About this file

This document is a Notice of Funding Opportunity (NOFO) for the Alzheimer's Disease Programs Initiative (ADPI) - Grants to States and Communities (Winter 2024). The NOFO is issued by the Department of Health and Human Services Administration for Community Living and provides cooperative agreement funding through the ADPI program (CFDA 93.470).

The NOFO contains two application options: Grants to States (Option A) and Grants to Communities (Option B). Option A 36-month cooperative agreements are available to state governmental entities designated for dementia-capability to create, expand and sustain dementia-capable home and community-based service (HCBS) systems. Option B 36-month cooperative agreements are available to community-based organizations (CBOs) to address specific service gaps in existing dementia-capable HCBS systems. No entity is eligible to apply for both options or hold more than one ADPI grant at a time. Priority consideration will be given to states and CBOs that have not received ADPI or predecessor grant funding since before 2014.
