Construction Notes
1) CONSTRUCTION SPECIFICATIONS: Standard Specification for Construction of Roads and Bridges on Federal Highway Projects, FP-03 U.S. Customary Units, shall apply to this contract.
2) Minor amounts of clearing and grubbing are required for culvert replacement. Limbs, logs, & stumps may be scattered outside the clearing limits. This work shall be considered incidental to Pay Item 20304-Removal.
3) Maintenance for traffic will be the responsibility of the Contractor. Road closures will be accompanied by road detours in accordance with the Manual of Uniform Traffic Control Devices guidelines. This work shall be considered incidental to Pay Item 15101-Mobilization.
4) Contractor shall not construct these projects during abnormally high flows.
5) The Engineer shall be notified of any road closures one week prior.
6) Contractor is responsible for properly disposing of old culverts off of Forest Service lands.
7) Install culvert in locations as staked by the engineer. Bedding and pipe placement shall be approved by the engineer before backfilling begins.
8) Reconstruction of the subgrade shall be required before placing surfacing material and shall be considered incidental to Pay Item 20407. No excavated materials shall be wasted outside of the clearing limits unless Approved by the Engineer.
9) All select borrow material shall be a silt and sand mixture and meet the standard of FP-03 Subsection 704.07.
10) Riprap shall be embedded into the embankment such that the final surface of riprap will be flush with surrounding embankment.
11) Seed mixture: 55% oats, 25% virginia wild rye (elymus virginicus) or canada wild rye (elymus canadensis), 10% slender wheatgrass (elymus trachycalus), 10% Native Forb Mixture, provided by U.S. Forest Service native seed reserve.
Seeding Rates: Oats = 15 pounds per acre All others = 10 pounds per acre
12) Apply Seed, Fertilizer, and Erosion Control Mat in all disturbed areas of the project.

NOTES:
1) Pay item 60202 includes all hardware, bolts, brackets, bracing, rods, deadman anchors, caps, headwalls, corner panels, and wing panels required by the manufacturer for assembly.
2) All panels shall be fabricated from aluminum structural plate as specified in ASTM B 746.
3) Headwalls are for vertical orientation only.
4) If side slope is flatter that 2:1, a double tie-back assembly is required for each deadman.
5) Standard headwalls are shown. HS-25 wheel loads must be kept a minimum distance of 36" from the wall face. Special headwall packages can be fabricated to meet other loading requirements.
6) All headwall panels must be trenched into the existing ground. If a stable rock foundation is encountered, the panels may be trimmed and placed into a cement - grouted keyway.
7) Headwall caps must be field drilled and bolted to the headwall panels.
8) Wingwall angles and slopes shall verified by contractor.
