About this file

This is Addendum No. 2 for Invitation for Bid (IFB) 04A7062 issued by the California Department of Transportation (Caltrans) for Construction Materials Lab Testing and Reporting services in various Bay Area counties. The addendum revises the qualification requirements for Laboratory Supervisors, allowing either NICET Level III certification or higher in Construction Materials Testing field OR a current and valid California Professional Engineering license. Laboratory Supervisors must have a minimum of three years of demonstrated experience acceptable to Caltrans in performing material tests that they supervise.

The addendum includes an updated Attachment 4 (Certification Statement and Required Certificates and Reports) that bidders must use when submitting their bids, with explicit warning that failure to use the updated form will result in bid rejection. The document requires bidders to submit copies of appropriate certifications or licenses, laboratory addresses for both prime bidder and subcontractors, and a signed certification statement confirming employment of qualified personnel for the duration of the agreement.
