ATTACHMENT 1

SECTION C--DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK

DIVISION 100 - GENERAL SPECIFICATIONS

101 – Statement of Work The Contractor shall provide any and all equipment, labor, materials, tools, supervision, equipment including safety and other incidentals necessary to perform all work for the maintenance activities located within the boundaries of the Sumter National Forest in accordance with the specification, exhibits and clauses contained or referenced herein. This work will be performed during the base year of November 1, 2005 to September 30, 2006 with 2 option years

102 – Location and Description of Work

The project is located on the Long Cane Ranger District, Sumter National Forest, South

Carolina. Roads to receive maintenance operations are shown on the attached vicinity map and Road Maintenance Schedule, exhibit ‘B’.

103 – Schedule of Work

The Road Maintenance Schedule, exhibit ‘B’, specifies the road number required maintenance activity, general sequence and frequency of performance and other pertinent information or special instructions necessary for planning and executing the work. Adjustments to these schedules may be made in accordance with CHANGES (FAR 52.243-1) PART II-SECTION I.25. Any such adjustments require prior written documentation from the Contracting Officer.

The Contractor shall inform the Contracting Officer's Representative(COR) of his proposed operating schedule on a weekly basis to facilitate timely inspections.

The contractor shall complete all maintenance required for one road before starting another road. Contractor shall inform the COR of each road completed

104 - Emergency Response Time In the event of emergency maintenance needs, which fall outside the schedule specified for the maintenance period, the Contractor must be able to respond to oral or telecommunicated orders under this contract within 24 hours. Such orders will be issued by the Contracting Officer and/or COR and will be confirmed by a written task order issued by the Contracting Officer or

Contract Specialist. The Contractor will receive compensation for the cost to mobilize equipment to the response location at the rate specified in the Schedule of Items. See Activity Code 250 - Emergency Equipment Movement of this contract for further details.

105 - Superintendence by the Contractor At all times during the performance of the contract and until the work is completed and accepted, the Contractor shall directly supervise the work or assign and have on the work site a competent superintendent who is satisfactory to the Contracting Officer and has authority to act for the Contractor. The superintendent shall be designated in writing to the Contracting Officer prior to the commencement of work on the first task maintenance schedule.

106 - Equipment Requirements and Transportation All equipment proposed to be used for performance of the work shall be of the size, type, number and in such condition to properly pursue the requirements of this contract. Grading equipment must be capable of cutting, crowning and shaping operations necessary to meet the requirements of Activity Codes applicable in the technical (performance standards) requirements and Typical Section Drawing.

The Contractor's proposed equipment shall be made available for inspection prior to award of the contract. If at any time during the course of this contract the equipment is deemed unsatisfactory, the Contracting Officer may order removal and may require a satisfactory replacement, at Contractor's expense. Replacement equipment, if ordered, is to be onsite and working within 48 hours.

Any movement or transportation of equipment to or from work areas required to pursue the work on designated roads, to repair or replace equipment, or for the Contractor's convenience shall be at the Contractor's expense.

107 - Public Safety and Traffic Control The Contractor shall exercise caution and care while pursuing the work to prevent unnecessary conflict with, or potential hazards to, road users or others within the work area.

The Contractor is to pursue all work on site in accordance with all Federal, State and Local laws governing the transportation and work being performed.

The Contractor will post warning signs on each end of the section of road being worked. Signs are to be of the size, quantity, and colors as required in the current edition of the "Manual on Uniform Traffic Control Devises", (MUTCD). A copy of MUTCD is located at the District Rangers Office and may be made available to the Contractor for in office reference on request. These signs shall be moved as needed to properly delineate and identify the section of the road being maintained. At no time shall these signs be more than two miles from operating equipment and shall be in place only when road equipment or persons are performing operations. All slow moving equipment is to have a reflectorized "slow moving" vehicle emblem properly attached.

All work is to be scheduled so that at the end of a working day, the road is passable for the type of traffic normally using the road. If for any reason traffic hazards (segments of unfinished work which may present a hazard to road users, such as rolls of surface material, berms on the shoulder, etc.) are left in or adjacent to the road after normal working hours, the work is to be properly signed as hazardous and visibly illuminated at night.

108 – Documentation

1. Accomplishment Reports

The Contractor shall maintain records of accomplishments using the Daily Accomplishment Report. These records are to be delivered to the COR monthly with an invoice for the services rendered that month and shall become the property of the Government. No payment will be made prior to receipt of these records and acceptance of the work performed.

