Include a drawing with limits of the project.
Proposed Project Activities:
· Move the Forest Service gate back approximately 75 feet.
· Place dirt and gravel surfacing material to level the grade entering the access road.
· Construct a 6-10 car parking lot to the south side of the current access road. This will entail placing geotextile on the existing ground and then spreading 24” of PA 2A material and capping with 4” of DSA limestone.
· Place plastic curb stops along parking lot to block direct access to fields.
· Install an information kiosk.

V. Chapter 93 Designation.___ _____ Cold Water Fisheries (CWF)_____ _____ VI. Location and Description of BMPs.
Temporary and Permanent Erosion and Sedimentation Controls:
1. Off-site surface water will be diverted away from areas to be disturbed.
2. Borrow and disposal areas will be located outside of the floodplain and adequately buffered to prevent runoff from entering the stream.
3. Pollutants – Care will be taken to prevent pollutants from equipment working on or entering the site from entering the stream channel.
a. Operations will minimize the encroachment of equipment into riparian areas.
b. Equipment and vehicles will be thoroughly washed before entering each new site to prevent the introduction or spread of undesirable or non-native invasive species.
c. Vehicles will also be properly maintained, periodically checked for leaks throughout the work day, and never left parked overnight in riparian areas.
d. Operating supplies of oil, fuel, and other such materials will be stored outside of the riparian area.
4. Minimize the area of disturbance - The area of disturbance (e.g., the volume of vegetation removed and soil disturbed) will be kept to a minimum. Native vegetation will be preserved wherever possible, and the amount of soil disturbance will be kept to a minimum.
5. Best Management Practices (BMPs) for sediment and erosion control will be in place before earth disturbance activities begin.
6. BMP maintenance program - A maintenance program will be followed for all temporary and permanent erosion and sediment control BMPs.
a. Inspect BMPs once a week, and after each significant rainfall event.
b. Maintain BMPs to ensure effective and efficient operation. Cleanout, repair or replace, and re-grade or re-stabilize as needed.
Best Management Practices (BMPs) for sediment and erosion control include:
1. Temporary immediate stabilization will be provided during and upon completion of any earth disturbance activity that could affect water quality in streams.
a. Mulching is the FS preferred method for providing temporary soil stabilization where adequate native vegetation cannot be maintained.
b. Straw bales, windrowed slash, silt fence or other BMPs for erosion control and sediment removal will be used to control sheet flow runoff from small disturbed areas.
c. Temporary seed mixes – Areas could be seeded with winter wheat or winter rye to temporarily stabilize soils until native vegetation has an opportunity to become established (e.g., over winter.)
d. Temporary erosion control BMPs will be removed as soon as the permanent controls are established (e.g., remove silt fences once vegetation has grown in.)
2. Permanent stabilization will be provided for all areas within 72 hours of the completion of work.
a. Natural revegetation can be used in areas where erosion and sedimentation are not a concern, and a seed source from native species is present in sufficient numbers to naturally revegetate the area.
b. Seeding and mulching is the FS preferred method for providing permanent soil stabilization in areas where the native vegetation is sparse or revegetation is unlikely after appropriate site preparation. Where seeding and mulching will be used as final stabilization, the road work at stream crossings will be completed by September 30 in order for the seed to germinate and stabilize the soil before October 15. Soils in these sensitive areas will not be left exposed past these dates.
i. Seed mix will consist of native and desired non-native species.
ii. Straw mulch will be weed free.
c. Bioengineering techniques (e.g., willow stakes and facines) have been successfully used on the ANF and will be considered wherever practical. Only native and desired non-native species will be used.
d. Erodable streambanks will be protected using permanent vegetation, rock or large wood structures, geotextile, or other non-erosive materials.
e. Erodable streambed – Rock weirs and large structures will be constructed to prevent down-cutting in areas where it can be expected to occur.
3. In-channel excavations – Wherever possible, all in-channel work will be completed from the tops of the banks. Where this is not possible, in-channel work will be limited to the extent possible and channel entrances will be restored and stabilized.

Temporary immediate stabilization will be provided during and upon completion of any earth disturbance activity that could affect water quality in streams. Temporary erosion control BMPs will be removed as soon as the permanent controls are established. Permanent stabilization will be provided for all areas within 72 hours of the completion of work.

X. Maintenance.
BMPs will be inspected once a week, and after each significant rainfall event. BMPs will be maintained, cleaned out, repaired or replaced, and regraded or restabilized as needed to ensure effective and efficient operation.
Required: Should any measures contained within this plan prove incapable of adequately removing sediment from on-site flows prior to discharge, or of stabilizing the surface involved, additional measures will be implemented immediately by the property owner to eliminate all such problems.
Required: After final site stabilization has been achieved, temporary erosion and sedimentation controls will be removed. Areas disturbed during removal of the controls must be stabilized.
XI. Proper Measures for Recycling or Disposal of Materials.
Borrow and disposal areas will be located outside of the floodplain and adequately buffered to prevent runoff from entering the stream.
