STATEMENT OF WORK
1. PART 1 – GENERAL
MARYS PEAK WATER SYSTEM DECOMMISSIONING
1.01 SCOPE OF PROJECT
A. This project consists of demolition of three concrete water tank structures and demolition and removal of two wooden pump-houses with their concrete foundations, roofing and insulation materials, plumbing, mechanical and electrical equipment, and appurtenances. Work shall be performed according to the Statement of Work and Specifications stated in this contract.
GENERAL. Contractor shall:
1. Ensure that electrical service in pump-houses is disconnected and that electrical equipment to be removed is de-energized prior to commencement of work.
2. Remove and dispose of off National Forest all electrical and mechanical equipment from pump-houses including plumbing and appurtenances prior to any demolition activities.
3. Remove and dispose of off National Forest all component materials from pump-houses, including their concrete foundations.
4. Remove and dispose of off National Forest any exposed piping (including corrugated pipes) from work areas; Cut and cap any plumbing that is to remain in the ground.
5. Demolish and abandon in place three concrete water tank structures.
6. Access project sites using routes flagged by the Contracting Officer (CO). Use same routes to perform construction activities in order to minimize impact on vegetation and soil.
7. Use small motorized equipment and excavators or small walking excavator (no more than 6 ft wide) to perform demolition activities.
8. Restore disturbed ground soil to match to natural site conditions. Use native materials as indicated by the CO.

10. The intent of this Contract is to complete all work at the Mary’s Peak in the summer 2011. At this time, a major road reconstruction project is scheduled to start, and the road to access to the project site will be closed for 45 days. After the 45 day closure, it is likely that the roads to the site will be covered by snow for the winter. Contractor may disclose the road closures to get access to the project site.
B. SITE SPECIFIC CONDITIONS. Contractor shall:

1. Remove or decommission in place (demolition) concrete water tank with dimensions as describe on picture No. 1.

2. Remove and dispose of electrical panel, mechanical and electrical equipment, plumbing, and appurtenances from pump house prior to any demolition activities; remove and dispose of 3 corrugated metal pipes with approximate dimensions as describe on Picture No. 5; and remove or decommission in place concrete water tank with dimensions as describe on picture No. 4.
Site No. 3:
3. Remove and dispose of electrical panel, mechanical and electrical equipment (including two metal pressure tanks), plumbing, and appurtenances from pump house prior to any demolition activities; remove and dispose of metal pipes located near the pump-house; and remove or decommission in place concrete water tank with dimensions as describe on picture No. 6.
4. Remove a small concrete dam from the Parker stream. Submit a dewatering and soil erosion plan prior to any demolition activities for site No. 3. Obtain CO approval for plan prior to any construction. Concrete dam structure and dimensions described in picture No. 8.

Dimensions of the building are approximately 5 ft – 4 in by 5ft – 11 in, by 8 ft high.
Building is supported by a concrete foundation that is approximately 12 inches thick.
Riser with concrete blocks, approximately 3 ft – 9 in wide by 4.1 ft long, by 2 ft high.
Raiser is seated on top of a concrete water tank. Concrete water tank is approximately 12 ft wide by 12 ft long, by 8 ft high. Water tank level is approximately 1 ft.

Building sits on a concrete foundation that is approximately 1 ft thick.
The pump house is also seated on top of a concrete water tank. Water tank is approximately 6 ft diameter by 6 ft high, by 6 in thick.

There is a water intake, a metal pipe 3 inch diameter that is attached to the concrete dam.

