About this file
This document is Addendum #2 issued by the Rockland County Department of General Services, Purchasing Division for Request for Bid (RFB) #RFB-RC-2024-019 for an Anti-Choking Device (ACD) that is FDA Registered. The addendum provides critical requirements for bidders, specifically mandating that all bidders must be an Authorized Distributor for LifeVac or an alternate product they are proposing for approval. Bidders are required to submit samples of alternate products prior to the bid opening, as specified in the bid documents, and samples of the LifeVac anti-choking device may also be requested after the bid opening.
The addendum emphasizes that samples must be delivered within 3 business days of request at no cost to the county, and a return label must be included if return is required. The document, signed by Paul J. Brennan, Director of Purchasing, is dated 10/16/24 and stipulates that failure to acknowledge receipt of the addendum in the bid proposal's signature page may result in bidder disqualification. The addendum supersedes any contradictory information in the original contract documents and forms an integral part of the contract documentation for this procurement opportunity.
