ADDP3 Beta-SAM Synopsis The Defense Health Agency (DAH) awarded a fixed-priced Contract (HT9402-21-C-0006) with Fixed Unit Prices to supplement the Active Duty Dental Program (ADDP) to United Concordia Companies, Inc. (UCCI) of Harrisburg, Pennsylvania on 29 April 2021. The ADDP provides comprehensive dental care services worldwide to eligible ADSMs of the Uniformed Services. The geographic area of coverage for the Continental United States (CONUS) includes the 50 United States, the District of Columbia, U.S. Virgin Islands, Guam, Puerto Rico, American Samoa, and the Northern Mariana Islands. Outside the Continental United States (OCONUS) includes all other countries, island masses and territorial waters. ADDP will supplement ADSM care provided in the Continental United States (CONUS) DoD’s Dental Treatment Facilities (DTFs), as well as provide care to those ADSMs living in regions, CONUS and OCONUS without access to DTFs. The contract base period of performance (transition-in) is 1 May 2021 through 30 April 2022. There are seven - one-year option periods (pursuant to FAR 52.217-9 Option to Extend the Term of the Contract), continuing through 30 April 2029, a one year residual phase-out period which follows the last exercised period of performance and the potential for a six month extension pursuant to FAR 52.217-8 Option to Extend Services. All Option Periods and the potential of a six month extension were evaluated in accordance with FAR 52.217-5, Evaluation of Options. The total evaluated price was inclusive of the base period, seven - one-year option periods, and the potential six month extension.
