About this file

This addendum to Invitation for Bid No. 2026-042 issued by Union County, North Carolina, Department of Public Works clarifies and modifies the contract documents for Government Center Exterior Envelope Repair work located at 500 North Main Street, Monroe, NC 28112. The project scope includes replacement of floor drains and concrete topping slabs, masonry tuckpointing repairs, concrete spall repairs, guardrail post pocket repairs, joint sealant replacements, and window glass replacement at select locations. The work also includes five unit price items for floor drain installation (6 each), tuckpoint mortar joints (2,300 linear feet), partial-depth spall repairs (12 cubic feet), guardrail post repairs (21 each), and joint sealant replacement (1,300 linear feet), as well as three alternate items for brick replacement (5 each), insulated glass unit replacement (18 each), and curtain wall cleaning (1 each). Sealed bids are due October 19, 2026, at 10:00 AM at Union County Procurement and Contract Management, 610 Patton Avenue, Monroe, NC 28110. The addendum resolves a discrepancy by confirming the correct project duration is 240 calendar days, not 90 days as previously stated in Section 1.8 of the original IFB document.

The addendum specifies that bidders must submit bid security equal to five percent of the total bid price and acknowledge receipt of all addenda on the bid form. Bidders must include a base bid price plus a 10 percent owner contingency allowance, with unused contingency funds to be returned via deduct change order. Liquidated damages are assessed at $500.00 per calendar day for delays beyond the 240-day completion period. The contract requires that bidders be properly licensed in North Carolina, obtain all necessary permits, provide performance and payment bonds, and maintain insurance acceptable to the owner. The project will remain open to public access during construction, requiring the contractor to implement approved safety and protection measures including covered walkways, railings, and barricades at all egress and primary access ways. Tie-back safety anchors on the building's roof have been load-tested and certified for use by exterior maintenance vendors, with the next annual inspection required by December 2026 and load testing required again by February 2030.

Annual Tie-back Safety Anchor Inspection

500 North Main Street

Monroe, North Carolina 28112

RSE Project No. 25-264 Report Date: December 16, 2025

December 16, 2025

Mr. Frank Stegall Facilities and Fleet Supervisor Zone 1 Union County Government 500 North Main Street | Suite 100 Monroe, North Carolina 28112 Telephone: (704) 305-4522

Subject: Annual Tie-back Safety Anchor Inspection Report

500 North Main Street Monroe North Carolina RSE Project No. 25-264

Dear Mr. Stegall, ReStruct Engineering (RSE) has performed a visual inspection of the tie-back safety anchors on the building located at 500 North Main Street in Monroe, North Carolina. The purpose of this inspection was to provide substantial compliance with current Occupational Safety and Health Administration (OSHA) regulations which require that the tie-back safety anchors be visually inspected by a competent person at an interval not to exceed 12 months. These services were authorized by you on December 15, 2025.

Project Background It is our understanding that Union County manages the subject building. The building is subject to OSHA Regulations - CFR (Code of Federal Regulations) 29 Part 1910 “Occupational Safety and Health Standards” and CFR 29 Part 1926 “Safety and Health Regulations for Construction”. According to OSHA 1910.27(b)(i), tie-back safety anchors used for rope descent must be inspected annually by a qualified person.

According to our records, the safety anchors were previously load tested and certified in February 2020 by PM&A. Based on the load testing report and our observations, the existing rooftop safety equipment system consists of 24 points identified for use as tie-back safety anchors, labeled C1 through C24. Anchors labeled C1 through C24 consist of existing structural steel columns.

Observations RSE’s field representative visited the subject building on December 15, 2025, for the annual visual inspection of the rooftop safety equipment. Observations were performed from the main roof level of the subject building where certified tie-back safety anchors are present. Each of the tie-back safety anchors were visually inspected and photographed. The photographs will be retained for comparison in future annual inspections. The following observations were noted during our site visit:

• The tie-back safety anchors appear to be in good condition.

Conclusions Based on our observations and provided documents, the observed rooftop safety equipment appears to be in substantial compliance with applicable OSHA regulations. The observed rooftop safety equipment components appear to be in good overall condition and may be used as rooftop safety equipment tie-backs in concert with suspended staging or as anchorage points for personal fall arrest equipment.

RSE recommends that the rooftop safety equipment be inspected on an annual basis as required by OSHA regulations. The next annual inspection should be scheduled and performed by December 2026. The subject tie-back safety anchors should be load tested and certified again by February 2030.

