PERFORMANCE-BASED WORK STATEMENT (PWS)

FOR

AEROFLEX AND AUTOMATED DISPLAY AND INDICATOR COMMON TEST SOLUTION (ADICTS)

TEST SYSTEM MAINTENANCE SERVICES

FOR

523rd and 524th ELECTRONICS MAINTENANCE SQUADRON (523/524 EMXS)

1. DESCRIPTION OF SERVICES:

1.1 This requirement is to provide on-call technical telephone/email support, logistics, preventative/on-site maintenance, and calibration to include: Test; Measurement; and

Diagnostic Equipment (TMDE) assistance, as well as software support for two (2) each of the Aeroflex 5800 Series Test Systems, their associated Interface Test Adapters (ITA’s), scheduled calibrations for two (2) 5820 Automated Test Equipment (ATE) Subsystems

(located within the Aeroflex), and ADICTS tester and its associated ITAs in support of the

523/524 EMXS.

NOTE: From this point forward, the Aeroflex 5800 Series Test Systems will be referred to as

‘Aeroflex Testers.’ Likewise, the Aeroflex 5820 ATE Subsystems will be referred to as the

‘Aeroflex 5820s.’

The Contractor shall provide personnel, sustainment engineering, the repair of spare parts, equipment, tools, materials, and other items necessary to complete technical telephone/email support, logistics, preventative/on-site maintenance, and all calibration services required within the scope of this PWS. After repairs are completed, the Contractor shall ensure the affected

Aeroflex Testers and/or their associated ITA’s are fully functional using verification testing and

Acceptance Test Procedures (ATP). The Contractor shall ensure the ADICTS tester having maintenance performed on it is fully functional, to the maximum extent possible dependent upon the availability of parts, using verification testing and ATP prior to departure. The contractor shall comply with this PWS in its entirety.

2. BASIC SERVICES:

2.1 The Contractor shall provide the Government all parts, labor, materials, software and hardware, along with on-call technical telephone/email assistance and on-site repair services for the Aeroflex Testers, Aeroflex 5820s, ADICTS tester and their ITA’s. The Contractor shall provide technical assistance by telephone, e-mail, or on-site to provide troubleshooting, diagnostics, maintenance, and repair of problems when found with the suspect Aeroflex Tester, Aeroflex 5820, ADICTS tester, and their associated ITAs, support equipment, parts, and/or software. The contractor shall provide the required services in accordance with (IAW) applicable guidance/regulations and/or manufacturer’s maintenance manuals.

2.1.1 Aeroflex Pre-Inspection/Self-Test

2.1.1.1 The Contractor shall perform a pre-inspection/self-test on the Aeroflex

Tester upon each visit or as required.

NOTE: It is understood the phrase ‘each visit’ found throughout this document indicates the following: The Contractor shall perform the indicated actions during

‘each visit’ when: 1) a problem exists with one or both of the Aeroflex Testers and

2) the Contractor is summoned by the Wage Leader (WL) or above to provide assistance with that problem. The Contractor will not arbitrarily perform a pre inspection/self-test on any Aeroflex Tester, Aeroflex 5820, and/or their associated

ITAs without having a specific reason or Government approval to do so.

The Contractor shall then provide a report, which includes recommended repair actions to be taken, required parts and a cost estimate, to the WL or above within five (5) business days of the visit. The WL or above and COR will give approval prior to any repairs being accomplished. (CDRL A001, Pre-Inspection/Self-

Test)

2.1.2 ADICTS Pre-Inspection/Self-Test

2.1.2.1 The Contractor shall perform inspection/self-test on each ADICTS as a minimum, and a pre-inspection/self-test on the ADICTS tester and/or it’s associated ITAs upon each visit.