Access to site No. 3 Hike on existing trail approximately
0.2 miles from the parking lot located near Mary’s Peak Campground to the project site (trail leading to Mary’s Peak). Existing trail is approximately 4 feet wide.
For access to construction activities, use small equipment such as a small back hoe or excavator. A temporary trail (approximately 150 feet) may need to be built. Restore conditions after demolition activities have been performed.
To access site No. 3, use existing trail. On existing trail, create an access path near pump-house downhill towards existing structure. CO will flag route access in presence of contractor.
Contractor shall cut and removed off the access trail to allow access to construction site the following:
Trees to be cut: 1 tree 3” DBH, 3 trees 6” DBH Stumps: 2 old stumps approx. 11” DBH Logs: 3 logs 11” DBH, 3 logs of 24” DBH, and 5 logs 33” DBH
Other vegetation in forest ground such as fallen trees, logs, and stumps may have to be partially cut and remove to allow full access to the project site No. 3.
SECTION 01300
1.PART 1 – GENERAL
MOBILIZATION
1.01 SCOPE
A. This work shall consist of preparatory work and operations necessary for the movement of personnel, equipment, supplies, labor, and incidentals to the project site, and for all other work and operations that must be performed or that cause costs to be incurred prior to beginning work on the various items on the project site. Bonding costs are included and paid under this section.
1.02 MEASUREMENT AND PAYMENT
A. Progress Payments
1.03 PROGRESS PAYMENTS WILL BE MADE AS FOLLOWS:
1. When 5 percent or more of the original contract amount is earned from other pay items, 50 percent of the amount for mobilization, or 5 percent of the original contract amount, whichever is less, will be paid.
2. When 10 percent or more of the original contract amount is earned from other pay items, 100 percent of the amount for mobilization, or 10 percent of the original contract amount, whichever is less, will be paid.
3. Upon completion of all work on the project, any unpaid amount for mobilization will be paid.
4. The total of all payments shall not exceed the original contract amount for this item.

SECTION 02210
2.PART 1 – GENERAL
DEMOLITION
1.01 SUMMARY
A. Site 1, 2, and 3: Contractor shall remove/decommission in place of 3 concrete water tank structures and remove and dispose of off National Forest their appurtenances according to applicable laws and this Specification.
B. Site 2 and 3: Contractor shall demolish and dispose of off Government land two wooden pump houses and their materials and components. Materials and components include concrete foundations, roofing materials, siding and insulation, light fixtures, mechanical and electrical equipment, plumbing, and appurtenances.
C. Site No. 2: Contractor shall demolition and disposed off Government land 3 corrugated pipes and any associated material.
D. Site No. 3: Contractor shall demolition and disposed off Government land concrete dam structure and any metal associated with the dam.
1.02 SUBMITTALS
A. Contractor shall provide a Hazardous Conditions Safety Plan to the CO prior to commencement of demolition work.
B. Contractor shall submit a dewatering and soil erosion plan prior to any demolition activities for site No. 3.
C. Demolition schedule activities.
1.03 QUALITY ASSURANCE
A. Regulatory Requirements: Comply with Oregon DEQ notification regulations before starting demolition. Comply with hauling and disposal regulations of authorities having jurisdiction. Contractor shall obtain and pay required ODEQ State permits.
Include these costs under mobilization.
B. Standards: Comply with the following:
1. ANSI A10.6: “Safety Requirements for Construction and Demolition.”
2. Applicable local codes for demolition work, safety of structure, and dust control, including Oregon OSHA.
3.2. PART 2 – PRODUCTS
2.01 RESTORATION OF GROUND
A. Site No.3: Restore site conditions after temporary access is not longer needed. Use native materials that visually match existing ground conditions. Perform activities as indicated by the CO.