2. Quality Control Plan

The Contractor shall submit to the CO, a Quality Control Plan (QCP) as part of their technical proposal that will detail the methods the Contractor will use to perform self-inspection to ensure the requirements of this contract are performed in accordance with the contract documents. The QCP shall at a minimum record the reference pay item, location, frequency, type of test and resulting outcome. QCPs shall be submitted with the Accomplishment Report and no payment will be made prior to receipt of these records and acceptance of the work performed.

109- Quality Assurance Surveillance Plan

This Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Performance Work Statement (PWS) for this contract. This plan sets forth procedures and guidelines that will be used in evaluating the technical performance of the Contractor.

PURPOSE OF THE QASP

1. The QASP is intended to accomplish the following:

a. Define the roles and responsibilities of participating government officials and the contractor;

b. Define the types of work to be performed;

c. Describe the evaluation methods that will be employed by the government in assessing the contractor’s performance;

d. Provide copies of Government quality assurance monitoring forms that will be used by the government in documenting and evaluating the contractor’s performance;

e. Describe the process of performance documentation

2. The contractor is responsible for performing inspection services pursuant to the inspection clause and for providing and maintaining the inspection forms approved by the government to perform the contractor’s inspection services.

ROLES AND RESPONSIBILITIES OF GOVERNMENT OFFICIALS

The following government officials will participate in assessing the quality of the contractor’s performance. Their roles and responsibilities are described as follows:

The Contracting Officer’s Representative (COR) and Inspector(s) are responsible for monitoring, assessing, recording, and reporting on the technical performance of the contractor on the schedule of inspections submitted by the contractor and approved by the government. The COR will have primary responsibility for completing “Quality Assurance Monitoring Forms” which will be used to document the inspection and evaluation of the contractor’s work performance.

4. The contracting officer (CO) will have overall responsibility for overseeing the contractor’s performance. The CO will also be responsible for the day-to-day monitoring of the contractor’s performance in the areas of contract compliance, contract administration, cost control, and property control; reviewing the COR’s assessment of the contractor’s performance; and resolving all differences between the COR’s version and the contractor’s version.

TYPES OF WORK TO BE PERFORMED/SCHEDULE OF INSPECTIONS

This contract calls for the contractor to perform the following tasks and subtasks and to perform inspections at the completion of the tasks/subtasks as a minimum and or for intermediate inspections as determined at contract award by the COR/CO based upon accepted Contractor’s quality assurance program submitted as part of technical proposal.

METHODS OF SURVEILLANCE

1. The government, through its COR, will be monitoring the contractor’s performance on a continuous basis. The method the government will use is 100% inspection of the contractor’s inspection results. The government COR will perform periodic independent inspections after the contractor has performed their inspection if questions develop regarding the results of the contractors inspections. For results of the independent inspections that differ from the results noted by the contractor on their inspections, the costs thereof for the reinspection will be assessed against the contractor. The contractor shall be required to rework the effort until the performance standard is achieved.

2. Inspections will be performed to the schedule submitted by the contractor’s Quality Control Plan and approved by the government. Variances from the schedule must be preapproved by the CO.

QUALITY ASSURANCE FORMS AND REPORT

1. The COR will use one quality assurance monitoring form to document and evaluate the contractor’s performance under the contract. The form when completed will document the COR’s understanding of what the contractor was supposed to do, what was actually done, and the impact or consequences of what was done.

2. The COR will judge each event in accordance with the performance standards and performance requirements stated in the Quality/Performance Requirements section in the contract in the performance requirement summary.

3. The COR must substantiate all tasks which they judge to be indicative of “unacceptable” performance. Performance at the “acceptable” level is expected and need not be substantiated

4. The contractor shall furnish to the COR copies of all inspection forms generated witnessing inspections performed. The COR will forward copies of all completed QA monitoring forms to the CO. The contractor is required to respond in writing to any negative QA monitoring form(s) within 5 working days after receipt of the form(s)

ANALYSIS OF SURVEILLANCE RESULTS

1. The CO will review each QA monitoring form prepared by the COR. When appropriate, the CO may investigate the event further to determine all the facts and circumstances surrounding the event considered in the COR’s opinions outlined on the forms. The CO will immediately discuss every event receiving a substandard rating with the COR and contractor to assure that corrective action is promptly initiated.