NOTE: It is understood the phrase ‘each visit’ found throughout this document indicates the following: The Contractor shall perform the indicated actions during

‘each visit’ when 1) a problem exists with the ADICTS tester and 2) the

Contractor is summoned by WL or above and Contracting Officer Representative

(COR) to provide assistance with that problem. The Contractor will not arbitrarily perform a pre-inspection/self-test on any ADICTS tester and/or it’s associated ITAs without having a specific reason for doing so.

The Contractor shall then provide a report, which includes recommended repair actions to be taken, required parts and a cost estimate, to the WL or above within five (5) business days of the visit. The WL or above and COR will give approval prior to any repairs being accomplished. (CDRL A001, Pre-Inspection/Self-

Test)

2.1.3 On-Call Technical E-mail/Phone Support

(estimated 40 hours per year)

2.1.3.1 The Contractor shall provide a primary and secondary phone number and email addresses, as contact details to enable the WL or above, to effectively communicate. The Contractor shall have up to two (2) business days after

Government request technical support (includes: on-call email/ phone support with troubleshooting, diagnostics and repair) to respond to WL or above. The

Contractor shall notify the Government of any point of contact changes.

2.1.4 Aeroflex On-Site Maintenance and/or Repair

(estimated 140 hours and $20K in material per year)

2.1.4.1 The Contractor shall repair and calibrate the Aeroflex Testers, Aeroflex

5820s, and/or their associated ITAs as required IAW the Original Equipment

Manufacturer (OEM) standard.

2.1.4.1.1 The Contractor shall perform a successful self-test and ATP upon completion of any repairs and provide results to the WL or above immediately after completion of the test. All maintenance will be documented using an Air Force Techinal Order (AFTO) form 244 IAW

Technical Order (TO) 00-20-1.

2.1.4.1.2 Repair of spares will be performed at the contractor facility.

2.1.4.2 The Contractor shall provide a technical report to include: suggested solutions; course of repair action; recommendations of all diagnoses, repairs, and solutions; repair actions taken; results of repairs; and present a copy of the report to include parts replaced and itemized list of labor hours expended to the

Government for the failures found on the Aeroflex Testers, Aeroflex 5820s, and/or their associated ITAs. The Technical Report shall be provided by the contractor and signed by Contractor and WL, or above. The Technical Report shall be provided to the WL or above, and COR within fifteen (15) days. Then, once the contractor is contacted by government, the contractor shall have a maximum of thirty (30) calendar days to begin on-site repairs. (CDRL A002, On-

Site Maintenance and/or Repair).

2.1.4.3 If for any reason the contractor is not able to meet this deadline, then the government may request that the contractor provide any parts, tooling, or instructions necessary to perform the needed repairs.

2.1.5 ADICTS On-Site Maintenance and/or Repair

(140 hours and $20K in material per year)

2.1.5.1 The Contractor shall repair the ADICTS Tester, and/or their associated

ITAs as required IAW the OEM standard.

2.1.5.1.1 The Contractor shall perform a successful self-test and ATP upon completion of any repairs and provide results to the WL or above immediately after completion of the test. All maintenance will be documented using an AFTO form 244 IAW TO 00-20-1.

2.1.5.2 The Contractor shall repair the ADICTS tester and/or its associated ITAs and associated equipment listed in the following Technical Manual/Work

Instruction (TM/WI) and TO: IAW TM/WI F-22-D-OO-033; TM/WI F-22-D-

OO-103; TM/WI F-22-D-OO-036; TM/WI F-22-D-OO-122; TO 5N29-22-3; and

TO 5N29-22-8-3.

2.1.5.3 The Contractor shall provide a technical report to include: suggested solutions; course of repair action; recommendations of all diagnoses, repairs, and solutions; repair actions taken; results of repairs; and present a copy of the report to include parts replaced and itemized list of labor hours expended to the

Government for the failures found on the ADICTS Tester and/or their associated

ITAs. The Technical Report shall be provided by the contractor and signed by

Contractor and WL, or above. The Technical Report shall be provided to the WL or above, and COR within fifteen (15) days. Then, once the contractor is contacted by government, the contractor shall have a maximum of thirty (30) calendar days to begin on-site repairs. (CDRL A002, On-Site Maintenance and/or Repair)

2.1.5.4 If for any reason the contractor is not able to meet this deadline, then the government may request that the contractor provide any parts, tooling, or instructions necessary to perform the needed repairs.

