About this file

This Addendum No. 1 is issued by the Michigan Department of Technology, Management and Budget on behalf of the Department of Environmental Quality for a remediation project at two sites: 355 Cottage Grove SE and 1515 Madison Avenue SE in Grand Rapids, Michigan. The addendum clarifies requirements and answers bidder questions regarding excavation and backfill work, including that backfill compaction testing will be the contractor's responsibility, work is anticipated to be completed in August/early September 2018, and the contractor should plan working hours of 7:30 AM to 5:00 PM with gate access managed through a lock box with provided access codes. The addendum addresses site access procedures, including contractor coordination with the professional and site owner regarding entry and exit routes, with preference for using the Madison Avenue gate to minimize disturbance to Cottage Grove pavements; the contractor must provide the professional with a proposed schedule and estimated daily truck traffic if the Cottage Grove gate must be used.

The addendum clarifies that groundwater present at depths of 22 to 24 feet below ground surface is not anticipated to be encountered during excavation, and if encountered, excavation will cease at that depth with no dewatering requested. A critical correction identifies that aggregate quantity specifications contain a typo: Page 01-3 should state 105 tons rather than 900 tons to align with the bid sheet. The bid opening date is August 1, 2018, and acknowledgement of this addendum in the Bid Summary Form is mandatory for submission of a valid bid. No additional utilities beyond the shown storm sewer are known to exist in the work area.

DTMB-0414 (R 1/15)

STATE OF MICHIGAN Date Issued: July 27, 2018 Department of Technology, Management and Budget Index No(s): N/A State Facilities Administration File No: 761/18238.SAR 3111 W. St. Joseph Street Department: MDEQ Lansing, Michigan 48917 Project Name: Cottage Grove and

Madison

Bid Opening Date: August 1, 2018

ADDENDUM NO. 1

TO: All Bidders

SUBJECT: Cottage Grove and Madison, 355 Cottage Grove SE and 1515 Madison Avenue SE, Grand Rapids, Michigan

INTENT: This Addendum is issued to respond to the following questions received from potential bidders:

Q: Clarify that the backfill QC/compaction testing is to be under the CONTRACTOR, not the PROFESSIONAL. There is at least one contradiction in the bid documents and bidders were unclear on this.

R: Backfill compaction testing will be under the CONTRACTOR.

Q: When is work anticipated to be conducted?

R: The work is anticipated to be completed in August/early September 2018.

Q: Any other known utilities in the work area besides the storm sewer shown on the drawings?

R: No additional utilities are known at this time.

Q: What hours can the CONTRACTOR work? What hours are the fences going to be open for the CONTRACTOR and do they need to coordinate with anyone to get the fence opened, or be responsible for locking it up?

R: The CONTRACTOR should plan on working hours of 7:30AM to 5PM. The gates will be open during these working hours. The CONTRACTOR will provide the PROFESSIONAL with their proposed schedule of operation and the PROFESSIONAL will provide this information to the site owner and site operator. It should be noted that the gate has a lock box and the selected CONTRACTOR will be provided with the access code in case the gate is locked when they need to access the site. The CONTRACTOR will be responsible for verifying that the gate is locked if they are accessing the site outside of the listed working hours. Following completion of the work and demobilization of the CONTRACTOR’S equipment from the site, the PROFESSIONAL will verify that the gates are locked, as appropriate.

Q: Can the CONTRACTOR come in unloaded through Cottage Grove and leave through the northern, Madison Ave. gate when exporting soil, and come in through Madison and leave empty through Cottage Grove when importing backfill? This would help minimize potential disturbance to 355 Cottage Grove pavements.

R: The site owner and operator would like to minimize the use of the Cottage Grove gate. If possible and there is a sufficient turn radius, the entry and exiting should be done through the Madison Ave. gate. If the CONTRACTOR finds that entry and/or exiting must be done through the Cottage Grove gate, the CONTRACTOR will provide the PROFESSIONAL with an estimate of the daily truck traffic. The PROFESSIONAL will provide this information to the site owner and site operator. The CONTRACTOR and PROFESSIONAL will agree upon the route prior to commencement of the work.

Q: Clarify that if the CONTRACTOR hits water/groundwater, there will be no dewatering requested.

R: Groundwater is present at depths ranging from 22 feet to 24 feet below ground surface. It is not anticipated that groundwater will be encountered during this excavation. If groundwater is encountered, the excavtion will be stopped at that depth and no dewatering will be requested.

Q: Page 01-3 Item 6 Says aggregate 900 tons, and bid sheet says 105 tons. Clarify that 900 tons is a typo, and that it should say 105 tons.

R: The 900 tons is a typo. Page 01-3 should indicate 105 tons of aggregate to match the bid sheet.

ACKNOWLEDGEMENT: This Addendum must be acknowledged by the bidder in the space provided in the Bid Summary Form submitted with the bidder's bid for submission of a valid bid. The changes and information shall become part of the contract documents. (Authority: 1984 PA 431)
