Please list prices for each of the line items on the list of gases (pages 3-5 below), and indicate if any prices are location specific other than for NVSL.

Please indicate tank sizes available and corresponding tank rental pricing. BPA call orders or purchase card orders will be issued on an as-needed basis.

□ A currently active Federal Contractors Registration at http://www.sam.gov.

An offeror shall include a printed copy of the first page of the SAM listing, or a

Certifications—Commercial Items. An offeror shall complete only paragraph

(b) of this provision (in Attachment 1) if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov for NAICS code 311612. If an offeror has not completed the annual representations and certifications electronically at the SAM website, the offeror shall include a copy of the completed provision with the quotation.

□ Past Performance References with previous contracts/POs with this agency or similar scope of work with federal/state/local entities. An offeror shall submit up to three customer references from the last 12 months including contact information and/or previous contract numbers with the USDA.

□ A technical statement explaining the terms of service to meet the requirements.
