This document is an addendum issued by the Commonwealth of Kentucky Department for Facilities and Support Services Division of Engineering and Contract Administration for the Interior and Restroom Modernization project at the Ravenna Readiness Center, Department of Military Affairs, in Estill County, Kentucky. The addendum provides clarifications and revisions to the original invitation to bid, including striking references to demolition of gas lines, the owner providing a temporary vault and furniture moving assistance, changes to the casework specification, and details on the installation of steel plates in the vault. An MEP addendum is also included, which addresses contractor questions about temporary bathroom facilities and gas line removal. The addendum is part of the contract documents, and bidders must acknowledge receipt in their proposal.
The addendum also includes a specification for the epoxy flooring system, which is a 3/16-inch textured epoxy surfacing with a decorative quartz broadcast finish. The specification details the required physical properties, mixing, and application of the flooring system. Bidders must provide samples and certified test results demonstrating compliance with the specified requirements.

a. Remove all existing projections, debris, and existing coatings that may be detrimental to adhesion of new flooring system. Remove dirt, oil con-taminates, grease, and surface coatings affecting bond.
2. Notify Architect in writing prior to commencing work of any conditions deemed unsatisfactory for the installation; installation of flooring materials is understood as acceptance of the substrate as satisfactory.
3. Concrete: The General Contractor shall be responsible for hiring an independent testing service to test for moisture content and moisture vapor emission rate; in-stall no flooring over concrete until the concrete has been cured and is sufficient-ly dry to achieve permanent bond with flooring as determined by material manu-facturer's recommended bond and moisture tests.
a. Contractor to request manufacturer’s product representative review the existing conditions. If needed, Effectively remove concrete laitance by steel shot blasting or other method approved by flooring manufacturer.
Surface profile must be a minimum CSP-3 profile according to Interna-tional Concrete Repair Institute Guideline #03732. Flooring contractor shall document surface profile using ASTM D-7682 test method.
b. Test existing concrete slab to determine presence of existing vapor barri-

A. Install all floor materials in strict conformance with manufacturer's instructions.
B. Route out all cracks larger than 1/16” width and fill with rigid epoxy material approved by Manufacturer of floor materials. Apply flexible membrane across the crack and 12 inches on either side at a spread rate of 40-50 square feet per gallon to achieve 32-40 mils dry. Optional membrane reinforcement: Fiberglass scrim cloth is applied to top of cured membrane. Sawcuts: Use this same procedure if covering sawcuts with finished flooring system.
C. Alternate to step B for extensive cracking or overlaying sawcuts: Apply flexible mem-brane over entire floor surface as a crack reduction membrane at a spread rate of 40-50 square feet per gallon to achieve 32-40 mils dry. Optional membrane reinforcement: Fi-berglass scrim cloth reinforcement is to be applied over entire membrane or individual cracks as outlined in step B. Precaution: If area to receive flooring may be subjected to excessive thermal induced movement from sunlight exposure, consult with manufacturer before proceeding with full membrane coverage.
D. Prime entire surface with recommended primer. If using moisture vapor control system, fill cracks and sawcuts prior to installation of crack isolation membrane.
E. Apply epoxy and broadcast decorative aggregate in two applications to achieve a mini-mum thickness of 3/16 inch.
F. Apply chemical resistant epoxy grout coat and seal coat(s) to provide a uniform, dense surface.
G. Match finished work to approved samples, uniform in thickness, sheen, color, pattern and texture, and free from defects detrimental to appearance.
H. Apply temporary protection until floor is fully cured. The General Contractor shall pro-tect the finished floor from the time that the sub-contractor completes the work.
I. Integral Cove Base: Where scheduled, provide integral cove base formed from flooring over tile backerboard as specified under 09250 - Gypsum Drywall. Optional: provide cove trim strip at top of base as recommended by flooring manufacturer and trowel mate-