2.1.6 Scheduled Calibrations

2.1.6.1 The Contractor shall be responsible for calibrating, and verifying the

Aeroflex 5820 Units, and spares to ensure functionality per the fifteen (15) month interval schedule referenced in table 2.1.4.1. The Contractor shall travel to Ogden

Air Logistics Complex (OO-ALC), and perform full calibration for each of the two (2) test units as required by Air Force Metrology and Calibration Program

Office (AFMETCAL).

NOTE: Six (6) Circuit Card Assemblies (CCA’s) requiring calibration are housed inside the Aeroflex 5820. Should any of the six calibrated CCAs within the Aeroflex Tester fail and require replacement, the replacement CCA must be calibrated. An individual CCA failure and replacement does not negate the

Aeroflex 5820 calibration cycle.

2.1.6.2 After successful calibration, the Contractor shall verify the Aeroflex

Testers’ self-test and their associated ITA self-tests are successful.

2.1.6.2.1 Upon completion, the Contractor will sign the self-test print out verifying the Aeroflex Testers and their associated ITAs pass and are fully functional and provide it to the WL or above within seven (7) calendar days. (CDRL A003, Scheduled Calibrations).

2.1.6.3 The Contractor shall provide a certification calibration letter to the government identifying test results of the calibration, IAW Precision

Measurement Equipment Laboratory (PMEL). Appendix D outlines the

Calibration and Certification and Report Format (CDRL A003, Scheduled

Calibrations).

2.1.7 Software Support

(100 hours per year)

2.1.7.1 The Contractor shall ensure: TM/WI F-22-D-00-055 Tables 3 and 4;

TM-WI F-22-D-OO-033 Table 2-1; TM/WI F-22-D-OO-122 Table 1-1; TO

5N29-22-3 Table 8-1; and TO 5N29-22-8-3 Table 1 are updated with all software revisions for the products listed.

2.1.7.1.1 The Contractor shall be responsible for ensuring the Air Force’s

Automated Computer Program Identification Number System (ACPINS) correctly lists the latest revision numbers for all applicable Aeroflex test and ADICTS test software. (CDRL A004, Software Support ACPINS).

NOTE: TM/WI updates are ultimately maintained by Lockheed-Martin.

TO updates are ultimately maintained by Boeing. Changes and revisions are ultimately tracked and maintained by ACPINS. Since ACPINS is the governing publishing agency for the latest revision numbers of applicable software for the Aeroflex Tester and ADICTS Tester, the Contractor shall deliver the most current technical information to ACPINS within seven (7) calendar days of when the actual release has occurred. With this in mind, it may take longer than seven (7) calendar days for updates to appear in the respective TM/WI and/or TO.

2.1.7.2 The Contractor shall be responsible for maintaining all software packages for the Aeroflex Tester, ADICTS Tester and their associated ITAs. This includes updating the existing software as needed to maintain the Aeroflex Tester, and the

ADICTS Tester at a fully functional state.

2.1.7.3 The Contractor shall provide to the WL or above within thirty

(30) calendar days of actual release a new CD-ROM containing updated software for the Aeroflex Tester, and ADICTS Tester. A notification of the release shall be provided to the COR. (CDRL A005, Aeroflex/ADICTS CD-ROM).

NOTE: CD-ROMs are ultimately maintained, categorized, and distributed by

Warner-Robins ALC (WR-ALC). It will be the Contractor’s responsibility to ensure the most current information is delivered to the appropriate Point Of

Contact (POC) at WR-ALC within seven (7) calendar days. It may take longer than thirty (30) calenday days for the updated CD-ROM to arrive and be uploaded to the Aeroflex Tester.

