About this file
This is a required Affirmation and Disclosure Form (Rev. 3/22) that contractors must complete to affirm compliance with Executive Orders regarding prohibitions on offshore services, offshore data storage, and purchasing from Russian institutions or companies. The form requires contractors to disclose the physical locations where their business operates, where contract services will be performed, and where state data will be stored, including any locations used by subcontractors.
The form establishes key requirements around data and service location transparency, mandating that contractors must immediately notify the state of any changes in service locations before, during, or after contract execution. The state reserves the right to immediately terminate contracts if services are moved outside the United States without an approved waiver. The document requires signature from an authorized contractor representative to acknowledge these terms and affirm their incorporation into any resulting contract with the state.
