About this file
This addendum is issued by the City of Portland, Oregon's Procurement Services department for Bid Number 2381118888 concerning the NE 122nd Avenue Safety & Access project from Beech Street to Wasco Street. The addendum, dated October 1, 2026, modifies the original bid documents and specifications for this stormwater and pedestrian safety project. Key changes include moving the bid opening date from October 1 to October 6, 2026, with bids due by 2:00 P.M. The addendum clarifies several technical specifications and resolves bidder questions regarding conflicting plan notations, particularly regarding PVC pipe installation at the downstream forebay, which was corrected to require 6-inch Schedule 40 PVC pipe rather than conflicting 4-inch and 8-inch specifications. Additional clarifications address the treatment of check dams, planter walls, and doweled forebays as non-separately-billable items included within catch basin costs, the inclusion of valley gutter costs within driveway construction, and the elimination of federal funding forms requirements contrary to the original bidders' checklist.
The revised Notice to Bidders clarifies submission requirements for all prospective contractors, including the mandatory current active Oregon Construction Contractors Board license, prequalification through the City of Portland's diversity compliance portal at least ten days prior to bid opening, and submission of the State of Oregon First-Tier Subcontractor Disclosure Form via email to the buyer by 4:00 P.M. on the day of bid opening for bids exceeding $100,000. Bidders must acknowledge receipt of all addenda before submission and comply with equal employment opportunity certifications, equal benefits compliance, and clean air construction fleet registration requirements. The project contact for questions is Emmanuel Amunga at emmanuel.amunga@portlandoregon.gov.

1.02 Notice to Bidders Uncheck the box for Federal Funded Project forms. See revised
Notice to Bidders.
1.03 Bid Plans Replace sheet D-4 with the attached updated sheet.
1.04 Q&A Question
On sheet D-4, construction 19 states to install 4” schedule 40 PVC in the downstream forebay but on the plan and profile on D-7, it states to install an 8” sleeve through existing and new check dam, install 6” schedule 40 PVC and grout to create watertight seal.
These two sets of notes appear to be in conflict. Please advise.
Response See attached revised Sheet D-4, Note 19 which has been revised as follows:
“Install 6" Sch. 40 PVC pipe at downstream forebay.
See Detail "A" on Sheets. D-7 & D-8.”
1.05 Q&A Question
Upon looking at the bid schedule, and project plans I see a Check Dams, Planter walls, and Doweled Forebays in the D-4 through D-8 stormwater plans. Are there missing bid schedule items for this work and quantity? If not, would this cost be included in the catch basin schedule of value lines 36-39?
Response Per Specifications 00415.90, no separate measurements or payment shall be made.
“No separate measurement or payment shall be made for aggregates and/or geosynthetic liners and there testing and or retesting, check dams, forebays and splash pads.”

1.06 Q&A Question
Is there a bid item for valley gutters or is it incidental to anything else?
Response The valley gutter is included in the cost of the driveway construction. See Specifications 00750.80 (b).
1.07 Q&A Question
The Bidder’s Checklist indicates that Federally Funded Project forms are required to be submitted with the bid. However, I’m not seeing any forms within the Bid Documents that appear to correspond to this requirement.
Could you please confirm which Federally Funded forms, if any, need to be included with the bid?
Response See revised Notice to Bidders.
1.08 Q&A Question
Do we email you the first-tier form? The Bid Documents include ODOTs standard form, and the instructions are to email ODOT but elsewhere it reads to email the Buyer.
Response The First-Tier form can be included with the bid form or email to the Buyer by 4 P.M. It should not be emailed to ODOT.