PART 3 – EXECUTION
3.01 SITE ASSESSMENT
A. Prior to initiate construction activities, assess with CO site conditions and access routes to project sites. Identify structures, equipment, and materials to be demolished and materials to be removed off government land.
B. Site No. 3: Prior to construction activities, identify access route and vegetation to be cut and removed. Access route(s) will be flagged by the CO.
C. When applicable, verify that electrical service and lines have been de-energized.
3.02 ENVIRONMENTAL CONTROLS
A. Protection of Natural Resources: Preserve natural resources within project boundaries and restore them to match existing natural conditions.
A. Site No.3: After decommissioning the concrete water tank and removing and disposing of the concrete dam, the creek channel shall be restored to match existing slopes and ground conditions.
B. Contractor shall restore temporary trails to access Site No.2 and Site No. 3. Restore trails to match conditions prior to construction work.
B. Water Resources: Comply with applicable regulations concerning the direct or indirect discharge of pollutants to underground and natural surface waters.
C. Dust Control, Air Pollution, and Odor Control: Prevent creation of dust, air pollution and odors.
D. Use temporary enclosures and other appropriate methods to limit dust and dirt rising and scattering in air to lowest practical level.
E. Properly maintain equipment to reduce gaseous pollutant emissions.
3.03 DEMOLITION – GENERAL REQUIREMENTS
A. Remove demolished materials from site as work progresses.
B. Do not burn demolished materials on site.
C. Transport demolished materials off Government property and legally dispose of them.
D. The Contractor shall report any signs or indications of contaminants and shall cease operations until the Contracting Officer has approved the resumption.
3.04 PUMP HOUSE – DEMOLITION
A. Ensure electrical panels and electrical equipment is de-energized.
B. Contractor shall remove any associated electrical, mechanical, plumbing, and any other attached components from the structures off Government’s property and legally dispose of them.

3.05 WATER TANK DEMOLITION
A. Water Tank Demolition: Use methods required to complete work within limitations of governing regulations and as follows:
1. Contractor shall disconnect, remove, and dispose of all electrical, mechanical, and plumbing components prior demolition.
2. Contractor shall cut and cap water lines to remain in the ground, except for water lines located 2 feet or more under the surface ground.
3. Contractor shall remove or decommission in place concrete water tank structures. Contractor shall ensure that concrete foundations and walls have been fractured and/or destroyed in order to allow natural drainage.
4. Contractor shall not leave any concrete structures or concrete pieces laying on the surface ground. Contractor shall remove and dispose of all concrete exposed on the ground and up to 2 feet below the ground surface. Break concrete structures in small pieces.
5. Fill tank voids with broken pieces of concrete and imported washed material such as sand, or 1 inch minus crush aggregate or gravel. Compact in layers using industry standards for compaction.
3.06 DAM REMOVAL
A. Prior to any demolition work, contractor shall implement the dewatering and soil erosion control plan
B. Concrete dam structure will be cut along edges of riparian area. Work to be executed in presence of the CO.
3.07 SOIL EROTION CONTROL
A. 15 days prior to the start of construction, submit a written soil erosion and dewatering plan that provides specific sediment control measures to minimize delivery of soil and turbidity into the stream during the construction period. Include the sequence of operations and information on equipment, materials and suppliers.
B. Stream Bypass Dam and Pipe.
(1) Bypass pipe. Place bypass flexible pipe as approved by the CO. Place the upstream invert of the pipe at the lowest point in the stream channel as practical. Install joints and elbows as needed to accommodate the site layout. Do not place backfill until the pipe joints have been approved by the CO. Allow water to pass through pipe only after a downstream splash apron has been prepared in a manner that will protect the stream from scour and turbidity, and protect aquatic organisms from harm. Construct the bypass in a manner that avoids injury to organisms.
(2) Downstream Dam. When water flows into the work area from downstream, construct a cofferdam as needed to prevent water from entering the work area.
(3) Bypass Pipe Collar. Install and maintain a leak-proof pipe collar as approved by the
CO.