2.1.7.4 New software, if applicable, shall be installed and TO updates shall be provided to the government following each visit within seven (7) calendar days.

The date of the TO Change will also be annotated in the technical report. (CDRL

A001, Pre-Inspection/Self-Test and CDRL A004, Software Support

ACPINS).

2.1.8 Travel Requirements

2.1.8.1 The Contractor may be required to travel to support the objectives of this

PWS. The allowable baseline for airfare cost is the “lowest priced airfare available to the Contractor as outlined in Federal Acquisition Regulation (FAR)

31.205-46(b) & (c) and in accordance with the Joint Travel Regulations (JTR).

All travel requires approval/pre-authorization from the Contracting Officer (CO).

The Contractor is responsible for making all necessary travel arrangements.

2.1.8.2 The Contractor shall provide itinerary and receipts for all reimbursable costs to the COR.

OO-ALC and perform full calibration for each of the two (2) Aeroflex 5820s units and spares to ensure functionality.

NOTE: The Tables listed in the TM/WI are ultimately updated and maintained by Lockheed-Martin. The Tables listed in the TOs are ultimately updated and maintained by

Boeing. The Contractor will ensure the most current information is delivered to the appropriate OEM within seven (7) calendar days. With this in mind, it may take longer than seven (7) calendar days for updates to appear in the respective TM/WI and/or TO.

Software Support CD-ROM:

The Contractor shall provide a new CD-

ROM containing updated software for the entire Aeroflex Tester suite.

(7) calendar days. It may take longer than thirty (30) calendar days for the updated CD-ROM to arrive and be uploaded to the Aeroflex Tester, and ADICTS Tester.

New software, if applicable, shall be installed and TO updates shall be provided to the government following each visit.

Travel Requirements:

The Contractor shall travel to Hill AFB within thirty (30) calendar days to perform on-site maintenance, repairs and/ or calibration to the Aeroflex Testers, Aeroflex 5820’s, ADICTS Testers and/or their associated ITA’s as required IAW the OEM standard.

(Satisfactory) When the Contractor has provided the required technical report to include solutions, course of repair, results and any recommendations of all diagnosises, repairs and solutions for the failures on the Testers and associated ITA’s. Repair work is done by OEM standards.

The contractor will have 30 days to complete repairs once all parts are received.

4. CONTRACT DATA REQUIREMENTS LIST (CDRL)

4.1 CDRL requirements. Contract information data shall be delivered electronically in accordance to the corresponding requirement for each Data Item listed in Table 4-1 (below). The contractor shall have no more than one error per quarter for all CDRL submittals. The contractor shall submit CDRLs according to the due date indicated and have no more than one CDRL per quarter submitted after the identified due date. Grammatical errors do not count as an error.

Substantive mistakes shall be corrected within two working days and corrective action taken to prevent recurrence of inaccurate late CDRLs.

5. GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES

5.1 The Government will provide necessary working space, including heat, light, ventilation, and electrical powered outlets for the use of Contractors’ maintenance personnel. The

Government shall provide access to the necessary facilities where the equipment resides. The place of performance shall be Hill Air Force Base (HAFB), UT.

5.2 Government telephones shall be used for official business only.

5.3 The Contractor shall be responsible for safeguarding all Government property provided for

Contractor use.

5.4 If the parts or materials are unable to be fixed at HAFB, UT, and will be shipped to contractor facility, then government property shall be handled in accordance with applicable contract clauses. Item identification will be provided by the Government and shall not be removed while in contractor possession.

