspection cannot be used until repairs are made.

c) Serviceable baffled mufflers with standard exhaust tailpipes are considered adequate on trucks, pickups, and sedans.

d) Equipment service areas and gas and oil storage areas shall be cleared of brush, litter, debris, and grass for a radius of at least 50 feet.

e) Each piece of motorized equipment not included in item F below shall be equipped with a round-pointed shovel and fire extinguisher.

f) Fuel trucks, service trucks, and the work camp shall be equipped with a shovel, DB axe, and class ABC pressurized chemical fire extinguisher of not less than 15 pounds capacity by weight. The shovels and fire extinguishers are in addition to those required for the Toolbox (4.a.). These shall be furnished and maintained at the Contractor's expense.

g) All crawler tractors and rubber-tired equipment suitable for fire suppression work shall be prepared for night time use by mounting of brackets for potable, self-contained, battery-operated lights, provided by the Forest Service.

h) Power saws shall be kept in a safe, serviceable condition at all times. Each power saw shall be equipped with an approved spark arrester/muffler system meeting the SAE J-335b standard.

1) The refueling of power saws shall be done on mineral soil and the Saw shall be moved at least 10 feet from the point of refueling to another barren spot before being started.

2) Power saws with warm or hot spark arresters/mufflers (running or not) shall not be placed or rested on sawdust piles, litter duff, stumps or spots that are easily ignited.

3) Each power saw shall be accompanied by one round pointed shovel in good condition and a metal fuel-safety can. One chemical pressurized fire extinguisher, 8 ounces or larger, is to be kept with the power saw operator at all times while working on the project. The shovel is to be kept with the fuel supply.

7. Smoking and Fires

a) Smoking

1) No smoking shall be allowed while traveling, except on surfaced highways. A surfaced highway is any road that is oiled, graveled or otherwise paved.

2) Smoking is not permitted while working. Smoking is only permitted on breaks in areas having a clearing of at least 3 feet in diameter to bare soil.

b) Fires - No warming, burning of debris, or cooking fires shall be permitted during periods of high or above fire danger except in designated places or by written permission.

8. Violations

a) Violations of the smoking regulations (36 CFR 261.52(a)(c)(d) and fire requirements will result in criminal action against the violator. The Contractor shall advise all employees of this regulation.

b) Violation or deficiencies of fire requirements shall result in immediate suspension of operations until the deficiency or violation is corrected. Repetitive violations will be grounds for default termination.

9. Historical Data - The following data on the Forest area involved is provided for the Contractor's information. The data is based on records of the past 10 years:

a) Total Fire Season - April 1 to October 31.

b) Historic Average Number of Days – The following table gives the historic average number of days each Fire Precaution Plan have been in affect:

152 - Contractor Survey and Staking

Description

152.01(c) Material.

Use required stake dimensions and materials. Pre-paint the top 2 inches of all stakes and lath, or mark them with plastic flagging. Use designated colors for paint or flagging. Mark all stakes with a stake pencil that leaves a legible imprint, or with waterproof ink.

Do not use aerosol spray paints.

Use moisture-resistant paper for survey notes. Keep notes in books with covers that will protect the contents and retain the pages in numerical sequence.

Construction Requirements

152.02 General.

When indicated on the plans, a preliminary survey line has been established on the ground. The project location line (road centerline) is established by offsets from this preliminary line.

Delete second sentence in second paragraph and replace with the following:

Reestablish missing reference, control lines, or stakes as necessary to control subsequent construction staking operations

152.03 Survey and Staking Requirements.

(b) Roadway cross-sections.

Replace the first three sentences with the following:

Take roadway cross-sections normal to centerline. When the centerline curve radius is less than or equal to 200 feet, take cross-sections at a maximum centerline spacing of 25 feet. When the centerline curve radius is greater that 200 feet take cross-sections at a maximum centerline spacing of 100 feet.

c) Slope Stakes & References:

Replace section with the following:

Slope stakes and references. When required, locate slope stakes on designated portions of the road.

Locate the slope stake catch points and use them to establish clearing limits and slope stake references.

Mark slope stakes with the station, the amount of cut or fill, the horizontal distance to centerline, and the slope ratios.

