157 - TEMPORARY SEDIMENT AND EROSION CONTROL

Foundation Scour Repair ONLY 08/22/2012 Delete Section 157 in its entirety and replace with the following:

157.01 This work consists of furnishing, constructing, and maintaining temporary sediment and erosion control measures. This includes protection of all phases of work during the life of the contract including storm events and removal of measures on completion of construction unless materials are a permanent part of the construction repair.

157.02 Furnish a material list and material specifications as part of the contractor technical proposal.

Construction Requirements

157.03 General.

10 days prior to the start of construction, submit a written Sediment and Erosion control plan that provides specific sediment and erosion control measures to minimize delivery of soil and turbidity into the stream during the construction period, including a partial channel diversion, temporary cofferdam construction, and site dewatering plan if the proposed repair requires dewatering of the construction site. Provide a plan that supports needed work in accordance with a contractor technical proposal that conforms to section 208. Include the sequence of operations and information on equipment, materials and suppliers to support temporary sediment and erosion control measures. The Sediment and Erosion Control Plan can be included in the technical proposal providing it is clearly identified as such. Once the plan or the technical proposal is approved by the CO, measures may be revised only with written approval by the CO.

Propose a plan that ensures that turbidity of the water 300 feet downstream from the construction site will not be visually greater than the turbidity of the water upstream of the project site.

When this turbidity requirement or other sediment and erosion control measures are not met, immediately take corrective action. Cease operations that are causing turbidity and pump or otherwise improve the diversion of the stream around the construction site according to this specification until the turbidity requirement can be met. When the interpretation of this requirement is in question, measure turbidity using a turbidity meter as approved by the CO, and provide documentation that operations are in compliance with FAR 52.236-7 Permits and Responsibilities, subsection 107.01 Laws to be Observed and subsection 107.10 Environmental Protection, and subsection 107.11, including but not limited to, the requirements of the National Marine Fisheries Service.

Do not begin work until the necessary controls for that particular phase of work have been implemented. Minimize the time required for sediment and erosion control features to be in place.

Operate in a manner that will avoid harm to aquatic organisms whenever possible.

Notify the CO of the intention to dewater the construction site, if required as part of the technical proposal, at least 10 days in advance. Do not divert stream flow to isolate and minimize flows in the construction site until approved by the CO. The CO will not approve a diversion until a fisheries biologist and other Government personnel are present and prepared to rescue aquatic organisms. Re-direct stream flows slowly and incrementally in order to facilitate the fish rescue.

The rescue operation will generally take several hours.

157.04 Controls and Limitations on Work.

Prior to construction, the CO will delineate critical riparian vegetation areas, wetlands, and other sensitive sites to prevent ground disturbance. Minimize disturbance to existing vegetation along stream.

Confine work within the construction limits marked by the Contracting Officer. Use existing roadways or travel paths whenever reasonable. Minimize the number of new access paths.

Dispose of waste material or contaminated water at approved sites.

For excavation or channel disturbing activities within the stream, use sediment control barriers or turbidity curtains at the downstream side of the excavation if turbidity levels specified in 157.03 are exceeded.

157.05 Filter Barriers. Construct silt fences and wattles, as needed, for filtering sediment from wastewater before wastewater re-enters the stream. Provide materials from a certified noxious weed free source.

157.06 Sediment Retention Structures. Construct sediment retention structures to detain wastewater pumped from the construction site and settle out sediment. Provide outlet protection.

157.07 Diversions.

Construct stream diversion, related appurtenances and measures as follows:

(a) In Stream Work.

1. Isolate construction area and coordinate with Contracting Officer for aquatic species removal by the Government. Coordinate aquatic species removal a minimum of 7 working days prior to beginning isolation of construction areas.

2. Cease project operations under high flow conditions that inundate the project area, except for efforts to avoid or minimize resource damage.

3. When approved, minimize time in which heavy equipment is in stream channels, riparian areas, and wetlands. Operate heavy equipment in streams only when Contracting Officer determines that such actions are the only reasonable alternative for implementation, or would result in less sediment in the stream channel or damage (short- or long-term) to the overall aquatic and riparian ecosystem relative to other alternatives.

4. Clean and repair all equipment used in the in-stream work prior to entering the project area. Remove external oil and grease, along with dirt and mud, prior to construction. Inspect equipment daily for leaks or accumulations of grease, and repair any identified problems prior to entering streams or areas that drain directly to streams or wetlands.

5. Fuel and service equipment used for in-stream or riparian work in an approved staging area outside of riparian zone. When not in use, store vehicles in the staging area.

(b) Temporary cofferdam. When partial stream diversion and construction site de-watering is required, construct a cofferdam of material handling bags as approved by the CO. The mining of material from the streambed or floodplain to construct the cofferdam is not permitted. Minimize the size of the cofferdam and ensure the partial stream diversion allows for aquatic species passage in the main channel while preventing aquatic species from entering the construction site within the confines of the cofferdam.