6. GENERAL INFORMATION

6.1 SPECIAL QUALIFICATIONS Not applicable for this requirement.

6.2 SECURITY REQUIREMENTS

6.2.1 The contractor shall comply with all security, force protection and Anti-terrorism requirements enforced at HAFB.

6.2.2 The contract administrator shall complete a Request for Identification Credential

Air Force Materiel Command (AFMC) Form 496 for each employee of the contractor requiring access to HAFB. The requests shall be submitted to Pass and Registration

(South or West Gate Visitors Center).

6.2.3 The COR/WL, or above will be responsible and ensure that the contractor is given base access at the South Gate visitors center either by escorting the individual(s), by phone, or on-line access request. The access process will be based on the security requirements at that time.

6.2.4 The Contractor may only use a non-Government-approved laptop in unclassified areas. At NO time shall such device be connected to a Government LAN.

6.2.5 Photography is prohibited in both the 523/524 EMXS repair locations without written permission. Written approval must be sought after and provided if photograph(s) are necessary and warranted.

6.2.6 Cellular phones are allowed in the 523rd EMXS Digital Shop Replaceable Unit

(SRU) Shop. Cellular phones are prohibited in the 523rd EMXS Displays/Indicators Shop

(D/I) due to security classification. (For PWS clarification purposes, C-17 HUD, F-22

HUD, and F-22 HUD Cameras are tested in D/I.) Cellular phones will be stored in provided lockable receptacles outside D/I’s main entrance. Cell phone use is allowed in

524 EMXS in designated areas.

NOTE: Aeroflex Tester, and ADICTS Tester test data is periodically downloaded to external media which are vulnerable to malicious code and other external threats.

External media to update/upgrade software must be virus-scanned prior to being uploaded to any Aeroflex Tester and/or ADICTS Tester. External media used to receive downloaded data must be virus-scanned prior to installation.

6.3 SAFETY AND HEALTH STANDARDS

6.3.1 The contractor shall follow Appendix C and adhere to the Air Force is part of the

Occupational Safety and Health Administration (OSHA).

6.3.2 Voluntary Protection Program (VPP). The contractor is required to attain familiarization with the requirements of VPP. Detailed information on VPP is available on the OSHA website at (http://www.osha.gov/dcsp/vpp/index.html). VPP requirements apply to contractors whose employees work more than 1,000 hours per quarter. The contractor shall flow down these requirements to all sub-contractors who qualify as applicable contractors under this contract.

6.3.3 The contractor shall provide from its OSHA 300 information the Total Case

Incident Rate/Days Away Restricted or Transferred (TCIR/DART) rate by 15 January of each year to the CO for submission as part of the installation's annual OSHA VPP self-evaluation report.

6.3.4 System Safety Program Plan. A contractor provided safety program will be implemented that ensures the protection of Government personnel and property.

6.4 HOURS OF OPERATION

6.4.1 The Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) C. Normal shop working hours at HAFB are typically 5:00AM – 3:30 PM local time Monday through Friday.

6.4.2 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities on the following federal holidays: New Year’s Day, Martin

Luther King Jr. day, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If the

Holiday falls on a Saturday, it will be observed on Friday. If the Holiday falls on a

Sunday, it will be observed on Monday.

6.4.3 All visits shall be scheduled by the the WL, or above. Schedules may have to be modified in the case of incidents or emergencies, to include natural hazards, furloughs or similar circumstances.

6.5 GOVERNMENT OBSERVATIONS

6.5.1 The Government will periodically evaluate the contractor’s performance through inspections IAW with FAR 52.246-4 Inspection of Services.

6.5.2 All Contractors shall be escorted by the WL, or above at all times.

6.6 ENVIRONMENTAL

6.6.1 The Contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The industrial complex at Hill Air Force Base has the potential to expose workers to hazardous materials, which may include, but are not limited to, hexavalent chrome, cadmium, beryllium, lead, etc. While the Contractor will not perform duties in a marked and signed regulated area (where exposures are expected to exceed the permissible exposure limits set by OSHA, there may be contact hazards with these materials during cleanup of dust, metal shavings, etc. The Contractor shall ensure proper protective measures and training is taken to ensure contracted employees are protected from these hazards.