Place slope reference stakes at least 10 feet outside the clearing limit and mark with the offset distance to the slope stake. Place sight stakes when required.

Prior to clearing and grubbing operations, move the slope stake outside the clearing limit to the slope reference stake. After clearing and grubbing and before excavation, reset the slope stakes in their original position.

Use the designated method to establish the slope stake catchpoint.

• Method I—Computed Method. Use the template information shown in the plans or other Government-provided data to calculate the actual location of the catchpoint. The slope stake “catchpoint distance” provided may be used as a trial location to initiate slope staking. Recatch slope stakes on any section that does not match the staking report within the tolerances established in Table 152-2.

• Method II—Catchpoint Measurement Method. Determine the location of slope stake catchpoints by measuring the catchpoint distances shown in the plans or other Government-provided data.

(d) Clearing and grubbing limits.

Add the following:

Establish clearing limits on each side of the location line (road centerline) by measuring the required horizontal or slope distances shown in the stake notes. Mark the clearing limits with flagging or tags on trees to be left standing, or on lath. Make markings intervisible, and no more than 90 feet apart.

After establishing clearing limits, move the location line (road centerline) stake outside the clearing limits for station identification purposes, and mark it with horizontal distance to location line (road centerline)

(e) Centerline reestablishment.

Replace with the following:

Reestablish centerline from instrument control points. The maximum spacing between centerline points is 25 feet when the centerline curve radius is less than or equal to 200 feet. When the centerline curve radius is greater than 200 feet, the maximum distance between centerline points is 100 feet.

(g) Culverts.

Delete this section

(h) Bridges.

Replace the subsection with the following:

Set adequate horizontal and vertical control, reference lines, and reference points for the bridge, abutment footings, abutment bearing seats, wing walls, and other major components. Establish the bridge control, reference lines, and reference points by setting hubs and guard stakes beyond the clearing limits on each side of the channel. Record the following reference stakes:

(1) Station or coordinates of actual point where the abutment centerline bearing intersects the road centerline and the skew angle;

(2) Reference points and lines for the location all footings, including elevations;

(3) Reference points and lines for wing walls and abutment walls locations including all elevations shown on the plans;

(4) Reference points and lines for all top of embankment stations and offsets;

(5) Reference points and lines for surface aggregate and pavement areas;

(6) Coordinates and elevations for riprap corners; and,

(7) Reference points, reference lines, and applicable offset distances for all rail post locations.

(8) Reference points and lines for precast slab placement.

Perform this work after clearing is completed and within the tolerances established in Table 152-1 under Precision Class A (Bridges). The structural component’s control is established as a local network and the tolerances are relative to that network.

153 - Contractor Quality Control

153.02 Contractor Quality Control Plan.

Submit proposals in writing for using alternate AASHTO or State approved test methods for approval.

Alternate methods may be allowed based on documented equivalence to the method specified.

153.02 (d) Personnel Qualifications Add the following:

(3) The contractor shall submit to the CO certifications of testing and sampling personnel. Testing and sampling personnel shall be either certified as NICET level 2 in highway materials or an equivalent state or industry certification such as Western Alliance Quality Technician Construction Certification

(WAQTC).

153.03 Testing

Add the following:

The contractor shall submit to the CO current documentation of the calibration and inspection of the testing equipment to be used for the project. The documentation shall include inspection date, inspector, calibration number, and the method of calibration. All testing equipment shall be calibrated and inspected tagged with the inspection date, inspector, and calibration number. The lab testing equipment and density testing equipment shall be calibrated and inspected on a yearly basis. The concrete testing equipment shall be calibrated and inspected every four months.

Material testing shall be performed at the frequency required in the subsection titled Acceptance within each applicable standard specification section.

The Contractor shall provide testing of the embankment (on-site soils and commercial), structural backfill, select borrow, and aggregate base materials to determine optimum moisture content and maximum density according to the method required within each applicable standard specification section. The Contractor shall provide testing of the structural backfill material to determine plasticity index, gradation, liquid limit, and/or soil classification. Report test results to the CO according to Section 154.04.

All retests shall be performed at the Contractor expense.

153.04 Records.

Delete all but the first paragraph.