C. Do not begin work until the necessary controls for that particular phase of work have been implemented. Incorporate all erosion control features into the project at the earliest practicable time, as agreed by the CO. Operate in a manner that will avoid harm to aquatic organisms whenever possible.
A. Notify the CO of the intention to dewater the stream, at least 72 hours in advance (not including weekends and holidays). Do not re-route the stream until approved by the CO. The CO will not approve dewatering until a Government personnel is present.
3.08 SITE CLEANUP
A. Contractor shall restore roads, parking lots, pathways, access trails, and landscapes damaged as a result of demolition activities and activities related to this contract.
Construction activities include but are not limited to mobilization, clearing, demolition, and hauling of and transporting removed materials. Restore sites to conditions found prior to execution of this contract. Reconstruction shall be directed by the CO.
B. Contractor shall remove and dispose of off National Forest all demolition debris (except as stated to remain) as a result of demolition activities.
4.PART 4 – MEASUREMENT AND PAYMENT
4.01 MEASUREMENT
A. Measurement will be lump sum for work activities conducted and described for and at each site, including but not limited to removal or demolition in place of concrete water tank structures, removal and disposal of pump-houses (with their foundations, appurtenances, electrical and mechanical equipment), removal and disposal of corrugated pipes located near both pump-houses, and removal and disposal of the concrete dam structure.
4.02 PAYMENT
A. Payment will be made at the contract unit price for work executed at each site, and related work activities accepted.

SECTION 02230
1. PART 1 – GENERAL
CLEARING AND GRUBBING
1.01 SUMMARY
A. This section specifies clearing, grubbing, removing, and disposing of all vegetation and debris within the clearing limits except objects designated to remain. Work includes preserving objects designated to remain.
B. Clearing Limits.
1. Clearing limits are within 3 feet on either side of trail centerline (unless otherwise staked on the ground).
2. Clearing limits are within 5 feet on either side of any building structure, including water tanks, pump-houses, and concrete dam (unless otherwise staked on the ground).
1.02 RELATED SECTIONS
Section 02210 – Demolition
1.03 DEFINITIONS
Merchantable Timber.
1. All logs and poles, at least 1/3 sound, having a minimum diameter of 6
2. inches under the bark, a minimum length of 8 feet, and a minimum of 10 board feet sound wood shall be considered merchantable timber.

3.01 SURFACE PREPARATION
A. Clearing. (Note: Contractor is required to perform a site visit to verify site conditions and access routes prior to Construction). Clear only trees designated for removal.
Forest Service will flag trees for removal.
B. Clear brush and shrubs within clearing limits subject to irreparable damage by construction operations.
C. Cut stumps to within 12 inches of the ground surface.
D. Grubbing.
1. Grub all stumps, brush, and roots (to a minimum 3” diameter) within the clearing limits.

3.02 TREE REMOVAL AND PRESERVATION (IF NECESSARY)
A. Falling of Trees.
1. Fall trees toward the center of the area to be cleared.
2. Use controlled falling to prevent injury or defacement to structures, other trees, or property.
B. Treatment of Trees and Shrubs Damaged by Contractor.
1. Trim branches flush with tree.
2. Within 3 days of the damage, treat all cut or scarred surfaces of trees or shrubs designated to remain. Use a product prepared especially for tree surgery.
3.03 DISPOSAL OF MERCHANTABLE TIMBER
A. Merchantable timber becomes the property of the Contractor.
3.04 CLEARING FOR STREAM
A. Minimize disruption of existing landscape.
B. Do not remove any trees or other vegetative material outside the clearing limit lines without approval of the CO.
4. PART 4 - MEASUREMENT AND PAYMENT
A. Measurement and payment for the work under this Section will be considered incidental to the work in other Sections of this Contract.