Use only clean sand or coarse concrete aggregate in the material handling bags to form the cofferdam. Loosely fill and tamp the bags in place to minimize seepage between, under, and around the bags. Place an impermeable membrane within or on the upstream side of the cofferdam and entrench it in the streambed. Ensure that turbidity requirements specified in subsection 157.03 are not exceeded during cofferdam construction and use.

During construction and use of the cofferdam, and prior to removing the cofferdam, pump any contaminated or sediment-laden water from the enclosure to a sediment retention structure. Ensure water leaving the sediment retention structure is free of sediment and turbidity prior to re-entering the stream.

In lieu of a sediment retention structure, use natural vegetation to disperse the soil and filter the contaminated or sediment-laden water. Discharge water onto areas of ground most advantageous for dispersal and filtration of sediment, e.g. flat heavily vegetated soil. When single point discharge does not function adequately, discharge water into a perforated pipe or series of pipes laid approximately level so that the brown water disperses over a wide area.

157.08 Maintenance & Cleanup.

When removing sandbags, spread sand away from the waterway; if coarse concrete aggregate meeting the requirements of Subsection 703.02 is used in the sandbags, the gravel may be distributed evenly across the waterway as approved by the CO.

Remove geotextile and other non-biodegradable materials used in dewatering and sediment control operations from Government property, unless otherwise approved by the CO.

157.09 Acceptance. Material for sediment control measures will be evaluated under Subsections

106.02 and 106.03.

Construction, maintenance, and removal of sediment and erosion control measures will be evaluated under Subsections 106.02 and 106.04.

Measurement and Payment

157.10 Do not measure Sediment and Erosion Control for payment. Compensation is made as an indirect payment.

171 - WEED AND DISEASE PREVENTION

171.00_nat_us_03_30_2005

171.01 This work consists of washing and treating construction equipment to remove seeds, plants, and plant fragments from the equipment before the equipment is used on National Forest System lands.

171.02 Conform to the following Subsection:

Water 725.01

Construction Requirements

171.03 General . Notify the CO in writing at least 10 days before moving any construction equipment onto National Forest System lands. 208 - STRUCTURE EXCAVATION AND FOUNDATION BACKFILL

Foundation Scour Repair ONLY 08/22//2012 Delete Section 208 in its entirety and replace with the following:

208.01 This work consists of a contractor developed and implemented technical proposal to perform excavation and foundation backfill work to re-establish foundation support for a bridge pier that has settled and may be subject to additional settlement due to scour below the pier. The pier is supported on two individual footings, which are further supported on a non-reinforced concrete seal footing. The entire pier and bridge have settled together. The work includes describing the proposed repair, including construction method, personnel, materials scheduling, and a safety plan; discussing the effectiveness of the proposed repair; identifying additional work at the site outside the scope of the proposal that would further protect the repair, such as placement of riprap; and demonstrating experience and effectiveness by identifying and describing past projects where similar work has been performed by the contractor making the technical proposal.

Incorporate the Construction Requirements in the subsections below in the technical proposal. This may include preserving and protecting active stream channels; in-stream excavation; preparing foundations; constructing cofferdams; dewatering; sealing foundations; backfilling with scour resistant materials; subsequent removal of safety features, sediment control features, and cofferdams; and other work required as part of the specific technical proposal.

Preferred construction methods and measures for the repair will include proposals that 1) do not require de-watering of the worksite, 2) do not allow any fresh concrete or grout to enter the active stream, 3) minimize the disturbance to the stream, and 4) minimize the amount of time the work will be performed in-stream. The technical merit of the proposals will be evaluated based on the effectiveness of the repair method, ability to incorporate preferred construction methods or show methods that exceed preferred options, as well as the experience of the contractor to perform the work being proposed.

Following Notice to Proceed, submit a Final Technical Repair Proposal to the CO a minimum of 10 days before beginning actual site work. As part of both the Request for Proposal as well as the Final Technical Repair Proposal, include a work schedule that outlines duration of construction, the planned mobilization schedule, and the duration of anticipated in-stream work.

Material

208.02 Furnish a material list and material specifications as part of the contractor technical proposal. For concrete or grout to be placed underwater or tremied into standing water as part of the backfill repair, include anti-washout admixture or discuss why it is not needed or recommended.

Construction Requirements

208.03 Preparation for Structure Excavation. Clear the area of obstructions according to Sections 201 and 203. Prepare the worksite to perform work by underwater methods with diver support or by work site isolation and dewatering in accordance with section 157. Survey and document the extent of the scour under the seal footing using hand measuring devices in accordance with section 152, prior to performing any excavation or the actual repair work.

208.04 General. Excavate loose non-compacted material from below the existing seal footing.

Ensure turbidity limits specified in 157.03 are not exceeded during excavation. When excavation is complete, request approval as to the character and suitability of the foundation material. Ensure the foundation surface will provide uniform support for the repair throughout its length and width, unless construction methods outlined in the technical proposal can accommodate inconsistencies while still meeting the overall intent of the proposed repair.

Follow OSHA safety regulations for all diving and construction activities, including working within a temporary cofferdam if that is part of the technical proposal.

