QUESTIONS RECEIVED AS OF 05/15/2015:

1. Is the Key personnel the main doctor who will be providing the care to the students and the proposed substitute the other doctors in the practice who may provide the care or are all the doctors in the practice key personnel?

a. As stated in AGAR 452.237-74, the Key Personnel is identified as the Optometrist, which would include the main provider and any substitute personnel.

2. Do you need resumes and a summary of experience of all the doctors providing the care?

a. As indicated on the Quoter’s Questionnaire, quotes must include licensure for all Key Personnel; and resumes can either be included at the time of submission, or be prior to award.

3. Are the 3 professional references required in reference to the Vision Center in general or is each doctor required to list 3 references?

a. Three professional references can be for the Key Personnel individually or for the company in general.

4. Section 5 B states that contractor personnel shall be licensed in the state of Kentucky. Since this bid is for Nebraska shouldn't that be the state of Nebraska?

a. Yes, the Optometrist should be licensed in the state where services are to be performed. A revised Performance Work Statement has been added to the solicitation in www.fbo.gov to reflect this correction.

5. Is the bid considered complete when all it information is provided on the Quoter’s Questionnaire and schedule of services is returned?

a. The quote is considered complete when all the required documentation indicated on the Quoter’s Questionnaire has been submitted before the response date has passed.
