The addendum clarifies the annual estimated usage quantities: small gloves 600 boxes, medium gloves 3,000 boxes, large gloves 6,000 boxes, X-large gloves 5,000 boxes, and XX-large gloves 1,000 boxes. Latex gloves are ordered on a semi-annual basis, with delivery required within three business days of order placement to one of two warehouse locations: 1841 East 33rd Street or 2400 E. Spring Street, Long Beach, California 90807/90806. Deliveries must occur Monday through Friday between 8:00 a.m. and 2:30 p.m., f.o.b. Long Beach Utilities Department Warehouse. The previous pricing for all five glove sizes (7 mil, 9-inch cuff, box of 100) was $8.68 per box. Bidders are required to present any approved equal products at the time of bid submission. The RFQ includes consideration for small business, minority business, and other business enterprise outreach preferences as outlined in section XXXII of the original solicitation. The addendum addresses eight bidder questions regarding estimated usage, ordering frequency, freight cost calculations, and submission procedures, providing clarification necessary for accurate bid preparation.