2. At the end of each month, the COR will prepare a written report for the CO summarizing the overall result of their surveillance of the contractor’s performance during the previous month. This report will become part of the formal QA documentation.

Section 200 - Performance Standards

ACTIVITY CODE: 210 - BLADING AGGREGATE SURFACED ROADS

Maintenance Requirements

210.01 - Blading operations are to be done over the entire riding surface in order to remove, by cutting out, all ruts, potholes, corrugations and berms. All suitable dislodged aggregate shall be smoothly redistributed over the entire riding surface to produce the proper cross slope or crown as specified. Road structural terms are illustrated on Typical Section drawings in Section J (Exhibit A).

210.02 - Aggregate surfacing which has been worked onto road shoulders by traffic maintenance activities are to be reclaimed and smoothly redistributed over the entire riding surface. Riding surface widths are to be perpetuated as existing. Dips and swales designated to control surface drainage are to be maintained. No undercutting of road surface nor material berms will be allowed. Assure proper road crowns and surface drainage after all operations.

210.03 - Usual amounts of grass, leaf litter or pine straw is to be smoothly spread within the riding surface after sufficient movement to sift out surface aggregate. Where grass, leaf litter or pine straw is excessive, as determined by the COR, this material will be piled in spots along the road shoulder and left to decay. Care must be taken to insure this material does not interfere with surface drainage nor present a safety hazard. Any such material is to be reclaimed in subsequent bladings and smoothly spread within the riding surface.

210.04 - Blading operations are to be conducted as required to remove surface material and accumulated debris from bridge decks. Care is to be exercised to not damage any part of the bridge. All material shall be smoothly redistributed over the riding surface at the bridge approaches.

210.05 - Loose rocks protruding three inches or more above the graded surface will be removed. It is permissible to waste these rocks off the riding surface but away from ditch lines.

Soil Moisture Conditions

210.6 - Suitable moisture conditions for surface blading shall be from 8 - 16 percent by weight. Unless authorized by the COR, no surface blading shall be performed while moisture content is outside this range. If moisture conditions are too dry or wet, the Contractor will not be required to meet the requirements of prosecution of work under section F.

Method of Measurement Measurement shall be made to the nearest tenth of a mile using an odometer calibrated over a measured mile.

Basis of Payment Payment shall be made for all work of this activity, inspected and accepted, in accordance with the following pay item and the bid unit price.

Blading Aggregate Surfaced Roads Mile

ACTIVITY CODE: 220 - SURFACE REPAIRS, AGGREGATE ROADS

Maintenance Requirements

220.01 - This activity consists of replacement of aggregate surfaces lost due to surface failure, traffic wear or erosion. The work includes preparation of the existing roadbed, furnishing, loading, hauling and spreading aggregate material and finished blading, as described in specifications. The task order will delineate the location of the necessary work and establish quantities of work required.

220.02 - Surface replacement for road segments shall be performed by cutting out, all damaged areas of the surface, and working aggregate material smoothly over the entire riding surface width. Avoid disturbing base or subgrade material during the placement of aggregate material. All roads to receive aggregate surfacing shall have the new aggregate deposited in the section being worked, mixed when required, and shaped to achieve proper grade, thickness and template as directed by the COR. Aggregate surface material shall not be allowed to remain in ditch lines. Aggregate surface material shall be compacted by operating the hauling and spreading equipment over the full width of each layer of aggregate. The entire area being worked shall be finish bladed in accordance with Activity Code 210: Blading Aggregate Surface Roads.

220.03 - Contractor is to provide and place crushed aggregate for spot surface repairs. The Contractor will deliver the crushed aggregate to the location of spot repair and provide weigh tickets for each delivery in accordance with Method of Measurements.

220.04 - Contractor is to provide and furnish crushed aggregate for spot surface repairs. The Contractor will deliver crushed aggregate to the location of spot repair or repair location.

220.05 - Crushed aggregate, Grading No. Crush and Run 1-1/4” is to be furnished by the Contractor, and shall meet all specifications and the following gradation.

All crushed aggregate provided by the Contractor is to be certified by the quarry source to be in compliance with the following:

1) The above gradation requirements

2) The Percent of Wear shall be greater than 40

3) The specific gravity to have a minimum of 2.5

Method of Measurement Aggregate will be measured by either tons or cubic yards as indicated in the bid schedule. Measurement will be made to the nearest one tenth of a pay unit for all materials provided, placed, inspected and accepted in place at the job site/spot repair location. Where material is measured by weight, the Contractor shall supply weight tickets from state approved scales showing aggregate type, source gross, tare and net weight. All materials supplied by volume shall be measured at the time of delivery by the COR. Material receipts shall be furnished by the Contractor for each truck load delivered and accepted.

