Quoters must submit SF 1449 (blocks 17a, 30b, 30c signed at 30a), completed Section B.1(1)(a) titled Contract Administration with contractor address and point of contact, signed amendments or acknowledgement of amendments, and completed Section B.2 Schedule of Supplies & Pricing. Questions must be submitted in writing to Contract Specialist Eunice Bones at eunice.bones@va.gov by July 29, 2026, at 11:00 AM EST. Quotes must be submitted electronically to Contracting Officer Maribel Santa at maribel.santadelapaz@va.gov and Contract Specialist Eunice Bones at eunice.bones@va.gov by the deadline specified in Block 8 of SF 1449. Multiple emails are permitted to avoid system limitations, hyperlinks are prohibited, proprietary information must be clearly marked, and quoters are responsible for the validity of all information supplied, including subcontractor information.

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL

PRODUCTS AND COMMERCIAL SERVICES

A.1 DESCRIPTION

EVALUATION PROCESS – In accordance with FAR 12.201, the Government will award a contract from this solicitation to the responsible quoter on a best value basis whose quote conforms to the solicitation and represents the best overall value to the government.

The award will be based upon a comparative evaluation of quotes. Comparative evaluation is the side-by-side pairwise comparison of quotes based on factors resulting in a Contracting

Officer decision for the quote most favorable to the Government.

A.2 EVALUATION FACTORS FOR AWARD:

The Evaluation factors for this Request for Quote (RFQ) are technical, past performance, and

Price.

1. TECHNICAL FACTOR: In this factor, it is important that the offeror complies with applicable rules and regulations and will provide quality services promptly. In that regard, the offeror must demonstrate compliance with the solicitation requirements. Consequently, it possesses and maintains the necessary permits, adequate and trained staff, and the parts and equipment necessary to perform the services as per the salient characteristics. The quotation will be evaluated to the extent to which it can meet or exceed the Government's requirement as outlined in the solicitation and the evaluation factors. At a minimum, your technical quote shall include the following:

a. The Manufacturer’s Product Specifications or a brochure.

b. The delivery information and installation shall be in line with the requested lead time/delivery date to the best of the contractor’s ability.

c. Warranty information.

2. PAST PERFORMANCE FACTOR: An offeror's past performance is important in determining its ability to successfully perform the services described in the salient characteristics. The

Government is interested in ensuring that the offeror has a successful history of recent and relevant past performance. In order to be considered relevant and recent, all references shall be similar in scope to the solicitation requirement.

a. Contracting will utilize past performance inquiries consistent with CPARS, FAPIIS, Previous

Awards, and Determination of Responsibility documentation.

b. In the case of a quoter without a record of relevant past performance or for whom information on past performance is not available, the quoter will not be evaluated favorably or unfavorably on past performance.

3. PRICE FACTOR: Price will be evaluated to determine price reasonableness. The award will be made as the response most advantageous to the Government. Responses should contain your best terms and conditions.

NOTE: The price quoted shall be limited to a maximum of two (2) decimal places. If the quoter's price includes more than two (2) decimals, the government reserves the right to round the amount up or down to limit it to two decimal places.

A.3 SOLICITATION DOCUMENT DELIVERABLES:

a. SF1449 - fill-in blocks 17a, 30b, 30c and sign 30a.

b. Completed Section B.1(1)(a) titled Contract Administration. (Include Contractor’s address and point of contact information)

c. Signed Amendment(s) (SF 30 – if applicable) OR completed ACKNOWLEDGEMENT OF

AMENDMENTS.

d. Completed Section B.2 SCHEDULE OF SUPPLIES & PRICING.

To maintain the integrity of this solicitation and subsequent award date, any questions must be submitted in written form via e-mail to Contract Specialist Eunice Bones at eunice.bones@va.gov and shall be received no later than July 29, 2026, 11:00 AM EASTERN

STANDARD TIME (EST).

Quotes are due by the date and time set for the receipt of offers shown in Block 8 of SF 1449

(page 1 of the Request for Quote). One electronic copy of all items requested below must be submitted to the Contracting Officer Maribel Santa at maribel.santadelapaz@va.gov contract

Specialist Eunice Bones at eunice.bones@va.gov. Late submissions shall be treated in accordance with the solicitation provision at RFO FAR 52.212-1(c).

A.4. QUOTE PREPARATION INSTRUCTIONS:

1. To ensure timely and equitable evaluation of quotes, prospective contractors (quoters) must follow the instructions contained herein. Quoters are required to meet all solicitation requirements.

2. The quoter response shall consist of technical, past performance, and price submission.

Submit to the Contracting Office by the time and place indicated in the solicitation.

3. Price quote Submission: Quoters shall price all line items listed in the price schedule.

4. Technical Submission: In demonstrating the ability to comply with the solicitation requirements, quoters shall meet and provide all solicitation technical requirements submissions with their quote in accordance with the Salient Characteristics section and Evaluation Factors for Award above.

5. Submissions can be sent in multiple emails to avoid the computer system's email size limitation to CO and CS. Quotes submitted by any other method will not be considered. All proprietary information must be clearly marked. The use of hyperlinks in quotes is prohibited.

6. The Quoters shall be held responsible for the validity of all information supplied in his/her quote, including information provided by potential subcontractors. Should subsequent investigation disclose that the facts and conditions were not as stated, the quote may be rejected and receive no further consideration.