6.6.2 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the Contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the Contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

6.6.3 Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 Code of Federal Regualtion (CFR) 302, including hazardous materials, into the environment, the Contractor or its agent shall immediately call 911. If the call is made from an on base phone it will go directly to HAFB dispatch. If using a cell phone it will go off base, ask the 911 dispatcher to be transferred to HAFB. The liability for the spill or release of such substances rests solely with the Contractor and its agent.

6.6.4 The contractor shall maintain current Safety Data Sheet (SDS) documents for all chemicals used in maintaining and operating the system, and shall provide copies upon request IAW Appendix C of this PWS.

6.7 MANPOWER REPORTING

6.7.1 The contractor shall report ALL Contractor labor hours (including sub-contractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil/.

6.7.2 Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.

While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the

Contractor Manpower Reporting Application (CMRA) help desk.

6.7.3 Uses and Safeguarding of Information. Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data. User Manuals: Data for Air Force service requirements must be input at the

Air Force CMRA link at http://www.ecmra.mil/.

6.8 QUALITY CONTROL STANDARDS

The Contractor shall establish a complete Quality Control Plan (QCP) per contractor format to ensure the requirements of this contract are met as specified. The Contractor's quality management program shall comply with the minimum quality management systems elements specified in the American National Standards Institute (ANSI), American Standards

Organization (ASO), and International Standard Organization (ISO) family of standards.

6.8.1 Quality Assurance

6.8.1.1 The COR is the authorized Government Representative(s) (GR) who will perform assessments of the Contractor’s performance. The COR or alternate

COR(s) will perform assessments of the contractor’s performance. Subsequent to contract award, the identity of the COR(s), with a letter defining duties and authority will be furnished to the contractor.

6.8.1.2 The COR(s) or alternate(s) will inform the task manager in person when discrepancies occur and will request corrective action. The COR(s) or alternate(s) will make a notation of the discrepancy on the assessment checklist with the date and time the discrepancy was noted and will request the task manager (or authorized representative) to initial the entry on the checklist.

6.8.1.3 The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.

6.8.1.4 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO.

6.8.1.5 The services to be performed by the Contractor during the period of this contract are at all times and places subject to review by the CO or authorized representative(s).

6.8.1.6 The Government will periodically evaluate the Contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The GR will evaluate the Contractor’s performance through inspections and receipt of complaints from base personnel.

6.9 FOREIGN OBJECT DAMAGE (FOD) AND TOOL CONTROL

6.9.1 The Contractor shall follow the applicable guidance in chapters/paragraphs/ sections of Air Force Instruction (AFI) 21-102, AFMC supplements to AFI 21-102, and any OOALC supplements to the AFI 21-101 concerning base tool and FOD control policies as they become available. The most current publications available shall be used in all cases. Cloth rags are considered tools and will follow the same regulations and procedures as tools.

6.9.2 If tools, and/or equipment are lost, the Contractor will report the loss to the wage leader or above and COR no later than two (2) hours after discovery. If tools are lost or not accounted for, then the Contractor shall implement the lost tool procedures of

AFSCMAN 21-102; and AFI 21-102 AFMCSUP AFMCGM2015-02 Chapter 10.

6.9.3 Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All Contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements.

The contractor will implement a safety program that ensures protection of Government personnel and property. The program will consist of, as a minimum:

1. Mishap reporting, as defined in para B1 below.

2. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

3. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for the Administrative Contracting Officer (ACO) approval of operations and changes at these specific sites.

4. All contractor personnel shall be trained and qualified to perform their duties safely.

5. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. The contractor shall notify the OO-ALC/SE Workflow (OO_ALC.SE.Workflow@us.af.mil) or the HAFB Command Post (801-777-3007) after normal duty hours, and the designated GR, i.e., the ACO, Procuring Contracting Officer (PCO), or Defense Contract Management Agency

(DCMA) COR within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to Department of Defense (DoD) property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than twenty (20) calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. The contractor shall cooperate with any and all government mishap investigations.