Payment shall be made for all work of this activity, inspected and accepted, in accordance with the following items and the unit price.

ACTIVITY CODE: 230 - CLEAN AND RESHAPE DITCHES

Maintenance Requirements

230.01 - All ditches are to be maintained in their original location and at a grade required to ensure proper drainage. Drainage structures and road appurtenances are not to be damaged during this operation and the Contractor will be responsible for any damage that occurs from this operation. Handwork required to properly connect ditch flow lines to drainage structure inlets is considered a part of this activity.

Road Ditches

230.02 - All suitable material, having the same character as the road surface, excavated through this activity is to be reclaimed and used within the riding surface. Unsuitable material is to be spread along back slopes or fill slopes to assure road surface drainage and prevent unsuitable material from being pulled onto riding surfaces.

Unless otherwise directed by the COR, ditches shall be cleaned so that depth of ditch is 1 foot below the shoulder of the road and the slope of the ground from the road shoulder to the bottom of the ditch is 3:1. Following this work, the traveled way shall be bladed in accordance with Activity Code 210: Blading Aggregate Surfaced Roads.

Lead Off and Tail Ditches Lead off and tail ditches are to be cleaned and reshaped to provide drainage away from the roadway. Material excavated from these ditches is not to be reclaimed onto the roadway.

Berm Removal

Berm removal shall consist of the removal of berms along the road shoulders which prohibit proper surface drainage. Material shall be removed to produce a proper cross slope or crown (1/4” to 1/2”).

Material shall be spread along foreslopes to assure proper road surface drainage. Following this work the traveled way shall be bladed in accordance with Activity 210: Blading Aggregate Surfaced Roads.

Cleaning of Culverts

230.03 - Maintenance Requirement - All culverts shall be inspected and cleaned as necessary of dirt, silt, muck, debris and vegetative matter to maintain a smooth flow of water. Silt shall be removed from culverts so that no more than 2” of debris is left in any pipe smaller than 18” diameter or 3” in culverts up to 36” diameter, except that all silt shall be removed from the first 3’at the outlet ends of culverts. Culverts larger than 36” shall be cleaned out of silt so that no more than 1/4 of the diameter of the pipe is silted in. Tail ditches at culverts shall be cleaned as needed to maintain the same grade and width as the pipe. Maximum distance to clean on any tail ditch shall be 10’. Culverts and tail ditches that are more than 1/2 silted in throughout its length shall be brought to the attention of the COR and shall be exempt from normal cleaning operations. This activity also includes debris removal at culvert inlets and outlets, catch basins, and other appurtenant structures. Grade ties with road ditches are considered a part of this activity.

Throughout this operation, care shall be taken to prevent damage to culvert pipes, end sections and all other appurtenances. All material, including brush, extracted in this operation shall be disposed of at the site outside the roadway and away from natural or man-made drainages.

Method of Measurement Road ditches and culvert cleaning will be measured to the nearest tenth of a mile, using an odometer calibrated over a measured mile. Measurement will be made for each side of the road. Lead off and tail ditches will be measured from the road ditch line to the end of the ditch to the nearest mile.

Payment will be made for all work of this activity, inspected and accepted, in accordance with the following pay item and unit price.

ACTIVITY CODE: 250 - EMERGENCY EQUIPMENT MOVEMENT

250.01 - Any movement or transportation of equipment to or from work area(s) required to pursue the work which was not scheduled in the maintenance schedule and the Contractor is required to respond within 24 hours will be authorized under this Activity Code. The Contractor will be compensated for each piece of equipment requested by the Contracting Officer to be moved to a location on the Sumter National Forest.

Method of Measurement

250.02 - Measurement will be paid to the nearest mile from existing location of the equipment on the Sumter National Forest. All measurements will be measured along the most feasible route ONE WAY. No mileage will be paid to return the equipment to its original location.

250.03 - Equipment requested to be moved designated as slow moving equipment (e.g. - Motorgrader that is required to have slow moving emblem) will be paid for under this pay item. No mileage will be allowed for delivery equipment (e.g. - transport) under this pay item.

Payment shall be made for all work of this activity, inspected and accepted, in accordance with the following items and unit price.