Do not release non-native excavated material into the waterway. As much as possible, use excavated materials to fill any voids in the stream channel adjacent to the worksite, where the future installation of riprap may protect that placement. Dispose of any non-native material removed as part of the excavation off National Forest System lands.

Include provisions for performing work underwater in the technical proposal or propose de-watering the worksite as an alternative method.

208.05 Channel Preservation. Perform work in or next to a running waterway as follows:

(a) Propose work to be performed underwater using divers and underwater construction equipment and methods; or excavate inside cofferdams, sheeting, or other separations such as dikes or sandbags;

(b) Do not disturb the natural bed of the waterway adjacent to the work, except as allowed in subsection 208.04.

208.06 Cofferdams. If included in the technical proposal, use cofferdams when excavating under water or when the excavation is affected by groundwater.

Submit working drawings as part of the technical proposal showing proposed methods and construction details of temporary cofferdams according to Subsection 104.03 and Section 157.

Cofferdams shall:

(a) Extend below the bottom of the footing;

(b) Be braced to withstand pressure without buckling and secured in place to prevent tipping or movement;

(c) Be as watertight as practicable;

(d) Provide sufficient clearance for the placement of forms and the inspection of their exteriors;

(e) Provide for dewatering;

(f) Protect fresh concrete against damage from sudden rises in water elevation; and

(g) Prevent damage to the foundation by erosion.

When no longer required, remove all cofferdam material down to the natural bed of the waterway.

Remove cofferdam material outside the waterway to a minimum of 2 feet below the surface of the finished ground.

208.07 Foundation Seal. Where a foundation area cannot be pumped reasonably free of water, provide a foundation seal of seal concrete. Design seal concrete according to Subsection 552.03, including an anti-washout admixture as needed.

Furnish and place seal concrete according to Section 552. While placing a foundation seal, maintain the water level inside the cofferdam at the same level as the water outside the cofferdam.

Ensure that concrete is sufficiently cured or dried before coming into contact with stream flow.

Do not dewater a concrete-sealed cofferdam until the concrete strength is sufficient to withstand the hydrostatic pressure. Ensure that there is effective separation between the seal and the cofferdam so that the temporary cofferdam can be removed at the completion of construction without damaging the new concrete.

208.08 Dewatering. While placing concrete, locate and operate the pumps outside the foundation form. If pumping is permitted from the interior of any foundation enclosure, pump in a manner to avoid removal or disturbance of concrete material.

208.09 Foundation Preparation. Excavate unstable and unsuitable material below foundation grade. Specify the mechanism of excavation in the technical proposal. When preparing the technical proposal, estimate the extent of excavation needed to prepare the foundation. Once excavation and foundation preparation have been approved by the CO, document the extent of the required excavation as part of the as-built plans for the project.

208.10 Backfill. Backfill any material excavated as part of foundation preparation, as well as the void created by channel scour, with permanent materials such as concrete and/or steel. Submit a technical proposal with the materials to be used, the method of repair, personnel required, and the construction schedule. In addition to the technical merit and effectiveness of the repair, as discussed in the proposal, preferred construction methods and measures for the repair will include proposals that:

(a) do not require de-watering of the worksite

(b) do not allow any fresh concrete or grout to enter the stream

(c) minimize the disturbance to the stream

(d) minimize the amount of time the work will be performed in-stream

Minimum backfill limits are the neat lines of the seal of the west pier footing, as shown on the As- Built plans, Section J.1, Exhibit b. Maximum backfill limits are 6 feet beyond the neat lines of the seal of the west pier footing. For any material placed outside the neat lines of the footings, do not obstruct the natural channel flow.

As part of the technical proposal discuss how material placed outside the neat lines of the footings will act to provide long-term protection of the repair and the original footing. For any placement, either within the neat lines or outside the neat lines, recommend mitigation measures, such as riprap, that will provide for additional protection of the repair and the adjacent channel.

Recommend the sizing and extents or dimensions of any materials to be placed.

208.11 Acceptance.

Material to be used in the repair will be evaluated under Subsections 106.02 and 106.04.

Structure excavation and backfill work will be evaluated under Subsections 106.02 and 106.04.

Cofferdams will be evaluated under Subsections 106.02 and 106.04.

Clearing and removal of obstructions will be evaluated under Sections 201 and 203.

Seal concrete will be evaluated under Section 552.

Measurement

208.12 Measure the Section 208 items listed in the bid schedule according to Subsection 109.02 and the following.

Measure structural excavation by the cubic yard in its original position. Do not include the following volumes in structure excavation:

(a) Material excavated outside vertical planes located outside and parallel to the limits of the existing concrete seal;

(b) Water or other liquid material;

(d) Material excavated before measurements of the original ground; or

(e) Material rehandled in to fill voids in the adjacent channel.

Measure foundation fill based on the materials used in its construction.

Do not measure seal concrete used in the optional cofferdam construction.

Payment

208.13 The accepted quantities measured as provided in Subsection 109.02 and above, will be paid at the contract price per unit of measurement for the Section 208 pay items listed in the bid schedule. Payment will be full compensation for the work prescribed in this Section. See Subsection 109.05.