Equipment Usage Recommendations RSE recommends the following based upon our observations and test results for the tie-back safety anchors:

• The existing safety equipment layout incorporates anchorage points located within 15 feet of an unprotected edge. Exterior maintenance vendors should be secured to another anchorage point when approaching equipment within 15 feet of an unprotected edge per OSHA regulations.

• We recommend that the vendors who perform façade access or work at unprotected edges secure their safety lifelines and equipment to the anchorage points that have been certified as required by OSHA regulations. Each employee must secure their safety line to a separate anchorage point.

• Rope descent systems employing counterweights, parapet wall clamps, cornice hooks, and other similar non-permanent anchorages should be secured to certified tie-back safety anchors as required by OSHA 1910.27(b)(2)(v).

• We recommend that any primary line, lifeline and tie-back line, shall be attached with minimal slack to an identified anchorage point in a straight line with no more than 15 degrees angulation, as recommended by International Window Cleaning Association (IWCA).

• We recommend that all primary and secondary lines be tied to separate tie-back safety anchors.

Never tie a primary and secondary line to the same tie-back safety anchor.

• This report should be submitted to the building exterior maintenance vendors for review prior to conducting activities that may require the use of rooftop safety equipment. Vendors should sign an acknowledgement letter stating that they have read, understand, and will comply with the recommendations provided herein.

• Other rooftop components including, but not limited to mechanical equipment, vent pipes, and exposed structural framing should not be used as an anchorage point until it has been load tested as required by OSHA regulations.

• Building exterior maintenance vendors should not construct horizontal lifeline systems between anchorage points unless designed by a registered professional engineer.

• The building exterior maintenance vendor utilizing temporary outrigger and davit system components is responsible for properly assembling and maintaining these components while using them.

• It is the responsibility of the building exterior maintenance vendors using the rooftop safety equipment to ensure that all lifelines are properly secured to the anchorage points.

• The compliant rooftop safety equipment should be visually inspected on an annual basis per OSHA regulations. Additionally, the existing rooftop safety equipment components must be load tested every ten years or after an accident as required by OSHA regulations and as recommended by the

IWCA.

• Our observations were limited to the roof areas containing compliant rooftop safety equipment and it was not the intent of this survey to locate every existing defect with the building or the safety equipment components. Defects may exist that were not observable at the time of our site visit or additional degradation may occur with time that was not evident, or had not yet occurred, at the time of our site visit.

Closure We appreciate the opportunity to provide this engineering service. We cannot account for further distress or deterioration that occurs following our site visit. If you have any questions or require additional information, please contact us.

Respectfully Submitted, Brian M. Rivers, PE Principal

500 NORTH MAIN STREET - SAFETY EQUIPMENT LAYOUT

SCALE: 1" = 20'-0"

GENERAL NOTES

1. TIE-BACK SAFETY ANCHORS LABELED C1 THROUGH C24 WERE LOAD TESTED IN FEBRUARY 2020. THE TIE-BACK SAFETY ANCHORS LABELED C1 THROUGH C24

SHOULD BE LOAD TESTED AGAIN IN FEBRUARY 2030, AFTER AN ACCIDENT, OR FOLLOWING ROOF REPLACEMENT.

2. THE ROOFTOP SAFETY EQUIPMENT LOAD TESTED IN FEBRUARY 2020 SHOULD BE VISUALLY INSPECTED ANNUALLY AS REQUIRED BY OSHA REGULATIONS.

3. THE EXISTING SAFETY EQUIPMENT LAYOUT INCORPORATES ANCHORS POINTS LOCATED WITHIN 15 FEET OF AN UNPROTECTED EDGE. EXTERIOR MAINTENANCE VENDORS SHOULD BE SECURED TO ANOTHER ANCHORAGE POINT OR HORIZONTAL LIFELINE WHEN APPROACHING EQUIPMENT WITHIN 15 FEET OF AN

UNPROTECTED EDGE PER OSHA REGULATIONS.

4. PM&A RECOMMENDS THAT ANY PRIMARY LINE, LIFELINE, AND TIE-BACK LINE, SHALL BE ATTACHED WITH MINIMAL SLACK TO AN IDENTIFIED ANCHORAGE POINT IN

A STRAIGHT LINE WITH NO MORE THAN 15 DEGREES ANGULATION, AS RECOMMENDED BY IWCA.

5. PM&A RECOMMENDS THAT ALL PRIMARY AND SECONDARY LINES BE TIED TO SEPARATE TIE-BACK SAFETY ANCHORS. NEVER TIE A PRIMARY AND SECONDARY

LINE TO THE SAME TIE-BACK SAFETY ANCHOR.
