About this file

This is an Affirmation and Disclosure Form required by the State of Ohio for contractors bidding on the South Bass Island State Park contract opportunity. The form mandates that contractors affirm compliance with Executive Orders prohibiting the performance of offshore services, locating state data offshore, and purchasing from Russian institutions or companies. Contractors must disclose all locations where services will be performed, where state data will be located, and the principal business locations of both the contractor and any subcontractors involved in the contract. The form requires detailed address information for each entity and location across multiple sections to ensure full transparency regarding service delivery and data management.

Contractors and their subcontractors are under a continuing duty to disclose any changes or shifts in service performance locations before, during, and after contract execution and must notify the State immediately of any such changes. The State reserves the right to immediately terminate the contract if services are performed outside the United States unless a duly signed waiver has been obtained from the State in advance. The contractor's authorized representative must sign and date the form, certifying that they have read, understood, and agree to these terms, which become incorporated into any resulting contract with the State.
