About this file

This is Addendum No. 1 to Invitation for Bid #07A4024 issued by the California Department of Transportation (Caltrans) on April 4, 2016, for Medical Examinations and Laboratory Testing services. The addendum clarifies three key aspects of the contract requirements: (1) Certified Physician's Assistants may conduct examinations but all must be signed off by a Certified Physician, (2) contractors must maintain medical files for the duration of employee's employment at Caltrans plus 30 years, and (3) Forced Vital Capacity (FVC) testing is to be included as part of the Respiratory Compliance Examination rate.

The addendum modifies the scope of work to clarify pricing structure, specifically noting that physical exams with FVC testing should be priced as part of the Respiratory Compliance Examination rate on Attachment 1. Additionally, it addresses record-keeping requirements that extend well beyond the standard 7-year medical record retention period typically used by medical facilities. No specific pricing terms, contract duration, or award dates are mentioned in this addendum.

ADDENDUM No.1

Invitation for Bid #07A4024

Medical Examinations and Laboratory Testing

NOTICE TO ALL PROSPECTIVE BIDDERS:

Addendum No. 1 is being issued for the above-referenced IFB. Bids for this work shall be with the full understanding of this Addendum.

THE FOLLOWING REPRESENTS QUESTIONS RECEIVED AND THE

DEPARTMENTS ANSWERS:

Question 1

Bidder’s Minimum Requirements states, "must be a Certified Physician's License". Our facility employs Medical Doctors, as well as Physicians Assistants, both of which are licensed with the State of California as well as DMV Certified. Does this contract allow your employees to be examined by Physicians Assistants?

Answer

Scope of Work has been amended to add the following clause:

P. A Certified Physician’s Assistant may complete examinations, but all examinations must be signed off by the Certified Physician.

Question 2

Exhibit A Letter J: It states Contractor shall keep copies of the medical files for the duration of employee's employment +30 years. Normally, this is the responsibility of the

Employer. Medical records at a facility are usually kept for 7 years, unless the patient is under the age of 18. This would incur storage fees at our cost, am I reading this correctly?

Per Exhibit A, Item J - Contractor shall keep copies the medical files for the duration of employee’s employment at Caltrans plus 30 years.

Question 3

According to Exhibit A letter D is equal to the Pre-employment Examination. On the

Attachment 1 item #1 and item #4 would be the same pricing. I do not see an item number requesting pricing for a Physical Exam to include the FVC. Am I reading this correctly?

Per Exhibit A, Item B – Respiratory Compliance Examination, Item 2 – Physical Exam, Forced Vital Capacity (FVC) shall be included as part of Respiratory Compliance

Examination rate on Attachment 1.