| B. Water Resources: Comply with applicable regulations concerning the direct or indirect discharge of pollutants to underground and natural surface waters. |
| C. Dust Control, Air Pollution, and Odor Control: Prevent creation of dust, air pollution and odors. |
| D. Use temporary enclosures and other appropriate methods to limit dust and dirt rising and scattering in air to lowest practical level. |
| E. Properly maintain equipment to reduce gaseous pollutant emissions. |
| 3.03 DEMOLITION – GENERAL REQUIREMENTS |
| A. Remove demolished materials from site as work progresses. |
| B. Do not burn demolished materials on site. |
| C. Transport demolished materials off Government property and legally dispose of them. |
| D. The Contractor shall report any signs or indications of contaminants and shall cease operations until the Contracting Officer has approved the resumption. |
| 3.04 PUMP HOUSE – DEMOLITION |
| A. Ensure electrical panels and electrical equipment is de-energized. |
| B. Contractor shall remove any associated electrical, mechanical, plumbing, and any other attached components from the structures off Government’s property and legally dispose of them. |
| 3.05 WATER TANK DEMOLITION |
| A. Water Tank Demolition: Use methods required to complete work within limitations of governing regulations and as follows: |
| 1. Contractor shall disconnect, remove, and dispose of all electrical, mechanical, and plumbing components prior demolition. |
| 2. Contractor shall cut and cap water lines to remain in the ground, except for water lines located 2 feet or more under the surface ground. |
| 3. Contractor shall remove or decommission in place concrete water tank structures. Contractor shall ensure that concrete foundations and walls have been fractured and/or destroyed in order to allow natural drainage. |
| 4. Contractor shall not leave any concrete structures or concrete pieces laying on the surface ground. Contractor shall remove and dispose of all concrete exposed on the ground and up to 2 feet below the ground surface. Break concrete structures in small pieces. |
| 5. Fill tank voids with broken pieces of concrete and imported washed material such as sand, or 1 inch minus crush aggregate or gravel. Compact in layers using industry standards for compaction. |
| 3.06 DAM REMOVAL |
| A. Prior to any demolition work, contractor shall implement the dewatering and soil erosion control plan |
| B. Concrete dam structure will be cut along edges of riparian area. Work to be executed in presence of the CO. |
| 3.07 SOIL EROTION CONTROL |
| A. 15 days prior to the start of construction, submit a written soil erosion and dewatering plan that provides specific sediment control measures to minimize delivery of soil and turbidity into the stream during the construction period. Include the sequence of operations and information on equipment, materials and suppliers. |
| B. Stream Bypass Dam and Pipe. |
| C. Do not begin work until the necessary controls for that particular phase of work have been implemented. Incorporate all erosion control features into the project at the earliest practicable time, as agreed by the CO. Operate in a manner that will avoid harm to aquatic organisms whenever possible. |
| A. Notify the CO of the intention to dewater the stream, at least 72 hours in advance (not including weekends and holidays). Do not re-route the stream until approved by the CO. The CO will not approve dewatering until a Government personnel is present. |
| 3.08 SITE CLEANUP |
| A. Contractor shall restore roads, parking lots, pathways, access trails, and landscapes damaged as a result of demolition activities and activities related to this contract. Construction activities include but are not limited to mobilization, clearing, demolition, and hauling of and transporting removed materials. Restore sites to conditions found prior to execution of this contract. Reconstruction shall be directed by the CO. |
| B. Contractor shall remove and dispose of off National Forest all demolition debris (except as stated to remain) as a result of demolition activities. |
| 4. PART 4 – MEASUREMENT AND PAYMENT |
| 4.01 MEASUREMENT |
| A. Measurement will be lump sum for work activities conducted and described for and at each site, including but not limited to removal or demolition in place of concrete water tank structures, removal and disposal of pump-houses (with their foundations, appurtenances, electrical and mechanical equipment), removal and disposal of corrugated pipes located near both pump-houses, and removal and disposal of the concrete dam structure. |
| 4.02 PAYMENT |
| A. Payment will be made at the contract unit price for work executed at each site, and related work activities accepted. |

| A. This section specifies clearing, grubbing, removing, and disposing of all vegetation and debris within the clearing limits except objects designated to remain. Work includes preserving objects designated to remain. |

| 3.01 SURFACE PREPARATION |
| A. Clearing. (Note: Contractor is required to perform a site visit to verify site conditions and access routes prior to Construction). Clear only trees designated for removal. Forest Service will flag trees for removal. |
| B. Clear brush and shrubs within clearing limits subject to irreparable damage by construction operations. |
| C. Cut stumps to within 12 inches of the ground surface. |

| 1. Grub all stumps, brush, and roots (to a minimum 3” diameter) within the clearing limits. |
| 3.02 TREE REMOVAL AND PRESERVATION (IF NECESSARY) |