Additionally if requested by government personnel or designated GR, i.e., the ACO, PCO, or

DCMA COR, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

3. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the government investigation.

4. The contractor shall support and comply with the safety investigation and reporting requirements of DAFI 91-204, Chapters 1 – 5.

C. General Safety Requirements:

The contractor is solely responsible for compliance with all federal, state and local laws, the OSHA (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, and the protection of their employees. Additionally, the contractor is responsible for the safety and health of all subcontractor employees. All documents required as a result of OSHA 29 CFR 1910 and 1926 regulations, (i.e. certifications, training, respiratory protection program, workplace hazard assessments), shall be made available to the COR upon request.

The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.

Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the OO-ALC

A Risk Assessment will be required prior to any work being accomplished, if munitions will remain in the facility.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to FAR 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

SECTION II – SPECIFIC REQUIREMENTS

PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; Department of

Defense Instruction (DoDI) 6055.4, DoD Traffic Safety Program, 20 Jul 99; and AFI 91-207, USAF Traffic Safety Program, 22 May 07. Each applies to all persons at any time on an Air

Force Installation and includes all leased, owned, or privatized property including housing areas.

In addition: AFI 13-213, Airfield Management, applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Hill Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA

Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

5.7.3 Calibration Certificate and Report Format.

The format of the certificate or report shall be specifically designed for each type of calibration, but the headings shall be standardized and include all information necessary for the interpretation of the calibration results. Each calibration certificate or report shall be accurate, clear, unambiguous and objective, in accordance with any instructions in the calibration authority.

Where the certificate or report contains results of calibrations performed by other than the

PMEL, these results shall be clearly identified. When issued, the certificate or report shall contain, as a minimum (NOTES shown below are to assist the PMEL’s interpretation and verification of the requirement):

a. Title, e.g. “Calibration Report” or “Calibration Certificate”.

b. Name and address of laboratory, and the location where the calibrations were carried out, if different from the address of the laboratory.

c. Unique identification of the calibration report or certificate (e.g., the serial number), and on each page an identification in order to ensure that the page is recognized as a part of the calibration report or certificate, and a clear identification of the end of the calibration report or certificate.

d. Name and address of customer.

NOTE

A requirement for the name and address of customer may be waived with customer coordination/concurrence.

e. Identification of the method used.

The calibration report or certificate may identify the method used in a manner similar to the following examples: a calibration procedure number; an American Society for Testing and

Materials (ASTM), International Electrotechnical Commission (IEC), or other standard specification; or a narrative description of the method used (i.e. compared with laboratory standard X at 10 points between 10% and 100% of full scale).

f. A description of, the condition of, and unambiguous identification of the item(s) calibrated.

g. Date of receipt of the calibration item(s) where this is critical to the validity and application of the results, and the date(s) of performance of the calibration.

h. Reference to the sampling plan and procedures used by the laboratory or other bodies where these are relevant to the validity or application of the results.

The calibration report or certificate might not reference a sampling plan as sampling plans rarely apply to calibration; sampling plans normally are used in testing laboratories.

i. The calibration results with, where appropriate, the units of measurement.

j. The name(s), function(s) and signature(s) or equivalent identification of person(s) authorizing the calibration report or certificate.

k. Where relevant, a statement to the effect that the results relate only to the items calibrated.

l. The conditions (e.g., environmental) under which the calibrations were made that have an influence on the measurement results.

m. The uncertainty of measurement or a statement of compliance with an identified metrological specification or clauses thereof.

A calibration report or certificate that contains a statement of compliance to original manufacturer's specifications (with any exceptions or limitations noted) may satisfy this requirement.

n. Evidence that the measurements are traceable.
