SECTION VII
QUALITY ASSURANCE SURVEILLANCE PLAN
INTRODUCTION
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor actions while implementing this service contract. It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective in the Statement of Work (SOW) in section V of the contract.
The QASP provides a systematic method to evaluate the services the contractor is required to furnish.
This QASP is based on the premise the government desires to maintain a quality standard in operating and maintaining facilities and that a service contract to provide the service is the best means of achieving that objective.
The contractor, and not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control program approved at the beginning of the contract provides the measures needed to lead the contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.

STANDARD: Customer complaints shall not exceed the thresholds cited above for each performance objective. The COR shall notify the contracting officer for appropriate action in accordance with FAR 52.212.4, Contract Terms and Conditions-Commercial Items (May 1997) or the appropriate Inspection of Services clause, if any of the above service areas exceed the customer complaint thresholds.
PROCEDURES: The Contractor shall provide means of communication with the Government at all times. Any customer that observes unacceptable services, either incomplete or not performed, for any of the above performance objectives should immediately contact the COR or Government inspector.
After confirming the complaint as valid, the COR will complete appropriate documentation to record the complaint. The COR shall verbally notify the Contractor’s Quality Control Inspector (QCI) of the customer complaint. The QCI will complete appropriate documentation to record the complaint. The documentation will not be recorded as a customer complaint if proper and timely correction of the unacceptable condition(s) is accomplished. The QCI shall return to the COR the written customer complaint document, properly completed with actions taken. The COR will file the complaint for monitoring future recurring performance. Recurring valid customer complaints are not permitted within 30 days of the original complaint for any of the above service items. If a repeat customer complaint is received indicating the same deficiency during 30 days immediately following the complaint, the COR should contact the Contracting Officer for appropriate action.
INSPECTION
SURVEILLANCE: The government inspector will evaluate the services required by each delivery/task order to ensure complete compliance.
STANDARD: The contractor shall perform all work required by the delivery/task order in a satisfactory manner. The COR shall not consider the delivery/task order complete until all deficiencies have been corrected.
PROCEDURES: The government inspector will inspect all work tasks required by the delivery order to ensure contractor compliance in all appropriate paragraphs of Section V Statement of Work (SOW) each time service is performed. The inspector shall record results of inspection, noting the date and time of inspection. If inspection indicates unacceptable performance, notify the contract manager and QCI of the deficiencies and have QCI correct. Contractor shall be given three hours after notification to initiate correcting the deficiencies and two hours after initiation to complete the correction procedure.
The inspector shall not issue a receiving report accepting the services for the delivery/task order until all deficiencies have been corrected.
This contract calls for the contractor to perform the following tasks and subtasks and to perform inspections at the completion of the tasks/subtasks as a minimum and/or for intermediate inspections as determined at contract award by the COR/CO. The contractor’s quality control plan/schedule will be modified accordingly and will be one of the basis for the award of the contract.

Submission of an offer for the basic contract period constitutes an offer to perform in the Option Period(s), if exercised. The government anticipates making award to more than one offeror as a result of the solicitation. This project is set-aside 100% for small businesses. Award will be made based on the best value to the government and can be made on initial offers.
Award will be made to those offerors (1) whose proposals are technically acceptable (2) whose technical/cost relationship presents the best value to the Government and (3) past performance
The Government reserves the right to make cost/technical trade-offs that are in the best interest and advantageous to the Government. The critical factor in making any cost/technical trade-off is not the difference between the technical evaluations but the significance of the difference. The significance of the difference between the technical evaluations will be determined on the basis of what the difference might mean in terms of performance and what it would cost the Government to take advantage of that difference.
The Government may reject any or all offers if such action is determined to be in the best interest of the Government.

(f) Late submissions, modifications, revisions, and withdrawals of offers.
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation.
If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)
(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and—
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers;
or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government’s control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. 52.212-2 -- Evaluation -- Commercial Items. (Oct 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Past Performance.
Subfactors (Equal Order of Importance):
o Quality of Workmanship o Customer Satisfaction o Timeliness of Performance o Safety Record
NOTE: In accordance with FAR 15.305, in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror will not be evaluated either favorably or unfavorably in this area, but instead will receive a neutral rating in the area of past performance.
Experience and Availability of Firm, Key Personnel and Equipment.
Subfactors (Equal Order of Importance):
o Relevant Technical Knowledge o Ability to Perform the Work and Availability of Key Personnel o List of Available Equipment – Brand, Type, Age, Ownership Information, Etc.
o Utilization of Local Work Force
Price. A price evaluation will be performed to determine the reasonableness of the proposed price.
Reasonableness will be determined considering other competitive prices received and comparison to the independent Government estimate (IGE). The Government will look at whether a proposed price is too high and may also consider whether a proposed price is too low, since this might indicate a lack of technical understanding on the offeror's part or a performance risk associated with the proposal that the agency may not be willing to accept. Whether a price is too high or too low may be considered in the context of comparison to other price proposals as well as to the IGE or to any other information the contracting officer deems useful in his or her discretion. If multiple contract line items are included in the price schedule, prices will also be evaluated to determine whether any line items are unbalanced.
Offerors are cautioned to distribute costs appropriately.
Technical, past performance and price are all looked at as importance.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(End of Provision)
FAR 52.212-3 -- Offeror Representations and Certifications -- Commercial Items. (Dec 2014)
